Radiological studies confirmed an obstructive mass lesion in the hepatic flexure of the colon. Due to bowel obstruction and suspicion of malignancy an emergency laporatomy and partial ileo-colectomy was performed. A complex polypoid mass comprising numerous slender straight finger-like villi lined by non-neoplastic colonic mucosa with serrations at the base and prominent lamina proprial smooth muscle splaying was identified and a diagnosis of filiform polyposis was made. We report this unusual manifestation of an inflammatory reaction presenting as a colonic mass clinically and radiologically mimicking malignancy.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"46 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114025565","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The high proportion of epithelial neoplasms in the lung is easily explained by the large surface area of the lung and the continued renewal of its epithelial lining and the exposure to carcinogens. In contrast, the supporting tissues only make up a small fraction of the lung parenchyma and their renewal turnover is low. This is especially relevant in the interstitial alveolar tissue of the gas-exchange compartment where the amount of connective tissue is minimal. Thus, primary mesenchymal tumours in the lung are very rare and most commonly arise in the larger airways. Most tumours with morphological mesenchymal phenotype (pure or mixed) are likely to be of epithelial derivation.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"18 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132739020","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

According to National Cancer incidence data thyroid malignancies were the 3 commonest malignancy in women in 2010. In men, thyroid malignancies were placed 3 among the age group 15-34 years and 5th among 35-49 years [2]. Thyroid fine needle aspiration cytology test (FNAC) is a front line investigation to assess the possibility of a neoplasm in patients presenting with thyroid nodules. The original ‘Thy’ classification of the British thyroid association of the Royal College of pathologists of United Kingdom (RCP UK) formed the basis for thyroid FNAC reporting in Sri Lanka, since 2007[3]. In May 2014, the College of Pathologists of Sri Lanka initiated a dialog with the clinical stakeholders regarding issues related to the ‘Thy’ based FNAC reporting. Following discussions at a workshop attended by representatives from the Colleges of Pathologists, Radiologists, Surgeons and Endocrinologists, the Bethesda system (TBSRTC) was adopted for thyroid FNAC reporting in our setting [4]. Reporting the risk of subsequent malignancy (ROM) for each category was however not adopted till the availability of local data. The hand book titled ‘Thyroid Cytology Reporting. National Guidelines for Sri Lanka’ (NGSL) based on TBSRTC was published by the Ministry of Health in 2016 [5]. Currently NGSL forms the frame work for thyroid FNAC reporting in our setting. Some concerns have since been expressed regarding TBSRTC based NGSL guidelines.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"58 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130661584","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

TME results in a lower recurrence rate than the traditional approaches such as the abdominoperineal resection. The circumferential resection margin positivity rate is about 5% or less for low anterior resections with TME, whereas it is between 10% and 25% for abdominoperineal excision of the rectum [2]. There is understandably, a higher local recurrence rate following the traditional methods of surgery for rectal cancer.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"93 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-12-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116571009","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Nodal status and many other important pathological features have a significant impact on tumour prognosis and therapy. The aim of this study was to determine the total number of lymph nodes (LNs) retrieved from PD specimens, whether grouping of LNs improves the total yield and to assess the level completeness of histopathology reporting of PD specimens. Forty two PD requests and histopathology reports  were assessed to determine the total number of LNs retrieved and whether the LN were grouped (G)or non-grouped (NG). The significance of difference in the number of LNs in the two groups were assessed using the Wilcoxon signed rank test. The tumours were subcategorized as AVT, DBDT, DT and PT and the reports were audited against the respective minimum data sets of the Royal College of Pathologists of United Kingdom to determine the overall completeness and the parameters poorly reported in the reports. The overall median LN yield was 14.5 and the median LN yield was 15 and 10 in G and NG respectively which was statistically significant. The completeness of the histopathology reporting was 63.6%- 77.3% in AVT (n-18), 73.9% - 95.6% in DBDT (n-5), 68.1% - 90.1% in DT (n-8), 70.8% - 83.3% in PT (n-11). The lengths of the bile duct, lesser and greater curvature of the stomach, tumour differentiation, involvement of resection margins and named blood vessels were poorly reported. In conclusion, the total LN retrieval improved by grouping according to the Union of International Cancer Control (UICC) protocol. Histopathology reporting of some of the data items requires improvement. Hence adoption of a pro forma for synoptic reporting and establishment of national guidelines on reporting and handling of specimens is recommended.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"48 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-11-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128879198","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The initial MRI scan was suggestive of an infective process, while the subsequent scan revealed radiological features suggestive of an osteosarcoma in the left distal femur. The biopsy was reported as an osteosarcoma in a different institution. Following neoadjuvant chemotherapy wide local excision of the tumour was performed.  On histologic evaluation, a low grade central osteosarcoma was diagnosed with progression to high grade leiomyosarcoma. Low grade central osteosarcoma is an uncommon bone forming tumour which occasionally dedifferentiate into a highgrade sarcoma, most commonly to conventional osteosarcoma. The young age of the patient and dedifferntiation into a leiomyosarcoma are unusual features encountered in this case. Furthermore, we discuss the challenges encountered in diagnosing a low grade central osteosarcoma which has deceptively benign features radiologically and histologically.","PeriodicalId":225127,"journal":{"name":"Journal of Diagnostic Pathology","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2017-11-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134633141","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
