{"title":"Associations of Molecular-Genetic Bioenergetics and Chemotherapy-Induced Fatigue Symptoms in Patients with Breast Cancer: A proposal development","authors":"Chao-Pin Hsiao","doi":"10.23880/nhij-16000278","DOIUrl":"https://doi.org/10.23880/nhij-16000278","url":null,"abstract":"Introduction: Cancer-related fatigue (CRF) occurs in 82%-96% of cancer patients receiving chemotherapy (CT). CT-induced CRF is a distressing, persistent sense of exhaustion related to the disease or its treatment, and negatively impacts health outcomes. CRF is one of the most prevalent side effects of CT in patients with breast cancer. Despite various attempts to investigate the etiology of CRF, the biochemical mechanisms remain elusive. Objectives: The study aims to explore the molecular-genetic pathway of mitochondrial bioenergetics and its association with CT-induced CRF. We hypothesized that the chemotherapeutic agent containing anthracycline targets cell cycle progression, which triggers genetic and cellular instability, altering expression of mitochondrial genes and proteins, inducing reduced electron transport chain enzymatic activity and impaired oxidative phosphorylation, resulting in adenosine triphosphate (ATP) depletion and excessive reactive oxygen species (ROS) generation, leading to the development and intensification of CRF. Methods: This is a prospective, hypothesis-testing, and longitudinal study design. A total of 60 patients with breast cancer undergoing CT will be enrolled. Validated instruments will be used to measure CRF, depression, sleep disturbance, and physical activity. Whole blood sample will be collected before, during, and at the completion of CT to determine profiles of mitochondrial bioenergetics. A linear mixed model repeated measures analysis will be used to examine associations between changes in study variables. Anticipated Results: Increased scores of CRF will be associated with altered mitochondria-related genes and mitochondrial bioenergetics (e.g., ↓oxidative phosphorylation, ↓electron transport chain complexes activity, ↓ATP content, and ↑ROS production) in patients receiving CT-containing anthracyclines. {"title":"Numeracy Skills for Undergraduate Nursing Students’ Clinical Skill Assessment: An Expository Analysis","authors":"S. Numeracy skills have been identified as one of the competency elements for outcome-based education in nursing. Studies have shown that undergraduate nursing students continue to perform poorly on clinically-related numeracy tasks, making errors that betray fundamental misconceptions about the underlying mathematics. These conditions can be eliminated when an effort is made and the effects can be rewarding for nursing students, nursing academics and as well as improving patient care. Aim: This study aimed to address the importance of numeracy skills for undergraduate nursing students’ clinical skill assessment. Method: This study used an expository analysis approach to address the issues of the importance of numeracy skills competency in the clinical skill assessment of undergraduate nursing students. We have analysed the current undergraduate nursing curriculum and clinical skill assessment components. Moreover, we also review the available literature related to numeracy skills competency for nursing students and newly registered nurses. Finding: Numeracy skills are one of the important elements of competencies skills that have been introduced to the current Malaysian undergraduate nursing programme. However, the achievement part of it is still questionable. Evidence has shown that new graduate nurses often lack the numeracy skills needed to enable them to do their jobs safely and effectively in the clinical setting. Among the errors done by the new graduate nurses and nursing students are drug calculation errors and which accounted for 30-40% in the clinical. Conclusion: Numeracy skills competency assessment is vital for undergraduate nursing students who have to make complex calculations and analyse the patient’s situation in their clinical setting. It is impossible to describe a defined set of customs and beliefs about the child rearing practices such as socio- economic status, education, as individual experiences vary from family to family Aims: The aim of the study was to assess and compare the knowledge and practice on child rearing practices among working and non-working mothers by self- Structured Questionnaire in selected community areas. Materials and Methods: An extensive review of literature was undertaken. Quantitative non-experimental cross sectional comparative research approach is used. The sample size consisted of 100 mothers (50 working and 50 non-working mothers) selected from the urban community areas of Mowa Raipur by using purposive sampling technique. The data were collected using a self-Structured Questionnaire and the Tool was organized in three parts. (Demographic variables, Knowledge and Practice questions on child rearing practices). The average time given for completion of the questions was 45 minutes in each mother. Results: The data was analyzed using the descriptive and inferential statistics where the result shows that the assessment of knowledge in working mothers 66% & In nonworking mothers 74% had average knowledge, In working 32% & In non- working mothers had 24% good knowledge, In working & non-working mothers both 2% had of excellent knowledge .The mean of knowledge among working mothers was 20.32 and mean % was 50.8%. And Mean of knowledge among non-working mothers was 19.62 and mean %was 49.05% Mean for overall of knowledge among working and non-working mothers was 19.97 and mean percent was 49.93%. The result shows that the assessment of practice in working mothers 72% & In non- working mothers 82% had average practice, In working 18% & In non-working mothers had 14% good practice, Correlation is computed by Karl Pearson coefficient of correlation (r = -0.66), there is moderately positive correlation between knowledge and practice, it shows that when knowledge increases, practice will also increases. All nations’ scientists are still trying to find a solution for it. Beating the virus completely seems to be difficult because of the nature of the virus. In West Bengal, India, schools reopened from February 12th 20218 for class IX-XII only. So school children up to class VIII are still confined at home. In this context, it is important to understand the effects of lockdown on children. Methods: Hundred school students were selected for online (Google form) survey during Oct-Nov 2020 through purposive sampling. After ethics committee approval school children between 6-18 years who were willing to participate were enrolled for the survey. Informed consent was taken from the parents and their children. Descriptive statistics were used for analysis of data. Results: 36% children had increased appetite whereas 12% reported a decrease in appetite during COVID-19 lockdown. Few (9%) students reported long sleeping hours (12-14 hours / day) and majority (61%) of the participants expressed spending their leisure time by watching TV. Most of them (89%) missed the school; 53% participants felt loneliness and 74% missed outdoor games. 60% children also reported the concepts to be unclear in online classes. 46% parents reported behavioral changes of their children like irritability (31%), getting angriness (36%) and temper tantrum (13%) during COVID-19 lockdown. Most (98%) of the school children are maintaining their personal hygiene but 82% children are not doing it in time. 99% had basic knowledge on COVID-19 and its preventive behaviors but 37% didn’t want to wear mask, 30% were not maintaining social (physical) distancing and 10% were not washing their hands regularly. Many participants expressed that their quality of life, quality of study, health and mental health condition deteriorated during the lockdown than the pre COVID situation. {"title":"Evaluation of Nurses Clinical Knowledge and Attitude Regarding Management of Critically Ill Patient in the Emergency Department","authors":"Y. Objectives: To evaluate nurses’ cognitive skill regarding management of critically ill patient in the emergency department. Methods: This was a descriptive cross sectional hospital-based study conducted at emergency department in Omdurman military hospital during the period between February 11 to March 4. 2022. Sixty nurses were participated in the study taken as total coverage. Results: Sixty five percent of participants were females and most of them had 1-5 years’ experience. Their average age was 32 years old. Ninety two percent of them holding bachelor degree. The overall participant’s knowledge for all the variables analyzed was 18 participants (30%) had poor knowledge, (18.3%) scored for Fair knowledge and 28 Nurses (46.6%) scored for good knowledge. Only 14 nurses (23.3%) attain very good results for the overall aspects of the knowledge variable. Also the participants were assessed for their attitude regarding management of critically ill patient in emergency department. Total mean of the attitude showed strongly disagree by (14.4), disagree (8.0), Neutral Agree (5.8), agree (16) and strongly agree (13.8). There was a significant correlation between participants’ level of knowledge and educational level, and between the knowledge and the training courses (P. value ≤ 0.01) respectively. Conclusion: According to the findings of the current study It was concluded that the study participants showed approximately forty six percent of them attained good knowledge, 23% attained very good knowledge and thirty percent of them gained poor Knowledge. The low attitude in the care areas needs qualified and full trained nurse. Due to certain socio-cultural beliefs, certain regions of Nigeria could be poorly aware of STDs and their consequences especially in the adolescents. Hence the need for assessment and health education on STDs in the adolescent population. Objectives: This study aims to assess the awareness levels of sexual transmission diseases among adolescents in selected public secondary schools in Sokoto State, Nigeria. Materials and methods: This was a cross-sectional and questionnaire-based survey on 346 randomly selected adolescent students aged 10 – 19 years from secondary schools. Using a pre-tested structured questionnaire, data on participants’ sociodemographic variables and responses related to their knowledge and awareness about STDs were collected. The responses were scored and analysed using Statistical Package for Social Sciences (SPSS) version 23.0. Results: Of all the participants, 306 (76.0%) were females, and most of the students, 220 (63.6%) indicated their peers as the major sources of information about STDs, whereas the least was via mass media, 30 (8.7%). Overall, 65.1% of the students do not discuss with their parents on adolescent reproductive health, 80.3% disagreed that reproductive health facility exists in our community/neighbouring community, and all (100.0%) agree that secondary school students need inclusion in sex education in their curriculum. Also, 70.0% do not believe in having a friend from the opposite sex. Over 65% do not know the major etiological agents of STDs. Conclusion: Based on these findings, the level of knowledge on STDs among adolescents was low. The secondary school students held a negative view of the existence of reproductive health care facilities in schools and communities. Childhood in itself is a sweet memory for each one of us; we all have our own bunch of memories to share. Childhood is even the most correct time to mould a raw person into a fine personality. Living in the 21 st century world, things have seemed to be changing a lot. Parenting, raising children has become a far more different aspect. Since parenting is also an ethical and moral issue, perspective may vary according to the culture and civilization for different people belonging to different set ups. As health personals we too come across many cases that arise merely due to maladjustments or psychological impairments that many a times lead to serious psychiatric problems. The third important thing is that parenting as a complete process and according to the changing time parenting is not the same as it used to be in the past. In the previous years we have been undergoing tremendous technological advancement which in terms is a boon for us. Our lifestyles have heavily changed, we no more live in a joined family, we all are working parents and our outlook for a settled life has changed the world around us. We all live in the world that is modern civilized. Leaving our civilization far behind. Raising kids in this advanced world is a challenge indeed. Since technology has brought curses as well as boons to our lives. We need to balance them both. The prevalence of child abuse globally is estimated up to 1 billion children aged between 2-17 years have experienced physical, sexual or emotional violence or neglect as per the statistics of 2020 by WHO. It is the responsibility of the caretakers to protect their child and be aware of all the legal aid which is available for children afflicted with child abuse. So the study was conducted with the objectives as follows: • To assess the knowledge of caretakers regarding legal aspects on child abuse. • To determine the association between knowledge regarding legal aspects of child abuse and selected demographic variables. A descriptive cross-sectional study was conducted among the caregivers of children below 15 years of age visiting paediatric OPD of SJMCH, Bangalore. A structured questionnaire was used to assess the knowledge regarding legal aspects of child abuse. The data was analysed by using descriptive and inferential statistics. In our study, only 4.2% of parents had good knowledge about the topic whereas 50.9% had moderate knowledge. It is a dependence syndrome seen among adolescents those who exhibit problematic behaviours. The main aim of this study was to assess the excess use of smart phone among the adolescents and to develop information pamphlet on ill effects of Smartphone among adolescents. The research approach selected for the study was descriptive survey approach and research design was descriptive design. The sample of present study comprised of adolescents (17- 18 years) in JSS Arts, commerce and science college of Mysuru. In the present study, 60 samples were selected through non probability convenience sampling technique. Data collection tool includes Personal variables proforma to collect the sample’s demographic profiles and Rating scale to assess the excess use of smart phone among adolescents (17-18 years). Findings of the study revealed that mean scores of excess use of Smartphone of adolescents was 40.7 with standard deviation of ±5.1 and the scores were ranged between 32-48.
