[Diffuse thickening of bronchial walls with multiple nodular mucosal protrusions]. [支气管壁弥漫性增厚,多发结节状粘膜突起]。

She had a history of xerostomia for one year. On examination, the bilateral submandibular gland and parotid glands were enlarged. Parotid and anterior cervical lymph nodes were palpable. There were rales in both lungs. The rest of the physical examination was unremarkable. Sialographic analysis showed normal caliber in the main duct, stenosis in secondary ducts, and dilation in the proximal ducts. Minor salivary gland biopsy demonstrated periductal lymphocytic infiltration. Chest computed tomography (CT) showed diffuse thickening of the tracheal and bilateral bronchial walls. Bronchoscopy revealed macroscopic multiple nodules mainly in the trachea and bilateral main bronchus. Endobronchial biopsy showed lymphocytic infiltration in the bronchial submucosa. She was diagnosed with Sjögren's syndrome and treated with glucocorticoids. The dose of prednisone was started at 30 mg/d and tapered gradually. Following treatment, the patient's clinical condition improved dramatically, with shrinkage of the enlarged lymph nodes, bilateral submandibular and parotid glands. A repeated chest CT scan revealed improvement of the tracheal and bilateral bronchial thickening. Multiple nodules in the airway regressed, as evidenced by repeated bronchoscopic examination. The final diagnosis was a large-airway disease associated with Sjögren's syndrome.Among airway diseases in Sjögren's syndrome, peripheral airway diseases including bronchiolitis and bronchiectasis are common; however, central airway lesions in Sjögren's syndrome, especially with macroscopic nodules, are rare. Recently, the Clustered Regularly Interspaced Short Palindromic Repeats interference (CRISPRi) combined with next-generation sequencing has provided many novel insights into the physiology and genetics of MTB. Tracheostomies have several advantages over endotracheal tubes, including less sinusitis or pharyngeal injuries, fewer sedative drugs, improved oropharyngeal hygiene, easier communication, more comfort, preservation of vocal cord function, and so on. However, patients with a tracheostomy require high quality care and often suffer from dysphagia, dysphonia, tracheal stenosis, or excessive airway secretions, <i>etc</i>. In China, there are currently no comprehensive evidence-based clinical guidelines or consensus documents on the management and rehabilitation of patients with a tracheostomy. Approaches to tracheostomy care are inconsistent among different institutions and clinicians. Therefore, a group of top experts specialized in this field in China were gathered to draft this document to reduce variations in practice in the management of patients with a tracheostomy and to minimize complications. Panel members were asked to complete surveys on various aspects of adult tracheostomy care, followed by a conference at which the results were presented and a number of themes outlined for a formal literature review and a preliminary draft. Several conferences were then held to discuss the differences of opinion among the panel members. In general, the panel reached a consensus and a total of 9 issues were covered in the document. At first, the importance of a standardized management protocol, pathophysiological characteristics, indications, complications, contradictions, and techniques used for tracheostomies were discussed. Then, more emphasis was placed on post-operative care such as airway clearance therapy, tube changes, and decannulation criteria, <i>etc</i>. In addition, the most important topic was elaborated, namely the methods used to promote the rehabilitation and decannulation of patients with a tracheostomy, including respiratory and other skeletal muscle training, dysphagia and verbal communication intervention, ventilation support, chest physiotherapy, and specifically the traditional Chinese medical therapies that were excellent in this area, such as acupuncture, moxibustion, and herbal medicine, <i>etc</i>. Finally, a flow chart was created to summarize the suggestions for managing and promoting rehabilitation or decannulation of patients with a tracheostomy. {"title":"[A clinical observational study on the preliminary effect of dupilumab in the treatment of severe asthma].","authors":"Y Li,&nbsp;H M Lin,&nbsp;X X Zhang,&nbsp;C X Ou,&nbsp;J X Xie,&nbsp;Q L Zhang","doi":"10.3760/cma.j.cn112147-20230713-00374","DOIUrl":"https://doi.org/10.3760/cma.j.cn112147-20230713-00374","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the effect and safety of dupilumab in the treatment of patients with severe asthma in a preliminary clinical observational study. <b>Methods:</b> This study retrospectively analyzed the clinical data of 20 patients with severe asthma who received dupilumab for 4-12 months between 2019 and 2022 at the First Hospital of Guangzhou Medical University, comparing pre-and post-treatment laboratory data, oral glucocorticoid dose (OCS), asthma control test (ACT) and adverse effects. The median age of the 20 patients was 48.5 (41.0-52.8) years, including 14 males and 6 females. The clinical data of 10 patients treated with other biologic agents were further analyzed to determine the reasons for switching to biologic drug treatment and the efficacy of dupilumab in these patients. Paired <i>t</i>-tests or Wilcoxon signed-rank tests were used for comparisons. Mann-Whitney analysis was used for inter-group comparison, and chi-square test or Fisher test was used for inter-group comparisons of count data. <b>Results:</b> A total of 20 patients were included in this study. All 20 severe asthma phenotypes were type 2 (T2)-high and completed at least the first 4 months of treatment, including 17 patients who completed 12 months of treatment. Among patients who completed 4 months of treatment, the asthma exacerbation score decreased from 1.0(0.3-1.0) episodes/4 months to 0.0(0.0-1.0) episodes/4 months<i>, P</i><0.001, and FEV<sub>1</sub>/FVC increased from 58.4% (50.5%-69.0%) to 66.9% (59.6%-77.7%), <i>P</i><0.01. The number of patients requiring OCS maintenance therapy decreased from 15 (75%) to 9 (45%), <i>P</i><0.05. Among patients who completed 12 months of treatment, the asthma exacerbation score decreased from 1.0(0.5-1.0) episodes/4 months to 0.0 (0.0-0.0) episodes/4 months<i>, P</i><0.01, and FEV<sub>1</sub>/FVC increased from 57.9% (49.6%-67.8%) to 72.7% (64.6%-78.7%), <i>P</i><0.01. The number of patients requiring OCS maintenance therapy decreased from 13 (76%) to 6 (35%), <i>P</i><0.01. In 10 patients with a history of previous biologic therapy, the most common reasons for switching to a biologic were a poor response to previous monoclonal antibodies (40%) and loss of control of asthma symptom control after discontinuation of monoclonal antibodies (30%). The remaining reasons were patients' uncontrolled symptoms of chronic rhinosinusitis (20%) and irregular or underdosed use of previous biologics (10%). After 4 months of switching to dupilumab, 10 patients experienced varying degrees of improvement in asthma control. <b>Conclusions:</b> The application of dupilumab for the treatment of T2-high severe asthma showed good efficacy and few adverse effects. Currently, mechanical airway clearance techniques are widely used in clinical practice and have significant effects in clearing respiratory secretions, improving symptoms of respiratory distress, and preventing and reducing respiratory tract infections. However, up to now, there is no consensus, guidance, or standardized procedures for the clinical application of mechanical airway clearance techniques. Therefore, the Respiratory Care Group of Chinese Thoracic Society organized relevant experts to write the consensus on the clinical application of mechanical airway clearance techniques, and systematically describe the definition, historical development, working principles and mechanisms, and clinical applications of mechanical airway clearance techniques. OCT has been used in the airway for more than 20 years, and researchers have explored and improved the imaging methods of OCT in the airway, focusing mainly on expanding the imaging site and enriching the imaging content. In terms of broadening the imaging site, it covers the airway from generation 0 to 9. In terms of enriching imaging content, additional assessment of airway wall blood vessels, airway smooth muscle, fibrous tissue, and airway compliance can be performed. It plays an important role in the study of various respiratory diseases. This condition eventually leads to right ventricular hypertrophy and ultimately death from right ventricular failure. Although the exact pathogenesis of pulmonary hypertension remains unclear, autonomic nervous system dysfunction is recognized as one of the contributing factors. Increasing evidence supports the hypothesis that activation of the sympathetic nervous system plays a role in the development of pulmonary hypertension. In addition, pulmonary artery denervation has shown some effectiveness in the treatment of this condition. It is caused by pathogenic variants in the <i>FLCN</i> gene located on chromosome 17p11.2. Although an increasing number of patients with BHD syndrome are being recognized in China, the missed diagnosis and delayed diagnosis are still common. In addition, appropriate management is difficult for most of them. Pulmonary cysts and pneumothorax are the main presenting features, but skin and renal lesions appear to be less common in Chinese subjects than those reported from European and American countries. Therefore, the consensus is established by experts from the related disciplines to improve the diagnosis and management of BHD syndrome. This consensus consists of 15 recommendations related to BHD syndrome, including clinical assessments, diagnosis, differential diagnosis, treatment, follow-up, and family management. In particular, it provides revised diagnostic criteria based on the Chinese situation. We hope to promote scientific and clinical progress in this rare disease and improve the prognosis of the patients.<b>Summary of recommendations</b><b>[Recommendation 1]</b> The folliculin (<i>FLCN</i>) gene is currently the only affirmative causative gene for Birt-Hogg-Dubé (BHD) syndrome, and the pedigree analysis of genetic testing of family members' samples can assist in the rapid identification of causative gene variants. The genetic testing methods, including Sanger sequencing, Multiplex Ligation-dependent Probe Amplification (MLPA), and Next-Generation Sequencing (NGS), can be chosen based on individual patient's care needs. (43 voting experts; 43 in favor, 0 against, 0 abstention).<b>[Recommendation 2]</b> Patients with BHD syndrome should undergo chest CT scan to evaluate cystic lesions in the lungs, and routine evaluation of the kidneys for tumor foci, using ultrasound, enhanced CT, or MR as appropriate. (43 experts voted; 43 in favor, 0 against, 0 abstention).<b>[Recommendation 3]</b> Clinicians should establish a diagnosis based on the appropriate clinical presentation and in conjunction with genetic test results and/or a family history of BHD syndrome. Patients with mild asthma account for more than half of the total asthma population. The diagnosis of mild asthma is much more difficult because the symptoms are mild and sometimes atypical, and even in the presence of near-normal pulmonary ventilation function, usually undetected by physicians or patients. Timely diagnosis, effective treatment and standardized management will prevent from the progression and reduce the burden of mild asthma. In order to guide physicians in the diagnosis, treatment and management of mild asthma, the Asthma Group of the Chinese Thoracic Society organized relevant experts to develop the Expert Consensus. This consensus includes epidemiology, diagnosis and evaluation, treatment and management, education and prevention of mild asthma, also involving diagnosis and treatment of mild atypical asthma. There is little understanding of the risk factors, clinical characteristics, and thrombolytic therapy for patients with acute pulmonary embolism at intermediate-high risk in Tibet Plateau. In this paper, we reported the data of 4 patients with acute intermediate-high risk pulmonary embolism treated with thrombolytic therapy in Lhasa People's Hospital. The demographic characteristics, clinical manifestations, treatment, and outcomes were analyzed. We summarized the clinical features and raised scientific issues.
