Integration between complex systems and their subsequent interactions can lead to significant risks, unintended behavior and unexpected consequences. Elicitation of information from big data, and the fusion of precise and uncertain data, remains a challenge for researchers progressively demanding increasing processing powers. Despite the constraints, they often enjoy a freedom of choices in the early design. This is because problems often can be solved in a variety of ways, and early choices are more easily made. These choices often remain, perhaps for the full project lifecycle and sometimes with the next generation of the product. If these early decisions are not proper, there are extra costs subsequently imposed on the project. Figure 1 highlights the increasing costs for mitigations as the project advances. In other words, designers often enjoy exploring design choices in early design phases, and they have the most influence on the design of products or systems, as well as their safety.","PeriodicalId":250838,"journal":{"name":"Journal of System Safety","volume":"10 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128348742","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The potential effects of residual hazards in the operational system context must be reduced to an acceptable level of risk. System reliability focuses on providing continued operational capability in spite of failures. Protecting life, limb and property are generally ignored, however, with the belief that most people avoid dangerous hazards and that accidents will not happen. There is a lack of understanding that a hazard is usually dormant until a change of circumstance occurs and the hazard becomes armed and able to cause harm. When a hazard produces harm in terms of death, injury or damage, it is often identified only as an “accident,” and the existence of a hazard is ignored.","PeriodicalId":250838,"journal":{"name":"Journal of System Safety","volume":"56 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134094494","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It explained that the human body is composed of thousands of control systems; some operate at the cellular level, some at the level of organs and some at the whole-body level. Examples of these systems include the regulation of oxygen and carbon dioxide in the extracellular fluid as well as the insulin control system that maintains the concentration of glucose in the blood. Life goes on when all these control systems function properly. Moderate dysfunction of some of these control systems results in illness, while extreme dysfunction results in death. This, I believe, has some parallels to complex engineered systems.","PeriodicalId":250838,"journal":{"name":"Journal of System Safety","volume":"30 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130493050","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This paper is an effort to push the boundaries of system safety by proposing a novel approach for discovering faulty or missing software requirements by adapting a proven methodology heretofore used in circuit analysis. Karnaugh Mapping is employed in Application-Specific Integrated Circuit (ASIC) design to minimize power consumption, facilitate temperature control, increase functionality and minimize the number of physical logic gates. Karnaugh Maps (K-Maps) are ideally suited to impose order on logical requirements that describe the operation of electronic circuits. With the assumption that software requirements are expressible as logical statements, this paper assesses the ability of Karnaugh Mapping to effectively deconstruct and rationalize developmental requirements in the analysis of software and seeks to demonstrate that K-Maps can be used not only to minimize the number of requirements, but also to detect missing requirements. The analysis conducted in the course of developing this paper indicates that K‑Maps can effectively identify faulty requirements in two examples of varying complexity, provided that sematic conventions are established and observed.","PeriodicalId":250838,"journal":{"name":"Journal of System Safety","volume":"9 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121151814","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This paper discusses the safety precautions that must be taken to adequately protect personnel when operating and/or testing lasers in a free-space environment. First, a brief overview of lasers and associated terminology are introduced. Second, the properties of one of the two most common beam profiles, Gaussian, are discussed. An understanding of the beam profile is important in correctly identifying the safe operating environment for the laser. Exposure to laser energy can result in damage to the eye and skin. Therefore, the concepts of Optical Density, Nominal Ocular Hazard Distance and Skin Hazard Distance are discussed. These parameters help ensure that the correct Personal Protective Equipment (PPE) is used and help identify the hazard zone. Finally, recommendations for selecting the appropriate PPE are discussed. The concepts presented in this paper can be applied to any hazardous laser system.","PeriodicalId":250838,"journal":{"name":"Journal of System Safety","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125276570","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
