Personality not only marks an impression on others but also increases self confidence. Various studies have been conducted worldwide and have concluded that perception of aesthetics is completely subjective and is influenced by an individual’s choice based on age, gender, caste, region, social culture, level of education, profession, etc. But the fact cannot be denied that a charming smile can open the doors to new opportunities, success and adds to the wellbeing of a person. Studies have been continuously evaluating different aspects of smile design. Still it seeks an extensive review and proper compilation of available data considering different factors which probes the raveled aspects and draws guidelines to conduct a research to further address unanswered questions. Summary: Literature was reviewed by electronic search of Pubmed and Google Scholar. The review aimed at (1) highlighting the difference of perception between layperson and professionals (2) To assess the factors such as colour gradation, tooth form, buccal corridor triangle, gingival placement etc. Numerous articles were retrieved using specific keywords amongst which selected articles were reviewed based on inclusion criterias. Conclusion: This review complies and concludes the gist of perception, satisfaction or the dissatisfaction according to patients’ psyche and aims to help clinician to plan treatment accordingly to match the blend of demands, aesthetics and functions in anterior dental aesthetic zone. DOI: 10.4103/0972-4052.306418 Implant Abutment Junction: Demystified Dr. Pradyumna Kumar Sahoo Institute of Dental Sciences, Bhubaneswar Introduction: Implant abutment connection is crucial synapse between the implant and abutment and responsible for transmitting of occlusal forces from prosthesis down to the implant fixture. The effect of implant abutment connection to the success of implant therapy was very less understood. Summary: The original external hex system (Branemark system) was used for many years, however various complications like screw failure and abmormal crestal bone loss were observed .Even single piece implants which had no such connection, were not successful in all cases. With the advent of time , platform switch concept came into existence which revolutionised the implant abutment junction systems . Various internal hex systems were also introduced each having its own advantages and disadvantages. The stress load performance , mechanical strength , microgaps, effect on biological width and crestal bone loss was different for various systems . A total of 40 discs measuring 15mm in diameter and 1mm in thickness were prepared with each group consisting of 10 discs each. The translucencies of the specimens were determined by calculating the contrast ratio. Data was recorded using a spectrophotometer. The contrast ratios were calculated using the following equation:CR=Yb/Yw. Result: The median CR value was the highest for Group 2 followed by Group 4, Group 3 and Group 1 in that order. In general, lithium disilicate was found to be more translucent than all the tested brands of translucent zirconia. Conclusion: 1.All the translucent zirconia systems were less translucent than Lithium Disilicate. 2. Lava Plus HT and ZENOStar were found to be more translucent than Ceramill Zolid. 3. There was no statistically significant difference between the translucencies of Lava Plus HT and ZENOStar. The purpose of this study was to evaluate correlation between lip form (according to a new classification given by Kim et al in 2017) and incisal display with lips in repose in young Indian population. Also to find most common type of Lip form present in young adults. Methodology: A total of 200 young voluntary subjects between age group 18-30 years were chosen randomly for the study. Facial photographic records were taken with lips in repose. Incisal display was measured manually by using digital Vernier calliper. Then type of lip form was classified using Image J software and also incisal display was measured digitally by the same software. Result: Out of 200 samples, 44 were males and 156 were females. The most common type of lip form was straight followed by moderate and high lip forms. The average incisal display in mm was 3.545 in females and 2.473 in males. A statistically significant difference was found between incisal display and lip form in female candidates. Conclusion: Most common type of lip form reported in young Indian population is straight lip form. Lip form can be used to determine incisal display in anterior restorations/prostheses. A harmonious occlusal relation allows the prosthesis to function harmoniously without disturbing the stomatognathic system resulting in less number of temporomandibular disorders, bruxism habit, trauma from occlusion and other associated diseases. Summary: Over the years various modalities have been used to evaluate occlusion such as waxes, articulating papers, foil/ film/silk strips and newer materials such as the T-Scan, dental prescale and virtual dental patient. The reliability, sensitivity, marking ability and method of use are different and based on their type and character. Factors like oral environment, plastic deformity and interpretation of clinician is also a contributing factor. Hence a thorough knowledge about the various available occlusal indicator methods/modalities, their method of usage, understanding of interpretation & limitation is essential prior to their usage. This review will give insight to various modalities commonly available discussing evidence and practicality of their application. The clinical challenge is to prevent the consequence of occlusal discrepancies by making appropriate, evidence-based minimum adjustments to deliver a realistic treatment with reliable outcome. Cleft can manifest alone or in combination of cleft lip or palate which can either be unilateral or bilateral. Alveolar cleft is a common finding with concomitant occurrence in 75percent of total cases, an incidence of 1.8 to 2.5 in 1000 live births. Alveolar cleft can either derange normal teeth eruption pattern or lead to tooth agenesis with maxillary lateral incisor being the most commonly affected tooth. Missing teeth in cleft region are either orthodontically camouflaged or prosthodontically rehabilitated which conventionally includes use of removable or fixed partial dentures. Both these rehabilitations can cause potential damage of adjacent structures with esthetic shortcomings. Summary: Use of dental implants in grafted cleft site is an encouraging modality helping in overcoming these obstacles. However, to undertake this treatment, it is important to know appropriate time after grafting when implant can be placed. This paper aims to determine an appropriate time gap between grafting and implant placement in alveolar cleft patients via a systematic review of literature published between January 2011 and February 2020 (Via electronic search). Conclusion: Tertiary bone grafting is frequently required despite a positive history of previous grafting procedure. Successful implant-based rehabilitation was reported irrespective of the followed method. More studies, preferably in form of control trials, with detailed descriptions in terms of data acquired and procedural difficulties are required to sensitize clinicians about probable timelines and treatment outcomes. This will help in developing a universally acceptable protocol, making future comparisons easier and comprehensive.
