Foreign bodies have great variation in size, shape and consistency posing a unique clinical challenge for gastroenterologists. We report a rare case of an esophageal stricture complicated by an impacted esophageal foreign body refractory to retrieval with conventional interventional modalities. There are many causes of TMAs, but thrombotic thrombocytopenic purpura (TTP) must always be considered because of its high mortality rate. Treatment with therapeutic plasma exchange and transfusions can reduce the mortality rate but those treatments carry their own morbidity. Severe cobalamin deficiency can cause a clinical picture similar to TTP. Understanding the pathologic changes of this pseudo-TTP can allow physicians to suspect this difference early in the clinical course. Analyzing both objective factors (such as grades and standardized testing) and subjective factors (such as recommendations, essay quality and personality) are involved in determining how good a candidate a given student is. Prior to the Match, residencies rank each applicant on their list for submission to the National Residency Match Program (NRMP). The final rank list scores of medical students who applied to, and were accepted by, the Joan C Edward School of Medicine’s (JCESOM) Family Medicine residency program between the years of 2005 to 2011 were analyzed to determine if there was any correlation between the order in which the students who entered the program were ranked during the Match process, and the order in which these same students (now residents) were ranked on their final scores (just before graduation, three years later) on the Core Clinical Competencies. Furthermore, the United States Medical Licensing Exam (USMLE) Part 1 and Part 2 scores were also compared to their final scores on the Core Clinical Competencies to determine their significance. Using the Pearson Correlation Coefficient, it was determined that there was a statistically significant correlation (0.621; 95% confidence interval 0.39 to 0.78) between the order that the residents were listed on the rank list and their order on the final evaluation. No such correlation was found for USMLE Part 1 (-0.13; 95% confidence interval -0.42 to 0.19) or USMLE Part 2 (-0.26; 95% confidence interval -0.53 to 0.054). Recent literature has begun to demonstrate the benefits of utilizing suppressive antibiotics as an adjuvant treatment in addition to surgical debridement. The use of oral suppressive antibiotics carries with it inherent hurdles regarding dosing, tolerance, side effects, and cost that need to be considered. {"title":"Residents' Views on Research and Quality Improvement Training Can Guide Practice-Based Research Network Collaboration.","authors":"Treah Haggerty,&nbsp;William Lewis,&nbsp;Christine Plaugher,&nbsp;Jun Xiang,&nbsp;Eric Radcliffe,&nbsp;Stacey Whanger,&nbsp;Adam Baus","doi":"10.21885/wvmj.2018.14","DOIUrl":"https://doi.org/10.21885/wvmj.2018.14","url":null,"abstract":"<p><strong>Background and objectives: </strong>This study was conducted by the West Virginia Practice-Based Research Network Learning Collaborative to assess research activity, confidence, and attitudes toward residency programs' research and quality improvement requirements and inform the integration of the state-wide practice-based research network (PBRN) as mentors to support practice transformation implementation initiatives across various resident training sites in West Virginia.</p><p><strong>Methods: </strong>This pilot study assessed residents' attitudes regarding (1) research activity, (2) confidence and (3) requirements of their program in research and quality improvement training by using an anonymous survey, administered during regular residency meetings.</p><p><strong>Results: </strong>Of the 68 residents,representing four DO and MD Family Medicine residency programs in West Virginia, 40 (58.8%) responded to the survey. About 64 percent of residents had worked on a quality improvement project, and more than half of residents (52.5%) submitted a research project for a competitive presentation within the most recent year. Sixty-five percent felt satisfied with the residency program's research and quality improvement curriculum. However, only 55 percent felt confident to perform a project and 52 percent submitted a project to a competitive forum.</p><p><strong>Conclusion: </strong>Only half of the residents demonstrate activity and confidence in research and quality improvement. While foreign bodies are most common in the pediatric population and in adults with underlying risk factors, we present an interesting case of a non-debilitated adult who aspirated a corn kernel. It is necessary for the clinician to recognize the symptoms, and for the radiologist to make the correct imaging diagnosis, to expedite proper treatment. Radiographs are often not useful in identifying the location due to the radiolucent nature of most aspirated foreign bodies. However, secondary findings may be visible. Computed tomography (CT) may be used to evaluate aspiration in clinically stable adults. Yet, not all women who desire it actually undergo the procedure. We studied clinical and demographic factors that were associated with receipt of tuba ligation among West Virginia Medicaid patients. A retrospective study was performed evaluating Medicaid patients in West Virginia requesting postpartum sterilization from May 2012 to November 2014 (N = 153). Demographic information was obtained on these patients. Clinical factors complicating pregnancy were analyzed. For those patients choosing not to undergo tubal sterilization, contraceptive plan at discharge was also evaluated. Statistical tests included Chi square and Fisher’s Exact Test where appropriate. 71% (n=96) received tubal ligation before discharge. 21% (n = 29) requesting post-partum tubal ligation went unsterilized. Women with a history of cesarean section (83% vs. 61%; p = <0.0007) and those delivering by cesarean section rather than vaginal delivery (86% vs. 55%; p = <0.001) were more likely to receive tubal ligation. All other factors did not significantly influence the completion rate for tubal sterilization.
