Prevention of the diseases is the cornerstone of medicine. Preventing of the diseases improves the quality and life expectancy, and also reduces the costs for the diagnosis and treatment of diseases. The main preventive measures include maintaining a healthy lifestyle (proper nutrition and regular exercise) and vaccination.Purpose: to study the influence of distance learning programs on the knowledge of doctors about the vaccination.Materials and methods: a distance learning cycle of lectures was conducted on the vaccination of elderly patients. Before and after the course of lectures, an anonymous voluntary survey of the course participants was conducted. The questionnaires included questions for free completion, questions for assessing attitudes on a scale from 0 to 10 (where 0 strongly disagree and 10 strongly agree) and multiple choices questions. Physicians were informed before the survey that the data would be subsequently analyzed and published.Results: 556 people took part in the first stage of the survey, of which 469 (84.3%) were women. 328 people took part in the second stage of the survey, of which 271 (82.6%) were women. The experience of doctors was on average 27±11,8 years, specialists from 72 specialties took part. Before the course of the lecture, on average, the opinion of doctors on vaccination was 8.7±0.1 out of 10, after the course of the lecture it became 9.1±0.1 out of 10 (p < 0.05). Before the start of the course, on average, doctors assessed their knowledge at 6.9±0.1, after the end of the course 7.7±0.1 (p < 0.05). Patients require a second stage of medical rehabilitation — in a specialized rehabilitation unit. The article presents the technology of medical rehabilitation, which is used for this group of patients, including assessment of rehabilitation prognosis, application of physical rehabilitation methods, correction of somatic pathology and psychological status.Aim: to improve the immediate and long-term results of hip and knee endoprosthetics, spine stabilization surgeries by developing a system of rehabilitation measures based on individual patient parameters and aimed at optimal restoration of function.Materials and Methods. The focus of the investigation was on the patients from the Medical Rehabilitation Department, Clinical Hospital of the ISC SB of the RAS. Patients affected with coxarthrosis, gonarthritis, osteochondrosis of the spine, who had undergone joint replacement or stabilizing spinal surgery in the Medical Rehabilitation Department of Irkutsk Scientific Center of Surgery and Traumatology, 5–7 days before transfer to the Medical Rehabilitation Department of Clinical Hospital of the ISC SB of the RAS. The subjects also underwent clinical, paraclinical, locomotive, physical examination and laboratory test; physical activity assessment and estimation of indicators of the integral rehabilitation scale, the medical rehabilitation scale and the physical activity scale were performed as well. In the course of medical rehabilitation, patients received physiotherapeutic appointments, physiotherapy exercises, correction of somatic pathology, and correction of psychological status.Results. In 2020–2021, 240 older patients were treated in the Medical Rehabilitation Department after large joints arthroplasty and spinal reconstructive surgeries. The technology of medical rehabilitation was used for these patients. As a result, pain syndrome was eliminated, contracture of the operated joint was stopped, locomotor function improved, patients' physical activity and self-care ability were optimized, and somatic disorders were corrected. In addition, our own scales have been developed and improved: the Integral Rehabilitation Orthopedic Scale, the Individual Physical Activity Scale.Conclusion. Zhelvakova","doi":"10.37586/2686-8636-2-2022-106-114","DOIUrl":"https://doi.org/10.37586/2686-8636-2-2022-106-114","url":null,"abstract":"Introduction: prevention of falls and fractures in older adults is a strategic task of the geriatric service.Aim: to estimate the prevalence of falls and its risk factors, as well as their association with geriatric syndromes in St. Petersburg.Materials and methods: analysis of the results of four studies conducted in St. Petersburg between 2009 and 2019. The total number of participants was 1398 persons aged 60 years and older. Main parameters: comprehensive geriatric assessment, measurement of cardio-ankle vascular index (CAVI) using Vasera VS-1500 volumetric sphygmography computer complex, detection of silent atrial fibrillation using «MyDiagnostick 1001R» device.Results: every third person falls at the age of 65 to 74 years old, and every second person falls at the age of 85 and older. In 50% of cases, falls are accompanied by injuries. The main factors associated with a higher risk of falls were arterial hypertension, atrial fibrillation, stroke/transient ischemic attack, cognitive impairment, sensory deficits, urinary incontinence, decreased physical functioning and CAVI above predicted value. Vision correction reduced the risk of falls by 46.7%, improved/good nutrition status according to self-assessment data by 55.3%, correction of urinary incontinence by 55.8%, improved cognitive functioning by 73.3%, and eating two or more fruits and vegetables a day by 66.0%. Individualized analysis of fall risk factors and development of fall prevention programs can reduce the risk of falls by a factor of nine within the first year.Conclusion: Screening to assess the risk of falls and compiling individual fall prevention programs can significantly reduce the risk of falls in older and senile age. The effectiveness of individual fall risk reduction programs decreases after the first year of implementation. Minimally invasive surgery seems to be nowadays the procedure of choice in treating urolithiasis. Quite frequently, an access to calculus can be complicated by concomitant pathology. The article provides a clinical case of urolithiasis preoperative diagnosis with concomitant hepatomegaly in a 67-year-old patient. An example of an interdisciplinary approach to surgical procedure planning is given in this research. Multispiral computed tomography was applied using multi-planar reconstructions, cutaneous radiopaque markers, and body positioning. The article details current information about the age-related features of metabolism and muscle tissue remodeling, which lead to the development of sarcopenia and its further progression. In most cases, OAB does not pose a threat to the life of patients, but significantly worsens the quality of all spheres of human life. Urge urinary incontinence is one of the main causes of falls in older patients, which often leads to fractures of the proximal femur and is accompanied by severe complications. Drug therapy with M-cholinoblockers (cholinolytics) is the leading direction in the treatment of patients with OAB. However, this type of therapy remains poorly understood in older men and women. The choice of an effective and safe cholinolytic for older and senile patients is the subject of discussion and scientific research. The study of Waldenstrom's macroglobulinemia has great relevance due to the dim clinical picture and high prevalence among the older population. This condition is relatively rare, but its timely detection is very important to prevent the development of complications.
