Materials and methods: There were considered and selected two patients who had brain tumors and had been treating in Tomsk Regional Oncology Center. All treatment plans were simulated using the Monaco treatment planning system v 5.11 (Elekta Instrument AB, Stockholm) on the Elekta Synergy linac with photon beam nominal energy 6 MV. For study there were created several plans with different technical parameters (number of arcs, increment, gantry angle, collimator angle, couch angle, minimum segment width). Each plan was evaluated by metrics such as Conformal, Homogeneity and Gradient indexes. Results: If we use a large increment value it may create few sectors and which can produce low quality plans, and increase treatment time, otherwise if we use a too small increment value it will give us more sectors and they can increase the quality of the plan. The dose which was measured with higher MSW showed to us better agreement with the calculated dose. Conclusion: It is valuable to carefully select technical parameters. The use of rigid increment value, segment width or increasing number of arcs can make plans have lower quality and to rise time of treatment.","PeriodicalId":222791,"journal":{"name":"Meditsinskaya Fizika","volume":"33 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132369016","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The market welcomed a new class of targeted DRPs. Dozens of PET centres and diagnostic rooms where DRPs with ultrashort-lived radionuclides are used for medical examination have been put into operation. All this dictates the need to modify the existing guidelines that govern this area. In 2021, there was an update in the procedural guidelines (PGs) MU 2.6.1.3700-21 “Assessment and registration of effective doses used in diagnostic nuclear medicine examinations”. The document presented the dose coefficients for new DRPs based on the latest data from the International Commission on Radiological Protection and the literature. This included calculation of the effective doses (using tissue weighting factors) from 1 MBq of radioactivity for approximately 280 DRPs used across five different age groups, as per ICRP Publication 60. These PGs apply to diagnostic nuclear medicine examinations performed using DRPs, scintigraphy, single-photon emission computed tomography (SPECT) and positron emission tomography (PET) alone or in combination with X-ray computed tomography (SPECT/CT; PET/CT). The guidelines do not apply to radionuclide therapies that involve DRP administration that require individual planning and assessment of the absorbed doses across the target regions, critical organs and other radiosensitive organs and tissues.","PeriodicalId":222791,"journal":{"name":"Meditsinskaya Fizika","volume":" 12","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132075898","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

To quantify the quality of treatment planning for patients with prostate cancer for three different radiotherapy technologies. Materials and methods. The quality of irradiation planning was assessed using the index of homogeneity, conformality and comparison of critical organs doses for 40 patients with prostate cancer using various irradiation technologies: 3D conformal radiation therapy 3DCRT, radiation therapy with intensity modulation IMRT, radiation therapy with intensity modulation in rotation mode RapidArc. The technologies are implemented on conventional electron accelerators of the Clinac iX (Varian, USA), Clinac 2300 CD (Varian, USA) and UniquePower (Varian, USA) models with a nominal photon radiation energy 6 and 18 MeV. Results. The dependences of the homogeneity indices HI, conformality CI within the target, and critical organs doses on the irradiation technology for patients with prostate cancer were obtained. Taking into account the predictive capabilities of the HI index for relapse-free survival, the optimal technology for irradiating patients was chosen. It has been shown that Rapid Arc technology dominates for prostate cancer.","PeriodicalId":222791,"journal":{"name":"Meditsinskaya Fizika","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130501851","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods: The study included 12 patients with prostate cancer of low, intermediate and high risk of progression according to NCCN. Planning for six patients was performed using the coplanar VMAT technique. For the second group, also of 6 patients, non-coplanar VMATs were used, where out of two full arches only one full arch (FULLARC 1), gantry 181°-179° CW, couch 0 was used. Results: Implemented plans in the study method showed that the use of non-coplanar VMAT therapy fields can significantly reduce doses in OAR, in particular, the reduction in the average dose to the femoral heads was 46.6 %. Conclusion: This study has demonstrated the potential for a non-standard approach to patient treatment with non-coplanar VMAT treatment planning.","PeriodicalId":222791,"journal":{"name":"Meditsinskaya Fizika","volume":"119 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133255813","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The principle of optimization, the most important and effective one, relies on the system of diagnostic reference levels (DRLs) that have been successfully used in global practice and are officially introduced in Basic Sanitary Rules for Radiation Safety-99/2010. \u0000The guidelines establish diagnostic reference levels for adult X ray imaging in medical settings and determine their application to balance radiation protection of patients and the provision of high-quality diagnostic information. The guidelines relate to medical X-ray diagnostic and radiological examinations and are intended for the staff and management of medical facilities, agencies that are responsible for radiation control in medical facilities, and can also be used by expert institutions and agencies for health security and disease control. \u0000The guidelines consider and compare various methods to determine the DRLs based on measured and/or calculated dosimetric parameters, such as the dose-area product, entrance dose, mean glandular dose, activity, and effective dose. Furthermore, they propose a scheme to determine the standard dose values depending on the type of X-ray diagnostic device used; present the steps to apply the DRLs in practice; establish measures in case the standard doses exceed the DRLs; and define the levels to perform optimization.","PeriodicalId":222791,"journal":{"name":"Meditsinskaya Fizika","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128922841","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
