This system a) collects field failure data, b) tracks/drives corrective actions, c) pulls the lessons learned from the corrective actions, d) places those lessons learned in the proactive analysis tools used in product development, e) drives product development to use the proactive analysis tools, f) keeps a library of proactive analyses to act as guides for future development, g) provides the proactive analysis for direct use by a root cause team driving to analyze and correct a field failure h) tracks and drives all activities - allowing instant summary \"dashboards\" or \"scorecards\" to be created. proactive and reactive records are stored coincidently in a single database system -- allowing real-time feedback. Reactive case resolution teams can tap into the development analyses or development teams can incorporate all relevant lessons learned from reactive case resolutions. All tools are attached to the database - to drive consistent usage and facilitate the integration of analysis libraries. All activities are mapped to a business process with pre-specified milestones and tracked to target dates - making this a real-time product reliability and safety management tool. Each record instantly becomes an ISO 9001 controlled quality record upon closure. A record tree structure allows cases/records to follow the configuration management of the product. The culmination of these features allows the user community to achieve product safety and reliability business management system over the full life cycle of a product. After two years of implementation and continuous improvement, a fundamental truth is proven. During the concept and prototype design, there are various system reliability and safety issues need to be addressed: capturing of customer's voice on system reliability and safety, comparing reliability and safety of this novel design with the current design (local protection system) in market to ensure that the new design has better market value, and obtaining a well established system model to be used by GE business in real product design, manufacturing, marketing and service. The effectiveness of prioritization can be significantly improved by using a simple graphical tool, as described by the authors. Evaluation of the adequacy of correction actions proposed to improve product/process/service, and the prioritization of these actions, can be supported by implementing the procedure proposed here, which is based on the evaluation of correction action feasibility. Differences in the interpretation of these metrics by designers, project managers and product support engineers within an organization may compromise the effectiveness of any technical or commercial decision-making that is based upon reliability prediction and field return data analysis. The same concern is true for customer and supplier interfaces with the organization, where reliability requirements and contractual guarantees, such as spares provision, are defined. The main issues appear to be related to an inadequate understanding of reliability requirement objectives. The process of negotiating and agreeing to requirements between a supplier and customer appears to be flawed in many cases by a lack of knowledge regarding the assumptions associated with the chosen metric. Holistic approaches to reliability such as REMM (reliability enhancement methodology & modeling) can provide alternative metrics from those traditionally used in the engineering community. This paper outlines the metrics available from such techniques and compares them with the more commonly used engineering metrics such as removal rate, mean time between failure (MTBF) and mean time between removal (MTBR). Recent innovation within the industry has led to the consideration of structured reliability centered maintenance techniques to improve maintenance and site performance. This paper presents the application of reliability centered maintenance to high speed, centrifugal dairy separation equipment at two New Zealand dairy sites. A pragmatic, practical approach is demonstrated, and the methodology employed in improving the equipment maintenance is fully described. The paper discusses the limitations of the previous maintenance strategy, and how the reliability centred approach reconciles these limitations in determining new, optimum maintenance activities. The importance of having meaningful, complete data in such an analysis procedure is discussed and the various data sources used in the analysis are presented. The role of the many stakeholders in the optimisation process is also reviewed and it is concluded that the success of the process depends largely on the input, belief and commitment of those stakeholders. Very few companies, however, fully realize all of the possible benefits of a FRACAS process, such as improving quality and productivity while reducing costs. Although many issues may prevent an effective implementation, there are three areas of concern that may cause negative impacts irrespective of the best-intentioned technology. In particular, complex organization interaction, inefficient and ineffective data tracking, and a lack of prioritized goals prevent the dramatic results that can be achieved with a FRACAS. In the MDA approach system requirements are specified in a formal language, most often the unified modeling language (UML), to form a platform independent model (PIM) of the system. A translation tool based on technology design patterns, transforms these specifications into a platform specific model (PSM) defined for the specific technology on which the system will be based. Then, using implementation design patterns, the PSM is transformed into the code model, which implements the system. Thus the system code is developed directly and automatically from the system requirements specification. The MDA approach was developed to enhance productivity and shorten the development cycle but it also gives significant improvements in software reliability. Wind energy resources are being increasingly recognized as cost effective generation sources in bulk power generation. In the modern age, we need failure-free electric supply. This leads to reliable supply, since most of the failures occur due to insufficient generation. The generation system reliability evaluation has become the most important component of power system reliability. This paper presents the results of a case study concerning reliability evaluation of Ramgiri wind farm in Andhra Pradesh, India. It consists of several 250 KW wind electric generators connected to the grid. The unit-wise data was collected for 2 years. The failure and repair rates were presented. \" We developed and used a method of sequential addition algorithm through a computer program in 'C' language\". We describe two Markov architectural configurations of the system, and the comparison based on the proposed safety-related metrics is demonstrated. The comparison results confirm and conclude that one of the two architectures performs safer than the other. Thus, we show that is possible to estimate the reliability of a product during its development with a small number of prototypes using accelerated life testing with the ability to repair when a failure occurs. This method allows us to improve the accuracy in the estimation of reliability parameters where the accuracy is linked to the number of failure times that are available.
