There is very limited data regarding the need for transfusion and its effect on the overall mortality of patients with coronavirus disease 2019 (COVID-19). The aim of our study is to determine the need for blood component transfusion in patents treated for COVID-19 infection. Methods. This retrospective observational study included 4,426 COVID-19 positive patients, who were treated, in the period between June 23, 2020, and May 2, 2021, at the Bezanijska Kosa University Hospital Medical Center. Of these patients, 826 were treated in the intensive care units of the hospital. Of the total number of patients, 326 (7.4%) received transfusions. The clinical presentation, the structure of the applied transfusion therapy, the laboratory parameters, and the treatment outcome were analyzed in this study. Results. Of the 828 patients treated in the intensive care units, 151 (18.2%) patients required transfusion, while transfusion was necessary in a total of 4.9% of patients treated in the hospital wards. Of the total number of all transfused patients, 86% received erythrocytes, one third of them received fresh frozen plasma, 10% received cryoprecipitate, while platelets were administered in around 6% of the patients. The mortality rate in the tested group was 46%. Conclusion. The frequency of the application of blood components was significantly higher in patients with a severe form of the disease. The presence of comorbidities did not affect the need for transfusion therapy. Echinococcus multilocularis causes multilocular or alveolar echinococcosis, which differs from infection caused by Echinococcus granulosus in clinical presentation in humans. The most common definitive hosts for E. multilocularis are foxes and jackals, while domestic mammals like dogs and cats are rare. Humans are rare and accidental intermediate hosts. Cystic echinococcosis in humans is endemic in Serbia, while more severe alveolar echinococcosis has not yet been recorded. Case outline. We present a case of a 67-year-old female from a small village in the Sremska Mitrovica municipality. The onset of symptoms was several years ago, with liver pain which progressed over time. Differential diagnoses included benign liver tumors like haemangioma, cystic echinococcosis and abscess formed in the cystic echinococcal lesion. Left lateral hepatectomy was performed, and S II/III liver segments were removed. Pathological examination showed numerous small empty vesicle spaces with chitin membrane without protoscolices, surrounded by massive fibrosis and infiltrative growth into the liver parenchyma, all indicative marks of multilocular echinococcosis. Surgical margins were found positive for echinococcal vesicles showing that echinococcal tissue was not completely removed. Thus albendazole therapy was introduced. Epidemiological interview revealed that the patient lived in an endemic region of multilocular echinococcosis, in a house with two hunting dogs and backyard where contamination of soil with fox feces could occur. Conclusion. Osteosarcoma (OS) is the most common primary malignant bone tumor in adolescents and young adults, with a tendency to produce variable amounts of osteoid, cartilage, and fibrous matrices. The objective of this study is to differentiate between osteosarcoma subtypes osteoblastic and chondroblastic according to their magnetic resonance (MR) signal intensities and X-ray findings. Methods. We performed a retrospective analysis for 21 pathologically proven osteosarcoma subtypes: osteoblastic (n = 14) and chondroblastic (n = 7). Conventional images of the bone of origin, periosteal reactions, lytic and sclerotic features, the presence of calcification, and pathological fractures were investigated with X-rays. We measured the mean ROI values for each lesion with MRI sequences. Results. Among the osteosarcoma lesions, 57% were localized at the knee. X-ray evaluations of the osteoblastic osteosarcomas revealed pure lytic lesions in 35.7% and pure sclerotic lesions in 35.7%. Chondroblastic osteosarcomas revealed pure lytic lesions in 14.3% and pure sclerotic lesions in 42.9%. Due to variable osteoblastic, chondroblastic, and fibroblastic areas and proportions of the ossified matrix, osteosarcoma lesions have a heterogeneous MR signal. However, no statistically significant value was detected. Conclusion. Homicide, a major public concern, has always attracted the attention of criminology, psychiatry, psychology, and other related disciplines. The objective of this study was to determine the frequency and type of mental disorders in 94 attempted-committed homicide perpetrators. Methods. The authors conducted a psychiatric assessment of all perpetrators based on psychiatric interviews, psychological testing, and the examination of available medical records. Results. The key findings of this study imply that there is a large percentage of violent crime perpetrators with mental disorders (62%). When we excluded people with personality disorders from this group, we found that the most common major mental disorders among the perpetrators were psychosis and alcohol use disorders (approximately 10% each). Conclusion. The results highlight the importance of the early identification and treatment of people with mental disorders in the general population, as this could reduce the possibility of criminal behavior. Aim of this study was to determine total number of patients, their characteristics, frequency of symptoms, the manner of disease manifestation and the success of therapy. Methods Study included 21 patients with diagnosis FRS made according to the 2012 EPOS guidelines. Results Based on VAS(visual analogue scale), feeling of localized pressure, ie. facial pain was dominant in 9.57 ? 0.98, than the secretion from the nose is 8.14 ? 1.62, problem of the breathing on the nose in 6.67 ? 3.25, while reduction the sense of smell was in 2.14 ? 3.00. T-test showed there is a statistically significant difference between mucosal changes on the diseased and healthy sides of the patient's face (p <0.0001). Intraoperative in 18 (85.71%) patients only one sinus was affected. The most commonly affected sinus was maxillary, in 13 (54.17%) cases, followed by sphenoid sinus in 5 (20.83%) cases. As a cause of rhinosinusitis Aspergillus was proven in 12 (57.14%) patients. Conclusion The dominant symptom of patients with fungal rhinosinusitis was localized pain / pressure in the area of the affected sinus. Endoscopically, on the side of the affected sinus, the pathological mucosa with thick, pithy, mucous secretion dominated. The maxillary sinus is primarily unilaterally affected, in more than half of the patients. Rheumatoid arthritis is a systemic autoimmune disease with inflammation of the joints as its hallmark. Extra-articular manifestations affect nearly half of the patients either at the onset of disease or later during the disease course. Case outline. A 43-year-old man complained over a chest pain, dry cough and fatigue. Diagnosis of pericarditis was made based on echocardiography findings. Due to worsening of respiratory symptoms he was admitted to the hospital. Initial diagnostic workup revealed elevated concentrations of acute phase reactants, pericardial effusion and bilateral pulmonary nodules. Pathohistological analysis of lung nodules ruled out malignancy and tuberculosis. He was treated with colhicine which led to a regression of a pericardial effusion. Afterwards, due to arthritis of the right wrist, high erythrocyte sedimentation rate and C-reactive protein, positive immunoserology and bone erosion at the distal ulna diagnosis of seropositive rheumatoid arthritis was established. He was treated with antimalarial, methotrexate and glucocorticoids until suffering from COVID-19 pneumonia which triggered arthritis flare. Owing to the loss of efficiency of combination therapy with methotrexate and glucocorticoid, baricitinib was added to the treatment. Low disease activity was achieved after 3 months of administering baricitinib and methotrexate, and no adverse events occurred during 20 months of this therapy. Conclusion. Nursing Activities Score (NAS) is one of the most accepted and widely used scores for assessing the workload of the nursing staff in regards of qualitative and quantitative adequacy. The aim of this study was to evaluate and analyze nursing workload in cardiac surgery intensive care unit (CSICU) using the NAS in a contemporary set of patients undergoing heart surgery. Methods. The study included 809 consecutive patients admitted to adult CSICU who had major cardiac surgery during the year of 2019. Demographic data were collected from medical records (gender, age, type of treatment, length of stay [LOS], and discharge), EuroSCORE II and NAS value. Results. The majority of patients underwent coronary (43.1%), valvular (32.6%) and combined (24.2%) cardiac surgery procedure. The average patient LOS in CSICU was 2.5 ? 3.4 days. The average NAS value in our sample was 100.8% ? 63.1%. NAS value during the first operative day was a poor marker of the outcome in terms of mortality (C-index 0.520, 95% CI - 0.422-0.617, p=0.676). Significant difference was observed in terms of average NAS value between the patients submitted to coronary surgery and combined surgery (p = 0.001). NAS has been shown to be useful for assessing activity in a CSICU, confirming the optimal workload of nurses, while higher NAS values in our hospital indicate increased workload compared to similar institutions. Conclusion. NAS provided viable information regarding the care and hospitalization of patients in a CSICU. Ileal leiomyosarcoma is unusual form of malignant gastrointestinal tumor. Often insidious in clinical presentation, it frequently presents a diagnostic challenge. Occasionally, a correct diagnosis is finally established due to emergency situation. The aim of this study is to present the role of magnetic enterography in determining the precise cause of small bowel dilation. Case outline. A 59-year-old female patient presented with small bowel obstruction. Erect abdominal radiograph identified the presence of small bowel obstruction and excluded pneumoperitoneum. Non-contrast computed tomography of the abdomen and pelvis noted transitional zone in the region of terminal ileum with collapsed bowel lumen distal to the transitional point, without determined underlying cause. Magnetic resonance enterography observed obstructive intraluminal soft-tissue mass with fatty component sized up to 4 cm in terminal ileum, with mesenteric involvement. The abdominal surgeon revealed ileal intraluminal tumor which affected the locoregional mesentery and serosa of the adjacent bowel. Histological and immunohistochemical analysis confirmed the diagnosis of ileal leiomyosarcoma with involvement of wall serosa and mesenteric fat tissue. Conclusion. Magnetic resonance enterography is a reliable diagnostic tool for detection and diagnosis of malignant small bowel tumors. Sometimes, tumors manifest clinically as bowel obstruction. Gojnić","doi":"10.2298/sarh221021012d","DOIUrl":"https://doi.org/10.2298/sarh221021012d","url":null,"abstract":"Metabolic changes occur due to the effects of placental hormones such as human chorionic gonadotropin and human placental lactogen in normal pregnancies. These effects enable the development of insulin resistance among all pregnant women that is them ost significantly pronounced in the third trimester. In pregnancies complicated with pre-gestational or gestational diabetes mellitus these changes are more intensive as they affect fetoplacental unit. In pregnancies complicated with diabetes the increased number of placental macrophages leads to the increased production of different cytokines which include leptin, tumour necrosis factor alpha and interleukins. Ovarian carcinomas are the deadliest gynecological tumors. Despite advances in treatment options, survival rates are still not at the desired level. Since clinical signs are not typical in early-stage disease, 2/3rd of patients are diagnosed late. Carbohydrate Antigen 125 (CA125) does not have sufficient sensitivity and specificity in early-stage disease and early post-relapse progression. There is a need for a simple and cost-effective marker that correlates with CA125. For this purpose, we aimed to evaluate the potential of systemic inflammatory markers? as diagnostic aids. Methods. Patients with platinum-sensitive recurrent ovarian carcinomas were preferred because the treatment options were more diverse than the resistant group. Using retrospective data collection, 105 patients with platinum-sensitive recurrent ovarian carcinoma, admitted in the last four years were included in the study. Complete blood count data was recorded based on recurrence and progression periods. Results. When the systemic immune inflammatory index (SII) values were evaluated in combination with CA125 in terms of progression during the control visits after platinum-sensitive disease recurrence treatment, progression detection proportions increased to 97.5%, which was 82.9% when only CA125 was used. On the other hand, false positivity, which was 18.5% for CA125 alone, decreased to 2.5% when combined with SII. Furthermore, neutrophil lymphocyte ratio, WBC, and neutrophil values showed correlations with high CA125 values. Conclusion.
