{"title":"Recomendaciones de práctica clínica de la Sociedad Española Multidisciplinar de Dolor (SEMDOR) para el buen uso médico de los opioides de prescripción en el tratamiento del dolor crónico no oncológico","authors":"E. On the other hand, we must not forget that pain control is also an internationally recognized human right. However, the fear and rejection of the use of opioids as a result of the epidemic situation experienced in the United States has led to an undertreatment of patients, as Meyers and Quenzer state in their book on psychopharmacology, where they talk about how an unfounded fear of addiction has deprived many patients of receiving adequate pain relief. The treatment of pain is the responsibility of several health professionals, with Primary Care doctors prescribing these drugs the most, either by their own decision or by continuation of a prescription from the specialist. On the other hand, the incidence and prevalence of pain is expected to continue growing in the coming years mainly due to the greater survival of patients suffering from oncological and non-oncological pathologies. In this environment, it is essential to provide guidelines for the correct use of opioid drugs by prescribers and dispensers. To this end, SEMDOR present these recommendations on how to make good medical use of opioid drugs to treat chronic non-oncological pain (persistent or recurrent) lasting more than 3 months. These recommendations provide specific guide for addressing the selection of patients eligible for opioid treatment, as well as for initiation, follow-up, monitoring, or discontinuation of treatment. Additionally, recommendations are established on the responsibilities and resources that affect doctors, pharmacists and patients. Finally, we provide some points of improvement that should be implemented in relation to the management of opioid drugs at the level of health systems to enhance the systems for measuring prescriptions and adverse events, optimize dispensing with the use of new technologies, advance in multidisciplinary coordination and establish quick and simple means to communicate clinical recommendations such as the one presented here to all stakeholders. {"title":"Dolor anal nocturno: Proctalgia fugax. Clinically, it manifests as intense pain lasting minutes, usually at night, that wakes the patient up, with the patient remaining asymptomatic between crises. There is no curative treatment, although we can shorten the duration of the crisis with inhaled salbutamol. Case presentation: a 52-year-old woman who reported episodes of paroxysmal anal pain that woke her up at night lasting less than 30 minutes, not related to defecation. Physical examination revealed no lesions, and complementary tests were normal. Conclusions: Proctalgia fugax is a rare pathology that interferes with night rest and impairs the quality of life of patients, which can improve after treatment with inhaled salbutamol. We present the case of a 58-year-old woman referred to the pain treatment clinic with a history of low back pain and diagnosed with “nutcracker syndrome”. Venous angiography is performed and a stent is implanted in the renal vein, the gradient and pain disappear. After three months the pain reappeared, so a psoas-iliac muscle block was performed with local anesthetic and corticosteroid. Analgesia is obtained for another 3 months. {"title":"¿Cómo es el uso de medicamentos opioides en España? Análisis de los datos de la encuesta EDADES 2017","authors":"E. Regueras, J. Guzmán","doi":"10.20986/mpj.2021.1005/2021","DOIUrl":"https://doi.org/10.20986/mpj.2021.1005/2021","url":null,"abstract":"Objectives: Quantify the level of use of opioid medicines in this general population, the level of follow-up to prescribed guidelines, the reason for use, and the means of obtaining these medicines in Spain. Methodology: Analysis of the raw data for the EDADES 2017 study that has been conducted in Spain every two years since 1995 among the general population (15 to 64 years of age). After categorizing and cleaning the data, several pivot tables were performed to extract the desired information. Results: In Spain the prevalence of any opioid drug use at 12 months was 6.7 % of the population between 15 and 64 years of age. 87 % of this use was associated with weak opioids (tramadol or codeine) and the rest was related to major opioids, with morphine being the most widely used with 8% of total opioid users. 92 % of people obtained the medicine through their own prescription while 4 % obtained it at the pharmacy without a prescription and an additional 3 % obtained it through friends or family. 74 % of patients had followed the dose and duration of treatment guidelines prescribed by the doctor, while 19 % had used opioid medicines in lower doses or for less time than prescribed. Conclusions: Use of strong opioids is low in Spain, most patients use it under medical supervision and following the guidelines prescribed by the doctor. We describe the case of a patient with chronic pain due to vertebral compression, who was successfully treated with cutaneous branches of the intercostal nerves in the mild axillary line “modified” block.
