{"title":"Inequality in the distribution of resources in health care system by using the Gini coefficient and Lorenz curve (A case study of Sistan and Baluchestan province over a five-year period)","authors":"A. Mojiri, Kambiz َََAhmadi","doi":"10.52547/payesh.21.3.227","DOIUrl":"https://doi.org/10.52547/payesh.21.3.227","url":null,"abstract":"Objective(s): One of the most important goals of health systems is to provide equal access to health care services and fair distribution of their indicators. The aim of this study was to evaluate the inequality in access to human resources, health care, and hygienic services in 14 cities of Sistan and Baluchestan province. Methods: This was a descriptive study. Unequal access to health care services was assessed according to several items such as the number of physicians (general practitioner, pharmacist, and dentist), specialist physicians, paramedics, hospitals, active beds, laboratories, and pharmacies. The data was collected from the statistical yearbooks of the province. The Gini coefficient and Lorenz curve were the criteria to measure inequality. Data analysis and visualizations are performed in R software. Results: Based on the five years studied, the number of physicians, specialist physicians, hospitals, and active beds per ten thousand persons in 2019 has increased compared to 2014. There was no significant increase in other measures. The mean Gini coefficient for physicians, specialists, paramedics, hospitals, active beds, laboratories, and pharmacies was 0.106, 0.324, 0.132, 0.317, 0.336, 0.120, and 0.299, respectively. The highest rate of unequal access was related to specialist physicians and active beds. Furthermore, the lowest rate was related to physicians and laboratories. Conclusion: Gini coefficients for specialist physicians and active beds in 2019 compared to 2014 have decreased significantly over time. However, inequality in the distribution of these two health indicators was high. For other measures there were no significant decrease in the Gini coefficient. The purpose of this study was to identify the critical success factors of hospitals in Tehran province. Methods: This qualitative research was conducted by phenomenological method. Semi-structured and in-depth interviews were used to collect data. The interviewees included 93 senior, middle and front-line managers working in 30 hospitals in Tehran province. Data analysis was performed by thematic analysis method using MAXQDA software. Results: A total of 51 critical success factors were identified for hospitals, which were classified into 8 main themes and 22 sub-themes. Key themes include management and leadership, planning, organizational learning, organizational culture, employee management, patient management, resource management, and process management. Transformational leadership, plan-oriented, organizational trust, the presence of renowned physicians, patient satisfaction, and the availability of resources and facilities were key factors in the success of hospitals. Conclusion: Hospital managers should strengthen these critical success factors. Methods: In this methodological study, the research team selected a number of items from the main instrument covering the main constructs of the HELIA. The selection was based on the most relevant items to each construct. A group of 12 experts and ten adults assessed content and face validity respectively. Exploratory and confirmatory factor analysis was performed to assess structural validity. The reliability of the instruments was evaluated by internal correlation (Cronbach's alpha coefficient) and intraclass correlation (ICC). Results: A nine-item questionnaire was developed. Experts identified the content validity of the short version as desirable (the CVR was more than 0.56, the CVI was more than 0.79). Also, the face validity as assessed by ten adults was satisfactory. The results obtained from exploratory factor analysis showed a two-factor structure for the questionnaire, namely basic skills and decision-making skills, that jointly accounted for about 61% of the variance observed. Furthermore, the confirmatory factor analysis indicated acceptable fit indexes for the data: X2/df =2.20, GFI=0.95, CFI=0.97, NFI=0.95, NNFI=0.96, SRMR=0.068, and RMSEA=0.074. The Cronbach's alpha coefficient and ICC for each of the two dimensions were satisfactory (alpha = 0.84 and 0.81 and ICC = 0.85 and 0.82). The values for the whole items were 0.91 and 0.81, respectively. Conclusion: The HELIA-SF, including nine items and two subscales (basic skills and decision-making skills), was found to be a reliable and valid instrument to measure health literacy in adults. Therefore, considering the inflationary conditions and its fluctuation which leads to uncertainty in the economy, the purpose of this study was to investigate the effects of inflation fluctuation on consumption and unemployment as key indicators in public’s health. Methods: In this study, the fluctuation and uncertainty of inflation was estimated using Stochastic Differential Equation (SDE). For surveying the shocks of inflation fluctuation on consumption and unemployment, the impulse respond (IR) that is so useful in structural Vector Auto Regressive (SVAR) models were used. The source of quarterly data was Statistical Center of Iran (S.C.I) from 2001 to 2020. Results: The results indicated the fluctuation of inflation was effective and meaningful on unemployment by 0.04 and consumption by (-0073).
