Exotaxins secreted by Clostridium \u0000bolulinum bacteria are heat-labile and irreversibly block the release of acetylcholine at the cholinergic ends of presynaptic and autonomic nerves at the neuromuscular junction after ingestion with food. The incubation period of botulismus is between 18-38 hours, and the first finding is blurred vision and diplopia symptoms due to the involvement of the eye muscles. The most important point in its treatment is early diagnosis and rapid administration of antitoxin. Although it appears earlier in patients with impaired blood sugar regulation, neuropathic symptoms are seen in almost half of diabetic patients who generally exceed 10 years. With early diagnosis and treatment, the quality of life of patients increases significantly. {"title":"Determining the clarithromycin-resistance of helicobacter pylori in first-line therapy with melting curve analysis","authors":"Kemalettin Yılmaz, A. Uygun, A. Kubar","doi":"10.51271/icjim-0007","DOIUrl":"https://doi.org/10.51271/icjim-0007","url":null,"abstract":"Aims: We aimed to determine the usefulness of melting curve analysis with the PCR technique in predicting the primary\u0000clarithromycin resistance of H. pylori before antibiotic use.\u0000Methods: 46 H. pylori positive clarithromycin based therapy naïve patients were included in the study. After proving H. pylori positivity with a real-time test, histopathological examination and rapid urease test, patients were treated with clarithromycinbased triple therapy for 14 days. Then patients were underwent a control endoscopy and tissue samples were taken 8 weeks after the initial endoscopy. The presence of H. pylori was investigated by using TaqMan-based real-time PCR and clarithromycin resistance was determined using two different molecular methods:melting curve analysis and DNA sequencing.\u0000Results: In total 18 patients (39.1%) were diagnosed as H. pylori positive on histopathological examination and 26 (56.5%) were diagnosed as H. pylori positive with TaqMan-based PCR, after triple therapy. According to results of melting curve analysis, primary resistance was found in 4 samples before treatment and secondary resistance was found in a different 4 samples after treatment. Previous studies have reported that\u0000central obesity associated with visceral steatosis is an important problem in digestive system diseases, especially reflux-related diseases. This study aimed to examine the effect of body fat distribution on the development of erosive esophagitis.\u0000Methods: A total of 131 individuals, 105 patients and 26 healthy volunteers, were included in this study. The FSSG questionnaire was applied to all individuals. Patients with an FSSG questionnaire score of 10 or more underwent upper gastrointestinal tract endoscopy. Patients with esophagitis on endoscopy were included in the erosive esophagitis group, whereas patients without esophagitis were included in the non-erosive group. Serum biochemistry (fasting glucose, insulin, lipid panel, uric acid, TSH, AST, ALT) analyzes were performed. Waist circumference was measured. Body compositions were evaluated using the bioelectrical impedance method (Tanita).\u0000Results: Erosive esophagitis was found in 68 of 105 patients enrolled in the study, and non-erosive esophagitis in 37. There\u0000was no statistically significant difference in age, BMI, and waist circumference between the erosive, non-erosive and control\u0000groups. The visceral fat percentage was higher in the erosive esophagitis group than the other groups (p <0.001). At the end of the pairwise comparison of the groups, it was found that visceral fat value was higher in the erosive group than the non-erosive and control groups, while the visceral fat value was similar in the control and non-erosive groups. {"title":"Relationship between thyroid function tests and bioelectrical impedance measurement parameters according to degree of obesity","authors":"VE Ürün, K. Üreten","doi":"10.51271/icjim-0008","DOIUrl":"https://doi.org/10.51271/icjim-0008","url":null,"abstract":"Aims: This study aimed to investigate the relationship between thyroid function tests and bioelectrical impedance measurement parameters showing body composition in groups of patients with varying degrees of obesity as determined by body mass index (BMI).\u0000Methods: A total of 120 people between the ages of 18 and 65 years were enrolled in the study, including 20 individuals with stage 1 obesity, 20 with Stage 2 obesity, 20 with Stage 3 obesity, and 40 with normal body weights. In the collection of data, a case report form prepared by the researcher for gathering information about the participants and the results of blood tests and bioelectrical impedance measurements made with a Tanita device were used. Blood samples were taken from the antecubital vein between 09:00 and 10:00 in the morning after at least 8 hours of fasting. HOMA-IR values were calculated by measuring fasting plasma glucose and insulin.\u0000Results: No significant correlation was found between median BMI and TSH, fT3 and fT4 levels in the whole population.\u0000While a negative correlation was found between median TSH and BMI in the group with normal body weight (r=-0.430,\u0000p=0.006), a positive correlation was found in the Stage 1 obese group (r=0.553, p=0.011). This relationship was not significant in the Stage 2 and morbidly obese groups. There was no significant correlation between BMI and fT4 and fT3 in all groups. A positive correlation was found between fat percentage and TSH (r=0.391, p=0.014) and fT3 (r=0.333, p=0.038) levels in the group with normal body weight, while no correlation was found with T4 levels.\u0000Conclusion: No statistically significant differenceswere found in terms of thyroid functions in different obesity classes as\u0000determined by BMI. This change can be determined by looking at the thiol/disulphide balance which is a biochemical test. We aim to emphasize the importance of oxidative stress due to obesity. Thus, by protecting obese individuals from oxidative stress (with antioxidant support treatments, etc.), studies to minimize the impact of obesity can be opened the front. Methods: The study group of our study was selected from 18-55-year-old female obese or overweight female patients who applied to Kırıkkale University Medical Faculty Internal Medicine and Endocrinology (Obesity) Polyclinics for slimming. Acontrol group consisting of healthy and normal weighted women who were in the same age range and applied to our hospital for general control were included in the study. According to BMI; groups consisting of 45 women, normal weight (18,5-24,9 kg/m²), overweight (25-29,9 kg/m²), stage 1 (mild obese; 30-34,9 kg/m²), stage 2 (moderately obese; 35-39,9 kg/m²) and stage 3 (morbid obese; 40-49,9 kg/m²), were formed. Participants were premenopausal women with no additional disease. Demographic data and routine investigations were obtained from the hospital system. Blood samples were studied with thiol/disulphide balance measurement tests. Results: We analyzed the five groups that were graded as normal weight, overweight, obese (mild, moderate, morbid) and the positive correlation between BMI and BC and BFP was significant in all groups (r =0.936, p<0.001; r =0.857, p<0.001, respectively). In all groups, the levels of native thiol and total thiol were decreased as BMI increased. There is a relatively lower difference between the normal thiol and total thiol levels of the normal weight and overweight group; there was a significant decrease in these values when passing from overweight to any stage of obesity. Negative correlation between all groups with native thiol (r =-0.473, p<0.001), total thiol (r =-0.472, p<0.001) and SH/total SH values (r =-0.296, p<0.001) were significant. The positive correlation between SS/SH (r = 0.296, p<0.001) and SS/total SH (r =0.296, p<0.001) was significant in all groups. The positive correlation between disulphide and all groups (r =0.103, p=0.25) was not significant. Conclusion: Increased fat tissue inflammation in obesity is associated with oxidative stress. Thiol-containing organic compounds are antioxidants for defense against oxidative stress. The presence of RAS mutation is also associated with poor prognosis in colon cancers and plays an important role in the choice of treatment. Platelet indices, which have recently been evaluated with an easy-to-reach method, have been discussed as an inflammatory marker in many studies. In this study, it was aimed to investigate the relationship between platelet distribution width (PDW), colon cancer stage and the presence of RAS mutation. Methods: File records of 132 patients who were followed up with the diagnosis of colon cancer in Kırıkkale University Faculty of Medicine, Department of Internal Medicine and Medical Oncology Department between January 1, 2015 and January 1, 2021 were retrospectively analyzed. Patients with RAS mutations and who did not receive chemotherapy were included in the study. Patients with and without RAS mutation were divided into two groups and their platelet index (especially PDW) values were compared. PDW values were compared according to the stages of the patients. The correlation between stage and PDW was examined. Results: Of the 132 patients included in the study, 82 (62.1%) were male and 50 (37.9%) were female. Participants; 12 (9.1%) stage 1, 23 (17.4%) stage 2, 29 (22%) stage 3, 68 (51.5%) stage 4 colon cancer patients. There were no RAS mutations in 73 (55.3%)patients, and 59 (44.7%) patients had RAS mutations. There was no significant difference in PDW between the two groups with negative and positive RAS mutations (p=0.826). According to the stages; PDW values were significantly different between the four stages (X2=9.878, p=0.020). Conclusion: In the study, PDW increased as the stage increased, but there was no significant relationship between RAS mutation and PDW. This was attributed to the fact that as the stage increases, the level of inflammation may be associated with an increase.
