We present the case of a 58-year-old woman referred to the pain treatment clinic with a history of low back pain and diagnosed with “nutcracker syndrome”. Venous angiography is performed and a stent is implanted in the renal vein, the gradient and pain disappear. After three months the pain reappeared, so a psoas-iliac muscle block was performed with local anesthetic and corticosteroid. Analgesia is obtained for another 3 months.
