Tsen","doi":"10.4172/2161-0932.1000502","DOIUrl":"https://doi.org/10.4172/2161-0932.1000502","url":null,"abstract":"Introduction: Cephalocentesis is a procedure that can be utilized to drain excessive Cerebrospinal Fluid (CSF) from a fetus with hydrocephalus along with severe associated abnormalities incompatible with survival or in a nonviable fetus with hydrocephalus in order to facilitate normal vaginal delivery and avoid maternal morbidity due to Cesarean delivery.Case Presentation: We present a case of a 36-year-old primigravida who had a fetus with severe hydrocephalus complicated by intrauterine fetal demise at 36 weeks of gestation, requiring cephalocentesis to facilitate vaginal delivery.Conclusion: Cephalocentesis can be a valuable procedure in avoiding cesarean delivery and related morbidity in situations where there is a nonviable fetus or fetus with very poor prognosis of survival after birth with hydrocephalus causing obstructed labor. Objective: To assess the effects of hyoscine butylbromide for cervical rigidity in the first stage of labor Study design: randomized, placebo-controlled, double-blind, parallel, pre-post clinical trial. Pregnant women delivering at University Hospital Germans Trias i Pujol in Badalona, Barcelona between January 2013 and January 2018 were eligible for inclusion. Our calculated sample size target was 70 participants 35 in each group with a 95% confidence level, an alfa and beta level of 5% and 80% power. The intervention group received 40 mg of hyoscine butylbromide intravenously, while controls received a placebo drip. Primary outcomes were: duration (minutes) of the first stage of labor, duration (minutes) from intervention to complete dilation and changes in cervical rigidity. We also collected data on maternal and neonatal variables. Results: Seventy-one women were included: 47 (66.2%) were nulliparous, and 35 (49.3%) had a spontaneous onset of labor. Fifty-seven (80.3%) women had vaginal deliveries: 37 (52.1%) were eutocic, 7 (9.8%) were assisted by obstetric vacuum, and 13 (18.3%) with forceps/spatulas; 14 (19.7%) were cesarean deliveries following complete dilation. Mean duration of the first stage of labor was 48.3 minutes shorter in the experimental group compared to the control (p=0.287), and mean time from intervention to complete dilation was 63.3 minutes shorter in the experimental group than control group (p=0.084). Methods: We carried out an analytical cross-sectional study with retrospective data collection from December 1st, 2017 to May 1st, 2018. We compared two groups of patients undergoing ICSI using ejaculated sperm: group I comprised of those in whom fresh semen was used and group II was comprised of those in whom cryopreserved semen was used. We collected the fertilization rate, embryo quality, and pregnancy rate. The Odds Ratio was calculated to determine the association between the variables. A p-value of <0.05 was considered significant. A logistic regression analysis was done. Results: when OATS sperm was used, we had higher fertilization rate 67, 5% vs. 46, 9%, p=0,042 and better embryo quality on day 3, 95% vs. 5%, p=0.008 with freshly ejaculated sperm against the cryopreserved semen. After logistic regression, the fertilization rate, embryo quality and pregnancy rate in the fresh and cryopreserved ejaculated semen were similar. The operative intervention itself is amid an elevated occurrence of post-surgical complications and post-operative pain and an extended recovery time. Moreover, the conditions during which takes place most of the caesarean sections are liable for an altered patient self-esteem and difficulties in creating the bond between the mother and her baby. We aim to present here the fruit of collaboration between French obstetrical surgeons dedicated to improve the patient’s experiences during the procedure and to facilitate the encounter between the mother and her baby. This approach includes specific environmental, psychological and technical aspects which will be sum up in 7 principles: (1) to determine during the pregnancy an idea birth adapted to the birth by cesarean delivery , (2) to permit the partner to be present at the time of cesarean delivery , (3) to specialise in the requirements of the baby and therefore the mother at every step of the procedure, (4) to assist the mother to move during the method , (5) to propose immediate and extended skin to skin contact, (6) to use a minimally invasive technique of cesarean delivery like the modified extra-peritoneal approach and (7) to facilitate the creation of the bond between the mother and therefore the baby. {"title":"The Reproductive Outcome in patients having Primary Infertility or Recurrent Miscarriages after performing Hysteroscopic Septal Resection","authors":"A. Abdou, M. T. Abdelfattah","doi":"10.4172/2161-0932.1000472","DOIUrl":"https://doi.org/10.4172/2161-0932.1000472","url":null,"abstract":"Objective: To assess the reproductive outcome after hysteroscopicseptal resection in women with unexplained infertility or recurrent miscarriage.Design: Prospective clinical trial.Setting: Zagazig university hospitals, Egypt.Patients: 47 patients (20 patients with primary infertility and 27 patients with recurrent miscarriage diagnosed to have uterine septum)Intervention: Hysteroscopicseptal resection.Main Outcome Measures: Pregnancy rate and its outcome (miscarriage, preterm birth, term birth and live-birth rates).Results: There was significant improvement in reproductive outcome in patients with primary infertility and uterine septum after hysteroscopicmetroplasty; pregnancy rate was 55% (11 pregnancies out of 20 patients). Abortion and preterm labor rates were low (5 and 10% respectively) while term delivery and live-birth rates were high (40 and 45% respectively).Also, there was significant improvement in the reproductive outcome in patients with recurrent pregnancy loss, there was highly significant reduction in the abortion rate to 11.1% (p=0.00) after hysteroscopicmetroplasty but there was non-significant reduction in preterm birth from 11.1% to 7.4% (p=0.63). There was highly significant rise in term delivery rate that increased from 3.8% to 51.9% (p=0.00007) and highly significant increase in live-birth rate from 7.4% to 55.6% (p=0.0001).Conclusion: Performing hysteroscopicmetroplasty could markedly improve the reproductive outcome in patients having recurrent miscarriage in the form of reduction in abortion rate and increase in term delivery and live-birth rates. {"title":"NK-Cell Count and Its Function in Producing Interferon Gamma Associated with the Cervical Cancer Natural History","authors":"T. We investigate the NKcell count and its function in producing IFN-γ on normal cervix, subclinical high risk-HPV (hr-HPV) infection, precancerous lesion of cervix, and cervical cancer in order to understand the natural history of uterine cervical cancer. \u0000Methods: This is a descriptive comparative numerical study with more than two unpaired group with total subjects of 40 females who met the inclusion and exclusion criteria. Samples were gathered from cervical tissue using cytobrush and were processed in the laboratory to calculate NK-cell count and NK-cell expression of IFN-γ by phyta hemagglutinin (PHA) stimulation using flow cytometry. Data was analyzed using Kruskal Wallis and post hoc analysis was done to determine the difference between groups. \u0000Results: The mean NK-cell count on normal cervix, subclinical hr-HPV infection, precancerous lesion, and cervical cancer were 2.6%, 11.6%, 12%, and 7.4%. The mean NK-cell producing IFN-γ on normal cervix, subclinical hr-HPV infection, precancerous lesion, and cervical cancer were 8.1%, 3.3%, 1.1%, and 1.8%. There is significant difference of NK-cell count between 4 groups (p=0.001) but there is no significant difference of NK-cell producing IFN-γ (p=0.577). \u0000Conclusion: NK-cell count on normal cervix was significantly lower than other group but it has highest expression of IFN-γ after stimulation. On the other hand, precancerous lesion and cervical cancer has lower expression of IFN-γ after stimulation. Eventhough statistically there is no NK-cell activity difference, this study shows that NK-cell activity in disease groups were impaired. Further study is needed to understand NK-cell activity and its role in cervical cancer natural history. Nowdays, NK-cells as immunotherapy are potential tools for cancer therapy. The specific objectives were to analyze evolving trends and propose solutions to improve the quality of obstetric care. Methodology: This was a descriptive cross-sectional study of all obstetric obstetric attendants in the period from January 2011 to December 2015.Results: We recorded 25,363 inmates in the delivery room with an annual average of 5017. The deliveries accounted for 22,975 entrants, or 90.58%. Caesareans were 4083 cases, or 17.7% of deliveries. Obstetric evacuation accounted for 3845 cases, or 16.3% of obstetric admissions. Stillbirth was 6.5 per 100 live births. We recorded a maternal mortality rate of 547.08 per 100,000 live births. The causes were direct in 70% of cases and indirect in 30%.Conclusion: The results of this work confirm a fact: the regional hospital center of Saint Louis because of its geographical position is a center of reference in the system in northern Senegal. The availability of SONUC has improved maternal and neonatal health indicators in the area. At the end, collected data were analyzed. \u0000Results: A total of 62 doctors participated in the survey, ~ 90% of them had ≥ 5 patients using Palomacare®. 98% of survey respondents indicated the overall satisfaction with Palomacare® in their patients is good or excellent. On the other hand, 84% of them stated that patients considered the product better or much better than previous treatments, respectively. \u0000Conclusion: After analyzing the data we assured that Palomacare® is a well-tolerated product with a comfortable application. Fetal growth is regulated at multiple levels. Besides maternal disorder, fetal structural and chromosomal anomalies are the added factors. The morbidities associated with IUGR could have long term implications in adult life which predisposes in a development of a number of chronic diseases. \u0000Objectives: To identify the various risk factors for IUGR and its neonatal outcome. \u0000Methodology: It is a prospective study conducted in the Department of Obstetrics and Gynaecology, College of Medical Sciences, Chitwan, Nepal in which total of 60 cases of clinically diagnosed IUGR were enrolled during the study period of 2 years. \u0000Results: The study revealed maximum number of cases (38.3%) belonged to age between 26 to 30 years. IUGR was common in Multigavida (75%), rural area (78.3%), lower socioeconomic status (63.3%) and in manual worker (56.7%). Maternal (41.66%) was the commonest followed by Placental (16.66%) and Fetal (1.66%) causes. IUGR was observed in 43.3% with normal AFI and severe oligohydraminos <5 cm was observed in 21.7%. Doppler velocimetry showed abnormal umbilical S/D ratio in 2 (15.38%). Most of the patients (61.66%) required caesarean section. A total of 36 (60%) neonate had birth weight ranging between 2.5 to 3.0 kg and 83.01% had asymmetrical IUGR.
