The treatment of ischaemic stroke (IS) in pregnancy and the puerperium is difficult and requires the attending physicians to take quick decisions. Postpartum vasculopathy (PV) may be one of the causes of postpartum stroke. The aetiology of PV is not fully understood. It most often occurs in the first week after delivery. The trigger factors include vasoconstrictive substances that are present in the blood of women in the postpartum period. Most frequently, this condition occurs in a subsequent pregnancy, and the primary symptom is a very severe headache. The characteristic feature of PV is the angiographic image of the cerebral vessels, where segmental vasoconstriction and tortuous course of the vessels are visible, which resembles beads impaled on a string. This disease is a monophasic condition and, once the disease has progressed, there are no contraindications for a subsequent pregnancy. No standard management has been established, but most authors agree that acute phase treatment includes cessation of vasoconstrictors using calcium channel antagonists, magnesium intravenously, corticosteroids, and treatment of associated conditions like headache, high blood pressure, cerebral oedema, seizure. It also applies ASA, heparin when it’s needed, we present a women with an IS in the course of PV treated successfully.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"2609 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126648765","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This worldwide societal tragedy has dramatic repercussions on the physical and psychological health and social well-being of victims. The objective of this study was to determine the epidemiological and clinical profile and to evaluate the management of sexual violence at Hospital Principal Dakar. Patients and methods: This was a retrospective study of all cases of alleged sexual assault received at the maternity ward of the Hôpital Principal de Dakar during the period from 1 January 2018 to 15 April 2022, i.e. 4 years. Results: During this period, 57 assault cases out of 77 were collated which represent 75% of medico-legal cases and 0.35% of admissions. The patients were immediately referred with a judicial requisition (91.2%) and were accompanied by their relatives in 90.7% of cases. The average consultation time after the attack was 17 days, with extremes ranging from 5 hours to 3 years. The epidemiological profile of the victim was that of an adolescent girl aged on average 15 years, a student (75%), nulligest (96.4%) and living in an urban area (51.9%). The sexual assault took place during the day (82.6%), at the victim's home (15.7%) or that of the alleged perpetrator (35.3%), and was perpetrated by an adult (81.6%) belonging to the victim's entourage in 78.6% of cases (parents, spiritual guide, teacher, driver, roommate, family friend). The mode of sexual abuse was genital contact in 62.3% of cases. The examination most often revealed hymenal trauma (78.5%), three quarters of which were old lesions, with no sign of physical violence in 91.1%. In almost all of the presumed victims, the psychological impact was not assessed on admission. Five pregnancies were recorded; emergency contraception was prescribed for 23.1% of patients. An infectious disease assessment, antibiotic prophylaxis, ARV prophylaxis and a psychological consultation were prescribed in 90%, 85.7%, 78.5% and 54.9% of cases respectively. Only 14.8% of the victims had been followed up. Conclusion: Sexual abuse, the scourge of our time, is a disturbing reality. The real prevalence of this growing phenomenon is poorly known because of the many taboos and patriarchy. Optimal care for victims inevitably involves raising awareness among the population in order to break the silence, setting up specialised structures and collaboration between the various actors to condemn these crimes and, above all, to provide medical and psychological assistance and safe social reintegration for victims.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133865002","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: an observational ambidirectional cohort study was conducted at a tertiary care university hospital. A total of 666 women with a Bishop score ≤6 requiring labor induction were included. A cohort of 418 patients with double balloon catheter were compared with a retrospective cohort of 248 women induced with prostaglandins, either misoprostol or dinoprostone. Perinatal outcomes were compared between groups, with a p<0.05 considered statistically significant. Results: The cesarean-section rate was 27% in the double balloon group, compared with 27.8% in the prostaglandin group (p=0.352). The mean induction time-to-delivery was longer in the double balloon group as compared to the prostaglandin group (26.6±9.3 hrs vs 19.1±8.6 hrs, p<0.001). Hyperdynamia and umbilical-cord arterial pH<7.1 rates were significantly lower with the mechanical method compared to prostaglandins (p<0.001 and p=0.043, respectively). Other maternal and neonatal outcomes were similar between the groups. Conclusions: Double balloon catheter has similar effectiveness in terms of cesarean-section rate compared to prostaglandins, but with a better safety profile. However, the balloon catheter is associated with a longer time-to-delivery compared to prostaglandins.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"45 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132274418","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Cervical cancer screening includes cytology and tests for oncogenic subtypes of human papillomavirus (HPV), impacting on a significant decrease in mortality. In patients with a diagnosis of high-grade squamous intraepithelial lesion (HG SIL), the recommended management is excisional treatment (ET). The higher rate of persistent disease is associated with a positive margin status, larger lesion size, endocervical involvement and HPV positive test six months after treatment, especially HPV 16 subtype. Objective: To identify clinical and/or pathological features capable of predicting the compromise of the margins in the excisional treatment specimens. Methods: It is an observational, retrospective study carried out in Sanatorio Güemes between September 2017 and June 2020. All those patients with HG SIL who underwent excisional treatment were analyzed. The variables considered were the pre-treatment status of the endocervical canal, the size of the specimen (height), the type of transformation zone, the presence of greater pathology in the definitive specimen, the delay in consultation and the smoking habit. The relative risk of presenting compromised margins for each of the variables was analyzed. Findings: Our study shows 124 patients who received excisional treatment for HG SIL, 32 had compromised endocervical margins of the resection specimen. The delay in consultation between the biopsy and the ET has a significant impact on the state of the margins (RR 2,26; IC95 1,27-4; p 0,009). The presence of greater pathology (microinvasive carcinoma or higher stage) in the group with compromised margins (34,4% vs. 5,4%) highlights the importance of a comprehensive pre-treatment advisement to the patient concerning the diagnostic role of the ET in our population.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"23 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124084328","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, in presence of any endometrial anatomical abnormality or disease, implantation is affected. Uterine or endometrial polyps are over growths of endometrial cells that extend into the uterine cavity. They are usually noncancerous. There can be one or many polyps. They range in size from a few millimeters to several centimeters or larger. They may cause vaginal bleeding in between periods, heavy periods, frequent or unpredictable periods and infertility. The objective of this study was to find out effects of endometrial polyps on embryo implantation in assisted reproductive technology cycles of Saudi women being treated at an infertility clinic. This was a case-control study. The participants were in ratio of 5:1 i.e., for every 5 patients there was one control. The polyp diagnosis was carried out with ultrasound and endometrial biopsy. We observed high incidence of implantation failure in patients with uterine polyps as compared to that in the control patients. The results of this study indicated that the presence of endometrial polyps is a risk factor for infertility and failure of conception after in vitro fertilization.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-04-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130707794","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is a slow-growing disease, with a tendency to worsen. We are reporting a rare case of primary sclerosing cholangitis revealed during pregnancy. Case presentation: A 30-year-old woman who had pruritus during 12 weeks of gestation with a disturbance of the liver biologic tests. Imaging investigations found a chronic liver with areas of stenosis and dilation of VBIH and right and left hepatic bile ducts suggestive of primary sclerosing cholangitis with evidence of portal hypertension. She was put under treatment by ursodeoxycholic acid and the pregnancy proceeded without maternal-fetal complications giving birth to a healthy newborn with uneventful postpartum follow-up. A palpable mass of the nipple associated with nipple discharge and erosion or ulceration is the common clinical presentation.1 This rare benign tumor can be mistaken for breast carcinoma, Paget's disease, or galactophoric ectasia.2 This case report describes a 35-year-old woman with a painful nipple lesion and serous discharge. Clinical examination revealed a hypertrophied left nipple with a well-defined, firm, bluish nodule. Biopsy confirmed the diagnosis of nipple adenoma. The patient underwent surgical excision and reconstruction of the nipple. Histological examination revealed simple florid ductal hyperplasia and no malignancy. Erosive nipple adenomatosis is a rare condition that should be considered in the presence of nipple erosion or tumor. Diagnosis is anatomo-pathological, and treatment is usually surgical. These diseases are rare and most often have a good prognosis. Management recommendations are based mainly on expert opinions, such as those published in 2020 by the European Organisation for the Treatment of Trophoblastic Disease (EOTTD). They have a common name, but their origins, clinical characteristics and treatment differ. Hydatidiform moles correspond to villi with an excess of paternal genetic material and having a malignant potential, higher for complete Hydatidiform moles than for partial Hydatidiform moles Invasive mole is responsible for most cases of localized gestational trophoblastic neoplasia, occurring during pregnancy, within a variable period, but in most cases it is diagnosed after a molar pregnancy. Histological proof is sometimes difficult on curettage product, and must then take into account imaging and serum hCG levels. The overall cure rate is nearly 100% in low-risk patients and 90% in high-risk patients, since the advent of chemotherapy. In rare cases, the molar tissue crosses the thickness of the myometrium, the serosa and leads to hemoperitoneum associated with an acute abdomen as well as metastases, which is infrequently encountered in the invasive mole. The best course of treatment is chemotherapy (depending on the stage and the score with mono or poly chemotherapy) and in patients whose fertility is not in question, a hysterectomy can be performed. On the occasion of a clinical case, we will expose a dissecting form with an ovarian metastasis of an invasive mole. The treatment was successful.
