This report describes a 23-year-old, right-handed male who presented in March 2024 with acute anterior pisiform dislocation following indirect trauma while lifting furniture. The patient experienced pain, mild swelling, and tenderness in the distal ulnar region of the right hand, with initial radiographs confirming the dislocation. Spontaneous repositioning occurred during transfer to a trauma hospital after cast immobilization, eliminating the need for further imaging or invasive treatment. The patient had no history of hypermobility syndrome. Early physiotherapy facilitated full recovery of range of motion. After one-year follow-up, the patient showed a Quick-DASH score of 0, indicating complete functional recovery; therefore, no control radiographs were performed to avoid unnecessary radiation exposure in this young patient. The Quick-DASH (Disabilities of the Arm, Shoulder and Hand) score of 0/100 confirms excellent objective and subjective outcome with no residual disability. The injury likely resulted from wrist hyperextension and forceful flexor carpi ulnaris contraction. Diagnosis typically relies on clinical findings and radiographs, though advanced imaging may be required in ambiguous cases. Treatment options include immobilization, closed reduction, open reduction with internal fixation, or pisiform excision, depending on the case. This report underscores the importance of considering isolated pisiform dislocation in young patients with ulnar-sided wrist pain post-trauma and highlights the efficacy of conservative management when spontaneous reduction occurs.</p>","PeriodicalId":23291,"journal":{"name":"Trauma Case Reports","volume":"61 ","pages":"101317"},"PeriodicalIF":0.0,"publicationDate":"2026-03-03","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12991944/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147481605","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

If they are linked with injuries involving other systems within the body, the order of morbidity and mortality rises severalfold. Isolated pelvic fractures due to trauma are rare and their association with bowel injury is even rarer. Our case, a traumatic pelvic fracture resulting in ileal obstruction due to herniation and entrapment at the fracture site, is one of these rare cases. This case may also be considered as a Traumatic Abdominal Wall Hernia (TAWH) - essentially the herniation of bowel secondary to abdominal trauma - an entity first coined in 1906. If so, our case qualifies as the 21st such case in recorded history. Low-energy force is sufficient to cause pelvic fractures in the elderly population while high-energy forces are required for similar conditions among younger patients. Thus, the multitude of risk factors that determine falls in the elderly population have to be minimized as even a small mishap may result in something grave. Damage Control Surgery was performed. Our patient's entrapped bowel was reduced and the same area was fashioned into a double-barrel ileostomy. While the protocol to be followed in cases of bowel involvement is uncomplicated, the dilemma that exists is whether to subject the patient to operative intervention or adopt a conservative approach vis-a-vis the pelvic bone fracture. In cases of isolated pelvic fractures, a non-operative line may be taken, although a significantly higher survival rate has been seen with those undergoing operative intervention.</p>","PeriodicalId":23291,"journal":{"name":"Trauma Case Reports","volume":"61 ","pages":"101318"},"PeriodicalIF":0.0,"publicationDate":"2026-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12954182/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147356516","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Initial management included transtibial traction under a Damage Control Orthopaedics (DCO) protocol, followed by definitive fixation 10 days later using a single-stage stepwise surgical approach.
