As previously no such research was found through an extensive literature survey, the current study aimed to evaluate gout patients residing in Lahore, Pakistan.\u0000\u0000Methods: To conduct this cross-sectional study, n=203 responses were obtained by a simple and objective-oriented questionnaire devised for the collection of data. Dependent variables association was measured by the Pearson Chi-square. A p-value <0.05 was considered statistically significant.\u0000\u0000 \u0000\u0000Results: A total of n=203 responses results had shown that male patients were predominant 127 (62.56%) and most patients were elderly and obese. Respondents were suffering in ignorance for a long time and many of their joints were affected by gout. Outcomes regarding the severity of pain were based on patient experience, including mild (40.39%) moderate (31.52%) and severe (28.07%). In terms of treatment, 101 (49.75%) were on Allopurinol/ febuxostat/ colchicine, 31(15.27%) followed by diet modification. 190 (93.6%) reported that gout treatment puts an extra financial burden on them. Restricted mobility was experienced by 173 (85.2%), 157 (77.3%) patients preferred to visit the rheumatologist for a checkup, and 91 (44.8%) seemed help from the pharmacists.\u0000\u0000Conclusion: Gout severely affects both physical and physiological health (p=0.001). The overall burden of gout is substantial and may be increasing with time. However, the levels of urinary amylase can be sought for being less invasive. The study aimed to find out the diagnostic accuracy of urinary amylase compared to serum amylase and serum lipase and their association with the degree of severity of acute pancreatitis.\u0000\u0000 \u0000\u0000Methods: A randomized clinical control study was conducted on n=180 acute pancreatitis patients (18-50 year) in the Ziauddin and PNS Shifa Hospital, Karachi from September 2019- August 2020. Serum amylase, serum lipase and urinary amylase levels were checked at the time of admission followed by 24 hours and at discharge. ANOVA with post-hoc Tuckey’s test was used to determine the association of amylases with the severity of acute pancreatitis and p˂0.05 was considered as statistically significant.\u0000\u0000 \u0000\u0000Results: The patients with acute pancreatitis had a mean age of 51.76 ±10.8. Urinary amylase had a strong significant association (p˂0.05) with acute pancreatitis compared to serum amylase and lipase (p=0.024). There was an insignificant association of urinary amylase with acute pancreatitis after 24 hours. Oral submucous fibrosis (OSMF) is a chronic inflammatory disorder, which is mostly treated with steroids. This clinical trial was conducted to assess the effectiveness of N. sativa oil in comparison to steroids for treating OSMF.\u0000\u0000Methods: This randomized clinical trial consisted of n=39 clinically diagnosed OSMF patients aged 18 years and above. Patients were divided through randomization into two groups. Group 1 was prescribed steroid lotion while Group 2 was given N. sativa oil (Cold-pressed) to be applied on the buccal mucosa for three months along with physiotherapy. Mann-Whitney U Test while the Wilcoxon Rank sum test were applied. A p-value of less than 0.05 was considered statistically significant.\u0000\u0000Results: Thirty patients (aged 35.33±2.07) completed the trial in which the majority were males (n=28, 93.30%) and were from Urdu-speaking communities [22 (73.30%)]. Most of the patients (n= 18, 60%) were consuming a combination of Pan and Areca Nut (Chaliya), 04 (13.3%) were consuming only areca nut while 03 (10%) used Gutkha as a habit. This includes cephalometric findings and study-cast analysis as influencing factors. This research aimed to identify variables that aid in clinical decision-making in the treatment of Class 1 malocclusion patients.\u0000\u0000Methods: This was a retrospective study conducted on pre-treatment records of Class I patients. All patients had a Class I dental and skeletal malocclusion. A sample of n=80 patients was included (40 extraction cases, 40 non-extraction). Cephalometric values and study-cast analysis for tooth-arch size discrepancy were performed and binary logistic regression was applied to run the univariate and multivariable analysis to investigate the association of different variables with extraction and non-extraction treatment. Odds ratio (OR) and 95% confidence intervals were reported and p <0.05 was considered statistically significant.\u0000\u0000Results: Univariate logistic regression showed that mandibular and maxillary crowding (p<0.001), overjet (p<0.001), SNGo (p= 0.01), nasolabial angle (NL) (p= 0.02), lower anterior facial height (LAFH) (p= 0.03) and upper lip in plane (ULE) (p= 0.05) had a significant association with the extraction and non-extraction groups. The objective of this study was to determine the effectiveness of Ondansetron and Dexamethasone on nausea and vomiting in patients undergoing spinal anesthesia for cesarean section.\u0000\u0000Methods: This was an experimental study conducted in the Department of Anesthesia and Department of Obstetrics and Gynecology, Hamdard University Hospital, from April 15, 2021-August 15, 2021. Pregnant women (n=93) with the age >18-<45 years with American Society of Anesthesiologists (ASA) class I and II were enrolled. Group-A (n=43) were given 8 mg of Ondansetron, while Group-B (n=50) received 8mg of Dexamethasone. Post-operative nausea and vomiting assessment was done by Apfel score and was measured at 1 minute, 5 minutes, 10 minutes and 30 minutes after spinal anesthesia. Chi-square and student t-tests were performed for analysis.\u0000\u0000Results: There was an insignificant difference (p=0.029) in terms of the demographic factors and vital signs between both the groups while a significant difference (p=0.05) was reported only in systolic blood pressure between both the groups. Insignificant difference was observed in parity, use of intraoperative opioids, sedatives, use of vasopressors, postoperative vomiting and shivering (p>0.05). Post-operative 11(22.0%) cases reported nausea and 1.0(2.0%) cases of vomiting in the dexamethasone group with significant difference (p=0.001), however, participants of the Ondansetron group did not report any symptom.\u0000\u0000Conclusion: Ondansetron 4mg/kg dose decreased the propensity of nausea and vomiting after cesarean section. Chromosomal abnormalities like translocations lead to the formation of oncogenes, some of which are strong predictors of prognosis and response to anti-leukemic therapy. This study aimed to find out the frequency of BCR-ABL gene translocation in B-ALL patients by fluorescence in situ hybridization (FISH) and its association with their clinicopathological parameters.\u0000\u0000Methods: Patients (n=150) aged 1-17 years with a confirmed diagnosis of B-ALL were selected. Peripheral blood and/or bone marrow aspirate samples were obtained and Breakpoint cluster region-Abelson murine leukemia viral oncogene (BCR-ABL) translocation by FISH was observed. The patient’s demographics, hemoglobin levels, total leucocytes count, platelet count, FISH results, CNS status and risk stratification were recorded. Data was stratified and the Chi-square test was applied, p-value < 0.05 was considered statistically significant.\u0000\u0000Results: There were 100 (66.67%) males and 50 (33.33%) females. The average age of the patients was 7.03±4.51 years. The frequency of BCR-ABL translocation in B-ALL was 16(10%). A significant association was found between age and BCR-ABL translocation (p-value <0.05), however, an insignificant association was recorded among gender, Hb levels, TLC, platelet count, CNS status, proposed risk stratification system of B-ALL and BCR-ABL translocation (p-value >0.05).\u0000\u0000Conclusion: The frequency of BCR-ABL translocation in B-ALL was significantly high in the targeted population. FISH improves detection of the BCR-ABL translocation in either metaphase or interphase cells. Uterine fibroids are benign mesenchymal tumors and sometimes infarction can occur after uterine artery embolization or torsion in the fibroid. We report a delayed and neglected case of uterine fibroid, which caused spontaneous infarction of the whole uterus and its appendages in a 39-year-old unmarried woman. She was presented with severe pallor and dyspnea for which she was kept in the intensive care unit to stabilize her condition. The patient had a history of an abdominal mass for 10 years which was first diagnosed as fibroid of 7cm. The patient was planned for laparotomy, bilateral salpingo-oophorectomy, total abdominal hysterectomy and omentectomy. The intra-operative finding showed a huge infarcted uterine mass with congested and massively necrosed ovaries and tubes. Histopathological examination reported polyp presence in hemorrhagic infarction. Sections from mass, tubes and ovaries showed extensive hemorrhagic infarction with congested vessels. Hence timely and early diagnosis of these disorders is of prime importance to halt their malignant transformation. A search of published works was done using the online databases of PubMed, Scopus, Web of Science, and Cochrane Library”. Data from resource-constrained laboratory settings worldwide shows limited documentation of the efficacy of advanced cytological techniques, including LBC and cell block preparations. The oral mucosa can be a suitable area for regular cytological screening due to its easy accessibility. Liquid-based cytology (LBC) can preserve the cellular details and reduce the overlapping of cells, enabling precise interpretation, reducing false-negative results and aiding in the diagnosis of premalignant and malignant lesions of the oral cavity with more accuracy compared to exfoliative cytology. The remaining sample in LBC can be used in cell block formation and various ancillary tests like immunocytochemistry, immunofluorescence, and molecular studies. Literature showed a scarcity of data available regarding Pakistan. To examine the safety, complications, and cost-effectiveness of the instrumental tie, ligaclip and endoloop procedures have been used for the closure of the appendix stump. The objective of this study was to analyze the clinical outcomes, as well as to compare the effectiveness of the endoloop, instrumental tie, and ligaclip in laparoscopic appendectomy procedures.\u0000\u0000 \u0000\u0000Methods: The Sir Syed Medical College Hospital, Karachi was the center of this Randomized Control Study, from June 2020 to December 2020. Acute appendicitis patients (n=120) having age 7-85 years were categorized into three groups: A, B, and C. Ligaclips were applied to those in Group A, Instrumental Tie to Group B, and Endolope to Group C patients. Each group consisted of 40 participants. The Kolmogorov Smirnov test was used to determine the variables for normal distribution. The Chi-square test was used to measure the association between the duration of surgery and hospital stay.\u0000\u0000 \u0000\u0000Results: The statistical significance (p<0.05) of the relationship between the length of hospital stay and the duration of surgery had been established. The aim of this study was to evaluate the effect of 635nm low-level laser on salivary flow rate and quality of life (QoL) in post-radiotherapy oropharyngeal cancer patients.\u0000\u0000Methods: A prospective study was conducted on patients (n=34) with persistent post-radiotherapy xerostomia received low level laser therapy (LLLT) (635nm) on the three major salivary glands twice a week for 12 weeks. For each point, dose was 18.75 J/cm2, irradiation time was 15 s, energy was 1.5 J. Energy for each session was 33 J and laser tip diameter was 0.08cm2. Extra oral points were marked over the parotid gland (6 points), submandibular (3points) and sublingual gland (2 points). Unstimulated and stimulated salivary flow rate (SFR) and quality of life (QoL) using University of Washington (UW) QoL questionnaire were assessed. Unstimulated SFR and QOL scores were normally distributed and hence paired t-test was used. A p-value ≤0.05 was considered as statistically significant.\u0000\u0000Results: The mean unstimulated SFR was significantly increased by 0.16 units after LLLT (p=0.001). Similarly, the median stimulated SFR significantly increased by 0.45 units post LLLT respectively (p=0.001).
