The physical impact could endanger the teenage mothers and even their infants. Psychological stress could trigger a trial to have an abortion which can lead to death. Other psychological effect, the teenage mothers will try to escape from responsibility or keeping to continue their pregnancies by compulsion. Meanwhile, besides, social judgement and scorn, as well as banishing made by surrounding community, the community will also reject the children with unclear parents and they are considered illegal children of adulteries (Husaeni, 2009). Based on data from the Directorate of Maternal Health, the biggest cause of teenage maternal death is bleeding as a result of premature pregnancy. The purpose of this study was to determine the socio-cultural, economic of an unwanted pregnancy in single parent of adolescent mothers. This type of research is qualitative with a phenomenological and ethnographic approach. The population and sample are pregnant women and single parent mothers (having toddlers). The sample size is 9 people. Purposive Sampling Data collection by in-depth interviews (In-depth interview) to adolescents. The research was conducted in TTU District, East Nusa Tenggara. Research Results: 100% teenage single parent mothers do not work, age 15-19 years with the average education of junior high school to senior high school. Socio-culturally, teenagers imitate city life, lack of or abandon themselves from spiritual life in the family, and having trend to try new things. Teenage pregnancies are mostly unwanted as they happened accidentally. It was due to unsafe sex which was partly triggered by feeling bored with life in dormitories, adopting city life, lack of control from parents and family when teenagers return home, low or none of sexual education, simply following others, mutual consent, forced by boyfriends, poor knowledge Lack of knowledge on reproductive health and the influence of social media causes teenagers to have sex which causes unwanted pregnancies and poor socio-economic conditions. Suggestions The importance of preventing unwanted pregnancies in adolescents in schools should be started from the elementary schools by introducing health reproduction. Promotional materials related to reproductive health should be adapted to the age and development level of adolescents. The purpose of this research is to know the characteristics and readiness of adolescents in the face of menarche in the era of industrial revolution 4.0. The type of research method used is descriptive survey. The population in this study was 45 pegandon junior high school students. Proportional sampling technique. Data analysis is done with statistics deskriftiv. The results of the student readiness study with less than 75.6%, and the readiness category in the good category as much as 24.4% of respondents. Ginger is kind of herbs which has been known to prevent nausea and vomiting. The purpose of this study was to analys the effectiveness of ginger herbal drink in reducing the frequency of nausea and vomiting in the first trimester pregnant women. The design of this research is Pre-Experimental Design with one group prestest-posttest. The population in this study were first trimester pregnant women. The sample in this study amounted to 32 taken using total sampling. Collecting data used using an observation sheet filled in by the respondent for 2 weeks in the form of pre-post. Analysis of the data used wilcoxon test. The result of the study was the value of the statistical test output is known as the Asymp value. Sig. (2-tailed) is worth 0.000. Asymp Value. Sig. (2-tailed) < 0.005, it can be concluded that \"Ha\" is accepted, which means that there is a difference between the frequency of nausea and vomiting in pregnant women before being given ginger drink and after being given ginger drink. So it can be concluded \"there is an effect between drinking ginger on the frequency of nausea and vomiting in pregnant women\". Lack of knowledge about nutrition is one of the most significant reasons for nutritional problems and consequently improper nutrition practices can lead to various complications that occur such as anemia. The purpose of this study was to determine the effect of nutrition education on anemia on the level of knowledge of adolescent girls in Masaran village, Sragen.This study uses a True experimental research design with Pretest and Posttest Control Group Design. The population is all young women in the Masaran Village, Sragen with a sample technique using purposive sampling, totaling 40 respondents. Data analysis using Univariate analysis and Bivariate analysis using t-test. Based on the average value of knowledge in the control group was 63.75 and the average value of knowledge in the intervention group was 85.50. Uncontrolled diabetes mellitus will cause an increase in complications, one of which is diabetic ulcers. Achieving better wound healing is the main goal of diabetic ulcer management. Moisturizing dressings can provide a supportive environment for cells to carry out the wound healing process and prevent further damage or trauma. This study was to determine the effect of modern dressing with moist healing on diabetic ulcers. This type of research is a quantitative research with a quasi-experimental research design with an accidental sampling approach. Data was collected using demographic data and the Wocare For Indonesian Nurses (WINNERS) Scale assessment sheet which is a modified Bates-Jasen score to predict and evaluate the average score of wound healing. WINNERS scale consists of 10 assessments. The results of the intervention that have been carried out have shown a significant improvement in diabetic ulcers as assessed using the Bates Jensen Wound Assessment Tools. Based on the preliminary study, it is known that there is 1 hospital administration officer who is not disciplined and his performance is not good. This study aims to determine the relationship between work discipline and the performance of hospital administrative officers. This research was conducted using a descriptive quantitative survey with a cross sectional study approach. The population and research samples were taken by total sampling, namely all hospital administration officers as many as 5 respondents. The research instrument used a questionnaire. Quantitative data analysis used the chi-square relationship test. The results of this study from 5 respondents based on the results of the Chi-Square test, the significance of p between the independent variables, namely public knowledge and the dependent variable of compliance with the use of masks was 0.002 (p <0.05), then Ho was rejected and it was stated that there was a relationship. The conclusion is that there is a relationship between work discipline and the performance of hospital administrative officers. Retention of medical record documents has been carried out in the last three years, namely 2020, 2021 and 2022. The obstacle in implementing retention is that there are no standard operating procedures regarding retention, storage systems and identification of medical records. This type of research is descriptive qualitative with a cross sectional research design. The research variable consists of three assessment elements on criteria 3.8.4. Collecting research data using interviews, observation and documentation. Processing, analysis and presentation of data is done descriptively. The results of the study are: the decree of the head of the public health center becomes the basis for the policy of implementing medical record retention without standard operating procedures, namely the decree of the head of the community health center Sawit Boyolali number 440 of 2017 concerning the storage of medical records. The implementation of medical record identification is regulated through standard operating procedures number 005/SOP/VII/UKP/2017 regarding patient registration. Medical record coding provides a medical record number code of eight digits, the first two digits are the village/kelurahan code, the second two digits are the medical record number, and the third two digits are the family card code/family status. The medical record storage system is centralized, that is, outpatient and inpatient medical records are stored in one folder/folder. Documentation of the results of examinations, treatment, actions, and other services that have been provided to patients by doctors, dentists and or health workers made immediately and after the patient receives services. The conclusions of the research based on accreditation criteria 3.8.4 are: (1) there is a retention policy in the form of a decree from the head of the public health center without standard operating procedures. Patient identification in medical records is regulated in standard operating procedures regarding patient registration. Medical record coding uses eight digits with a centralized storage system. When span 80 and tween 80 are mixed in a suitable composition and in their manufacture the aqueous phase is dispersed into the oil, span 80 and tween 80 will be arranged alternately at the interface of the oil phase and water phase forming a monolayer that surrounds the droplets so as to produce good emulsifying properties. and form a stable O/A type emulsion. Determine the optimum concentration of span 80 and tween 80 in the formulation of ethanol extract cream of Moringa leaves (Moringa oleifera Lamk) by testing the physical properties and cream of the cream In this study, concentration variations of span 80 and tween 80 to see the best physical properties. The statistical analysis using the Simplex Lattice Design versi 11 method with the parameters of adhesion, dispersion, pH, viscosity, and protective power. The optimum formula is in formula 2 with a ratio of span 80: tween 80 is 0:1 which indicates a desirability value of 0.986. Conclusions. Activities and actions as well as diagnoses contained in the medical record must be coded and then indexed to facilitate services in presenting information to support the functions of planning, management and research in the health sector. This study aims to determine the application of the 4th character code to the diagnosis of Insulin Dependent Diabetes Mellitus (IDDM) based on ICD 10 in inpatient medical records at RSAU dr Siswanto Lanud Adi Soemarmo in 2020. The research method uses descriptive. The approach used is retrospective with observation and interview data collection. The sample used was 117 inpatient medical record documents with a diagnosis of Insulin Dependent Diabetes Mellitus (IDDM) with a sampling technique using saturated samples.The procedure for implementing the coding is not yet in accordance with the SPO and the SPO for the application of the 4th character code does not yet exist. The accuracy of the application of the 4th character code in the diagnosis of Insulin Dependent Diabetes Mellitus (IDDM) was 57% (67 documents) and 43% (50 documents) incorrectly due to an error in selecting the 4th character code. The factors causing the 4th character code inaccuracy based on ICD 10, namely medical personnel factors and medical record personnel (coder). Researchers suggest that hospitals should socialize to medical personnel about writing a diagnosis, this will minimize errors in the 4th character code.
