Methods: The study was conducted from July 2018 to December 2019 at the Provincial Hospital Center of El Jadida. It is a descriptive study, aims to describe the determinants of the implementation of the risk management approach at the PHC El Jadida through an interview with the heads of the HRM unit, a questionnaire for hospital managers and a questionnaire for professionals of the said PHC. Results: Among the 338 participants in the study, more than half of them said that training and awareness are the main determinants for the implementation of the HRM approach in addition to the commitment of the administration. All of the managers interviewed agreed that the existence of the HRM unit is the starting point for any implementation of the HRM approach, while 87.1% stated that there was no HRM policy. 87.6% of the professionals have not had any HRM training, while 97.6% of them have expressed their needs in terms of HRM training. 85% of the professionals are unaware of the HRM procedure and the obligation to declare risks, which has impacted the implementation of this approach. Discussion: The confrontation of our results allowed us to determine the determinants of the implementation of HRM, namely: (a) establishment of the HRM unit, (b) commitment of the administration, (c) awareness raising, (d) training, (e) design of an HRM policy, (f) institutionalization and regulation of the approach, (g) design of the HRM process. They are biosynthesized from arachidonic acid by the enzymes cyclo-oxygenase-1 and -2 and inhibitors of these enzymes are widely indicated as drugs for treating inflammation, pain, and fever. Following structural elucidation of the pharmacologically defined prostanoid receptors, drug design largely switched from COX inhibitors to pharmacology. Potent and selective antagonists for each of the prostanoid receptors (DP 1-2 , EP 1-4 , FP, IP, TP) were developed but these have not translated into widely used new drugs, despite the clear importance of prostanoids in disease. Responding to this situation, a new polypharmacological approach was adopted whereby multiple activities were embodied in a single molecule. The receptors selected for antagonism were selected based on known roles in mediating inflammation and fibrosis. Receptors opposing pro-inflammatory events, notably EP 2 , were left open so that PGE 2 may be converted from a pro-inflammatory to an endogenously released anti-inflammatory mediator. This resulted in compounds with greater anti-inflammatory efficacy than antagonists selective for a single prostanoid receptor and cyclo-oxygenase inhibitors. Against the background of a literature review, the pathomorphological characteristic, the necessary immunohistochemical panel and the difficult differential diagnosis with the wide range of benign and malignant mesenchymal tumors are discussed. The necessary complex oncological treatment, directly depending on the prognostic risk factors for each of these synchronous tumors is analyzed. For the first time in the English medical literature, a clinical case with synchronous tumors - rectal achromatic malignant melanoma and gastric solitary fibrous tumor is presented. Endothelial dysfunction/injury caused by high shear stress promotes initiation of atherosclerosis, followed by subsequent adhesion of circulating inflammatory cells to the dysfunctional endothelium. These processes are further aggravated by accumulation of cholesterol in the arterial wall and lead to atherosclerosis [2]. One of the identified features of endothelial cells in response to local inflammation is dedifferentiation and switching of endothelial-to-mesenchymal transition which may be involved in the loss of endothelial phenotypes towards unspecialized mesenchymal-like cells that could undergo redifferentiation into mesodermal cell types, including adipocytes, in response to local inflammation [3]. This process can be triggered by various inflammatory cytokines and TGF-β which are produced in inflamed adipose tissue [4]. Adipose tissues play a significant role in atherosclerosis progression and are broadly classified as white adipose tissue (WAT) and brown adipose tissue (BAT). Initial studies have supported the hypothesis that dysfunctional WAT might be positively associated with atherosclerosis development, whereas activation of BAT may protect against atherosclerosis development.
