We treated it surgically with a good clinical and radiological evolution. In the U.S., tissue Plasminogen Activator (tPA) is the only approved thrombolytic drug to re-canalize occluded arteries in patients with acute ischemic stroke (AIS). With timely administration, tPA may improve the patient prognosis. The percentage of AIS patients that receive tPA in the U.S., however, varies from 3.0% to 8.5%. Further, there is scarce information on the impact of tPA and short-term mortality among Floridians hospitalized with AIS. This study investigates factors associated with in-hospital mortality among patients admitted to Florida hospitals with AIS who received tPA compared to those who did not receive the thrombolytic. Methods. This is a secondary analysis of the Florida Stroke Registry for 2008-2012. We assessed the association between tPA administration and in-hospital mortality utilizing logistic regression to estimate unadjusted and adjusted odds ratios. Pearson correlation coefficients were used to diagnose for collinearity. Results. A total of 133,052 ischemic stroke patients (51.9% women, average age 71.7±14.4 years) comprised our study sample. Approximately 5% (6,357) of AIS patients received tPA. After adjusting for potential confounders, AIS patients who received tPA were twice as likely to die than AIS patients not receiving tPA (OR=2.0; 95% CI= 1.8-2.2). AIS patients 90 years or older were five times more likely to die than AIS patient 6 days (OR=1.2, 95% CI= 1.1-1.2), and not being assigned emergent priority at admission (OR=2.7, 95% CI= 2.5-2.9). Conclusion. Our findings suggest that the likelihood of in-hospital mortality among Floridian patients with AIS is twice as higher among those receiving tPA than those AIS patients not receiving the thrombolytic. Since the mid-1970s, the total number of deaths related to AMI in the United States has not declined. Studies suggest that women with AMI have worse outcomes compared to men. However, there is limited information regarding this topic among Hispanics. This study was a secondary analysis of the Puerto Rican Heart Attack Study, which reviewed the records of Hispanic patients of Puerto Rico hospitalized for AMI at 21 academic and/or non-teaching hospitals in 2007, 2009 and 2011. This study set examined the differences in in-hospital mortality rates between genders. A p-value of 0.2 was used to select possible confounders and the chi-square test was used to examine associations between categorical variables. Factors associated with in-hospital mortality rates were identified using logistic regression. Collinearity was assessed using Pearson correlation coefficients. The 95% confidence interval and a p-value of 0.05 were used to determine statistical significance of odds ratios. Analysis was restricted to patients with ICD-9-CM code 410-414 who are above 18 (n = 2265). In our sample, there were more men than women (1291 versus 974, respectively). Men were younger and smoked more compared to women. Compared to men, women were older and suffered more comorbidities, such as stroke and congestive heart failure (CHF). Women had higher rates of in-hospital mortality compared to men (OR = 1.4, p = 0.040). Factors associated with higher rates of in-hospital mortality included age and CHF (p<0.001). Patients with CHF showed higher rates of in-hospital deaths compared to patients who did not have CHF (OR = 1.6, p = 0.026). Patients over the age of 86 showed higher odds of in-hospital death compared to younger patients (OR = 10.5, p <0.001) Significant disparities existed by gender in this sample of Hispanic AMI patients, with women showing higher in-hospital mortality compared to men. It may arise de-novo or it may present in association with acute myeloid leukemia. It can also occur in patients with myelodysplastic syndromes, myeloproliferative disorders, and as blast transformation in myeloproliferative neoplasia. Myeloid sarcoma of the head and neck area can pose a diagnostic challenge because of the low frequency of occurrence, and the vast diversity of tumours occurring from multiple lineages in this anatomic region. This can lead to a broad spectrum of various differential diagnoses. Therefore, a high index of suspicion is required when dealing with a possible case of myeloid sarcoma, as it is a time sensitive diagnosis with various diagnostic dilemmas. Here , we report an example, a case of tonsillar myeloid sarcoma. L’urbanisation, le developpement economique et la mondialisation sont a l’origine de modifications des habitudes alimentaires. Le regime alimentaire marocain, de type mediterraneen, est base sur une large consommation de cereales et de fruits et legumes. L’alimentation se diversifie progressivement, surtout pour les menages urbains et les classes plus aisees. Elle comprend davantage d’aliments riches en micronutriments, mais la consommation de produits d’origine animale reste tres limitee alors que les ressources du pays, en poisson notamment, sont tres importantes. Les aliments prets a consommer et la restauration hors domicile deviennent plus courants en milieu urbain favorisant la consommation d’aliments riches en sucre et en graisse. Cette evolution temoigne de la transition nutritionnelle en cours en milieu rural aussi bien qu’urbain. Elle touche environ 1% de la population mondiale. C’est une affection invalidante qui conduit plus ou moins rapidement a la destruction cartilagineuse et osseuse et, a long terme, en l’absence de traitement, a l’incapacite fonctionnelle et/ou au handicap severes. L’importance de son diagnostic et de son traitement precoces est majeure. Au Maroc, le retentissement socio-economique de la maladie est important. Les difficultes a l’acces au traitement des patients, les consequences sur leur vie personnelle et familiale ont ete mises en evidence. Parmi elles, les consequences negatives sur la scolarisation des petites filles sont significatives. De ce fait, la prevention et l’information sont majeures dans la prise en charge des patients de meme que le recours a un traitement a « un cout raisonnable ». Par ailleurs, la comparaison des donnees, grâce a l’inclusion dans des etudes internationales mettent en evidence la severite de la maladie, essentiellement en raison du retard au diagnostic, du retard a la prise en charge en rhumatologie, du bas niveau socio-economique. La mise en place d’une cohorte de polyarthrites recentes suivies sur une periode de deux ans, a mis en evidence l’importance d’un diagnostic et d’une prise en charge tres precoces et l’amelioration significative du pronostic de la maladie qui en resulte.
