The article is devoted to the study of the organization of preferential drug provi- sion for outpatient patients with coronavirus infection in the Republic of Tatarstan and the analysis of the range of medicines dispensed under preferential prescrip- tions in pharmacies in Kazan. Methods. The initial data of the study were regulatory legal acts of the federal level, as well as those developed and approved by the Government and the Ministry of Health of the Republic of Tatarstan. The objects in this study were the primary accounting documents for the release of medicines under preferential prescriptions of large pharmacies of the state unitary enterprise «Medical Equipment and Pharmacy of Tatarstan» in Kazan – pharma- cies No. 1, 2, 3, 4 (conditional numbering) located in different administrative dis- tricts. Statistical and analytical analysis methods using Microsoft Excel were used for data processing. Results. The paper examines the interaction of structures and orders regulating the proce- dure for providing outpatient medical care, preferential drug provision to citizens. The analysis of medicines released under preferential prescriptions for 4 large pharmacies in Kazan was carried out. The most frequently prescribed medications by attached polyclinics in the period 2020-2022 were studied.; the structure of medicines purchased and released to pharmacies during the specified period ac- cording to the criteria: source of financing (federal, regional budget), as well as the organization of the competitive procurement system by source of financing, indi- cating the authorized representative for the organization of the competition (the organizers were the Ministry of Health of the Republic of Tatarstan, the Ministry of Health of the Russian Federation, which purchased medicines at the expense of the federal budget budget, and the purchase of medicines from the regional bud- get was carried out by the Ministry of Health of the Republic of Tatarstan). The total number of prescriptions dispensed, the amount and the average cost of 1 prescrip- tion were selectively analyzed. Conclusion. The study showed that in a short time the country has done a lot of work to overcome the pandemic of coronavirus infection: Temporary methodological rec- ommendations of the Ministry of Health of the Russian Federation “Prevention, diagnosis and treatment of new coronavirus infection (COVID-19)” have been de- veloped, as well as an algorithm for preferential drug provision for outpatient pa- tients with coronavirus infection and interaction with medical organizations that timely, in the course of changing the situation with coronavirus infection, they were rebuilt to solve the task. {"title":"THE SYSTEM OF PREFERENTIAL PROVISION OF MEDICINES IN SMOLENSK REGION: MODERN FEATURES AND PROBLEMS","authors":"N.A. Methods. The preferential provision of medicines was carried out by the economic and sta- tistical analysis of data from Smolensk department of health and completed by an assessment of infrastructure availability of medicinal support. Results. There was an increase financing for program of medical support 2.32 times over the past 7 years. The number of prescriptions increased equally the number of ben- eficiaries who did not refuse a set of social services (1.05 times). The rate for one pa- tient was stable and tended to increase. The actual costs for one person increased (2.47 times) and exceeded state’s standards. The number of points of sale is grow- ing and leading to the increase of availability of medicines. There was a decrease of infrastructure burden with an increase of financial burden on 1 pharmacy. Conclusion. The growth financing with decrease in the number of beneficiaries, including those who retained the right of the preferential provision, with a decrease in the population, can be explained by an increase in costs for each citizen per month. This fact may indicate that there are patients with chronic diseases in the system. The decrease in those who refuse drugaccess program allows to think about insig- nificant returning of citizens to the system. However, a sufficient number of those who replaces medicines into cash, the discrepancy between the standard and the actual costs of treatment for one person indicate problems associated with incom- plete coverage of the need of medicines and require to revise approaches of system in region. The level of the trade margin on the goods amounted to 20.96% in 2021, and already 18.68% in 2022., there is a decrease in the margin on medical devices, suture material and specialized nutrition. Materials and research methods. Occupational standards that determine the responsibilities of specialists, the competencies of specialists for their implementation. The results of inspections of medical and pharmaceutical organizations by regulatory and supervisory authori- ties, the results of an expert review by insurance organizations of compliance with the procedure for providing medical care, clinical recommendations, standards of medical care, the validity of prescribing drugs to privileged categories of citizens, and the results of an internal audit of the organization. The following methods were used in the study: situational-logical, content analysis, structural, systemic, statistical and computer technologies. Results. The educational organizations that implement programs of additional profession- al education, as well as training centers of health authorities, professional asso- ciations of medical and pharmaceutical workers can carry out assessment of the level of possession of professional competencies of medical and pharmaceutical workers. Unfortunately, this variety of ways does not always allow obtaining a complete picture of the possession of professional competencies by medical and pharmaceutical workers. In 2021, the Ministry of Science and Higher Education of the Russian Federation granted the Tyumen Medical University the right to create a Federal Innovation Platform - The Center for Independent Assessment Of Medical And Pharmaceutical Employee Competence. The goal of the Center is to identify and eliminate the lack of professional competencies of medical and pharmaceu- tical workers. Conclusion. Methods. The study used the methods of logical, comparative, structural and content anal- ysis. Results. A set of proposals has been formulated to improve user interaction with the web- site of online pharmacies, including: an indication of the actual expiration date of available drugs, an indication of a public offer and a license, information that the site is not an aggregator, information on labeling, and opportunities: the choice of analogues of a drug with taking into account their cost, online consultations with a pharmaceutical / medical professional, feedback, the ability to assess the pharma- cological compatibility of purchased / or used drugs. Conclusion. The study formulated a set of proposals for the standardization of digital services of online pharmacies on the basis of regulations governing the activities of phar- macy organizations. A model for improving sites is proposed, which provides for the expansion of functionality and the improvement of the quality of pharmaceu- tical assistance in the online trade of drugs and other pharmacy products, reducing the time and simplifying the procedure for ordering and purchasing them. The ad- vantage provided by the given set of criteria is the possibility of obtaining online pharmaceutical advice on the selection, use and storage of medicinal products, the pharmacological compatibility of medicinal products, the possibility of choosing analogues of medicinal products, taking into account the price and expiration date, as well as the ability to verify the authenticity of the medicinal product by labeling. In case of severe pneumonia in hospital, infusion of Amoxicillin preparations is used. In the Russian pharmaceutical market during the study period, there are no solutions for parenteral administration in pediatric dosages, which leads to high financial losses of medical organization and environmental risks in the disposal of antibiotics. Objective of the study evaluation of losses of drug weight and financial losses when using «adult» doses of anti- bacterial agents for infusion therapy of community-acquired pneumonia in children (on the example of the drug «Amoxicillin+clavulanic acid, powder for preparation of solution for intravenous injection»). Materials and methods the objects of the study were clinical guidelines on treatment of community-acquired pneu- monia in children, information from the State Register of Medicines, instructions for medical use. The source of data on prices was the information from the State Register of maximum selling prices. Analytical, statistical, logical, and computational-constructive methods of re- search were used in the work. Results: the study calculated the material losses of the drug in mg per vial and 12 vials (treat- ment course of 4 days). With a single therapeutic dose of 300 mg, the material losses of the drug, using a dosage of 1200 mg, were: 1200 - 300 = 900 mg (75% of the dosage form). Fi- nancial losses in rubles calculated for the course of treatment with the drug Amoxicillin + clavulanic acid 1200 mg amounted to 756.00 rubles (79.05%) per 1 child, and for treatment with the dosage of 600 mg - 252.00 rubles (52.70% of the cost of the drug). The development of the dosage forms for intravenous administration of Amoxicillin + clavu- lanic acid in the doses of 180 and 240 mg would allow decreasing considerably the losses of the drug during preparation of infusion solutions in pediatric practice. Conclusion 1. When administering antibacterial therapy for community-acquired pneumonia in chil- dren, the drug Amoxicillin + clavulanic acid, powder for preparation of solution for intrave- nous injection, manufactured in doses of 1200 mg and 600 mg, is used. 2. Modeling of weight losses of Amoxicillin + clavulanic acid powder for intravenous solu- tion preparation for treatment of community-acquired pneumonia in children aged 1 year showed that when 1200 mg dosage was used, losses were 900 mg/ vial (10800 mg per treat- ment course), and in 600 mg dosage - 300 mg/ vial (3600 mg per course); 75 and 50% of drug weight respectively. 3. Calculations have been made and significant financial losses have been established in treatment of community-acquired pneumonia in children under 5 years old with the drug «Amoxicillin + clavulanic acid, powder for preparation of solution for intravenous injection» 1200 mg and 600 mg, due to incomplete use of the package. Goryachev","doi":"10.30809/solo.1.2023.1","DOIUrl":"https://doi.org/10.30809/solo.1.2023.1","url":null,"abstract":"Target Substantiation of the content and semantic meaning (semantics) of the complex concept (concept) «drug supply» based on the choice of the most appropriate term from a number of existing options and clarification of its definition as a priority interdisciplinary area of pharmacy. Methods The initial materials of the study were publications on the organizational problems of pharmacy for the period 1971-2020, selected by the method of non-repetitive random selection from specialized journals, materials of conferences, congresses, dissertations and collections of papers. The information array included 2634 works, including 596 (22.63%) publications on the problems of drug supply. The scientific and methodological apparatus of the study included the scientometric method used to count the number of publications, content analysis and terminology meth- ods: logical-semantic, lexical, classification, comparison, and others. Results The array of terms used by specialists to designate drug supply was formed on the basis of content analysis of publication titles. Terms were selected that contained the words «drug», «pharmaceutical», etc. in combination with the words «pro- viding», «supply», «help», etc. The terms «drug care» and «drug provision» were most actively used, their total frequencies amounted to 127 and 289 positions, respectively. A logical-semantic analysis of the concept of «drug supply» has been carried out. To distinguish between the terms under discussion, a comparison of the definitions of «drug care» and «pharmaceutical care» is given. The modern se- mantic content of the concept «drug supply» is substantiated, taking into account the signs of the genus and type of the structural model of the formed definition. The relationship between the concept of «drug supply» and the concepts of «phar- maceutical activity», «circulation of medicines» and «logistics» is analyzed. The author’s classification of the types of «drug supply» by type of consumer is present- ed. As a result, a modern version of the definition was formulated: drug supply is a logistics process in the drug circulation system aimed at bringing drugs from the manufacturer (supplier) to the end user.
