The Dawson’s bimanual technique is a time tested and established method to guide the mandible into CR. On the other hand the author of this study also has copyrighted a technique (wax ball orientation technique©) to guide the mandible into CR. Hence, this methodological study was performed to compare the two CR techniques to guide the mandible in CR position using clinical, myographical and radiological assessment methods. Aim: To compare and assess which amongst the two (Dawson’s bimanual technique and wax ball orientation technique) CR guiding techniques best guides the mandible to CR position using clinical, electromyographical and radiological assessment methods. Materials and Methods: This interventional and clinical study was conducted at School of Dental Sciences, KIMSDU, Karad, Maharashtra, India, from May 2019 to May 2020. The study included healthy dentate individuals having Angle’s class I malocclusion without any occlusal interferences and an intact dentition except for 3rd molars. Two CR technique to guide the mandible were technique 1-Dawson’s bimanual technique and technique 2 - wax ball orientation technique. The study was carried out in three phases: clinical, electromyographical and radiological. Difference between the centric points, workload on elevators, condylar position was statistically analysed. Descriptive statistics was used to analyse the difference between the centric points marked using both the techniques. Independent t-test was applied to compare the gender and the mean centric distribution. Results: Among the 45 study subjects considered for the study, 32 were females and 13 were males with the mean age of 21 years. No statistically significance difference was observed between the two techniques. Paired t-test showed that workload on elevators of both sides, Right Masseter in technique 1 and 2 had p-value= 0.088, left Masseter in technique 1 and 2 had p-value= 0.3, Right temporalis in technique 1 and 2 had p-value= 0.463, left temporalis in technique 1 and 2 had p-value= 0.429. There was difference between the Anteroposterior (AP) and Superoinferior (SI) position of condyle in relation to the fossa. AP measurement in the right side in tech 1 and 2 with p-value= 0.448, AP measurement in the left side in tech 1 and 2 with p-value= 0.178, SI measurement in the right side in tech 1 and 2 had p-value= 0.803, SI measurement in the left side in tech 1 and 2 had p-value= 0.259, which were statistically insignificant. Multivariate test showed statistically significant difference between gender and EMG, results whereas it was insignificant in Magnetic Resonance Imaging (MRI). All other implant positions showed stresses less than 2.46 MPa indicating increasing trend in stresses with increase in the distance. Highest stresses of 0.93 (unilateral loading) and 0.92 MPa (bilateral loading) were observed with implants at 15/15-degree angulations. For lesser angulations, the stresses were ranging from 0.05 to 0.87 MPa on implants. Conclusions: Implants placed more posteriorly exhibit higher stresses compared to implants placed more anteriorly. Implants placed with greater angulations displayed higher stresses than implants placed with lesser angulations. DOI: 10.4103/0972-4052.306372 Knowledge, attitude and practice of dentists regarding provisional restorations: A cross sectional study Dr. Saee Deshmukh, Dr. Karan Jaiswal VSPM Dental College and Research Centre, Nagpur Introduction: The importance of provisional restorations in fixed partial denture can not be overemphasized. At the same time it is very important to understand the dentists’ opinions and practices regarding the same. Methodology: A descriptive cross-sectional study was done amongst the dental practitioners of Nagpur region in 2019. A questionnaire with total 11 questions including open and multiple-choice type was designed and validated by experts in the field. It was circulated using Google forms as well as printed version to a total of 100 dentists selected randomly from private dental clinics and dental institutes. Data from the completed questionnaires were analysed using the SPSS Statistical Software Package (version 25). Data was expressed in terms of frequencies. Results: The response rate was 79%. In the given sample, 81% of dentists preferred giving provisional restorations in selected cases. 68% of them give provisionals in special cases like full mouth rehabilitation and laminates. Out of these, 61% use self polymerized acrylic resin, 34% use bis-acryl composite resin (Protemp IV,3M) and 5% use heat polymerized resin as material of choice. 46% use indirect technique, 20% use direct technique and the rest use combination technique. Gingival inflammation was most commonly encountered problem by 51% of the dentists. 54% of the dentists preferred to give the provisionals for a period of 7-15 days. Only 23% felt the need to perform diagnostic wax up prior to giving provisionals. Conclusion: In the given set of participants the level of knowledge and practices regarding provisionalization in fixed prosthodontics was found to be satisfactory. However, to make them more proficient continuing dental education programmes on recent advances in terms of newer materials and techniques should be conducted. Result: A comparative assessment of amplitude (indicating electromyographic activity of the masticatory muscles) between osteoporotic and non-osteoporotic groups revealed lower values in osteoporotic subjects than normal subjects for both Masseter and Temporalis. The results were statistically significant (Masseter p=0.005 and Temporalis p=0.03). Conclusion: Osteoporosis may produce significant adverse effects on the masticatory function of complete denture wearers. As a future prospect, we might allow for proper treatment protocol to be followed to improve the bone mineral density of the osteoporotic patients and evaluate any changes in electromyographic activity in edentulous patients. A Pilot randomized controlled trial Dr. Sachin Haribhau Chaware Mahatma Gandhi Vidya Mandir’s Dental College & Hospital, Nashik Introduction: Speech errors and associated oral sensory difficulties in ASD subjects are principally related to lack of synchronization between oral sensory –motor receptors, and incomplete motor planning. However, current speech therapy works on imagination and simulation. Hence, there is a need of the SSD which stimulates an oral sensory-motor receptors for better speech and decrease proportion of associated oral sensory difficulties. Methodology: The study was conducted by randomly selecting 10 ASD children between the age group of 4-15 years. The five children received routine speech therapy and in other five children, SSD device was used followed by speech therapy. Speech therapist was blind for which child used SSD device. SSD device is a programmable microcontroller vibrator device. it comprises of a plurality of vibrators in order to create multiple vibrations of vibration patterns for stimulation of oral sensory region. Each vibrator is connected to pulse width modulation pin of the microcontroller which allows frequency of vibrator to desired range. Pre-speech recording before therapy and post-speech recording after therapy with recording graph in both the subjects have been used for comparison. Result: ASD subjects showed more clarity is speech particularly articulation and phonological error by using the SSD device followed by speech therapy than speech therapy alone. Associated sensory difficulties such as inappropriate feeding behavior, biting on object, attrition, and drooling saliva reported with less proportion in SSD device group . Conclusion: SSD device improves the speech articulating and phonological errors, improve feeding and eating behavior, reduces the severity of attrition and biting on object.
