The association between chronic HCV infection and different lymphoproliferative disorders is well known. Antiviral therapy for HCV, including direct acting antivirals, has been proven as effective therapy of HCV-related lymphomas. So far, only few cases of lymphoma after HCV eradication have been reported in the literature. We report a series of 3 cases with HCV-related liver cirrhosis, two of them with sustained virologic response 12 weeks after direct acting antiviral therapy and one with spontaneous eradication of HCV infection, that were diagnosed with large B cell non-Hodgkin’s lymphoma, that are still in remission actually, with a favorable evolution. Engaging the multidisciplinary team is tremendously important, since the diagnosis and management of those cases are successfully made by close collaboration between the gastroenterologist, hematologist, radiologist and pathologist, using imaging techniques, histopathological and immunohistochemical analysis. In the era of highly effective and safe direct acting antivirals, achieving sustained virologic response has been proven to prevent hepatic and extrahepatic malignant complications of chronic HCV infection, including lymphoma. However, recent reports showing the persistent risk for hepatic malignancy in some rare cases, demonstrate that HCV eradication is not the end point and this could be also in line with our cases of extrahepatic non-Hodgkin’s lymphoma.","PeriodicalId":22101,"journal":{"name":"Surgery, Gastroenterology and Oncology","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87299859","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Heterotaxic syndrome is a general term that describes the abnormal positioning of the viscera in the chest and abdomen. Signs of polysplenic syndrome include: bilobed lungs, liver located on the median line, but also intestinal malrotation. The prevalence of this condition is 1.4 patients per 10,000 children. The clinical aspect is closely correlated with the specific pathology of polysplenism. Apparently, incidental signs and symptoms are encountered, unexplained and unassociated from the etiopathogenic point of view, until the imaging examination that elucidates the causes of the observed clinical picture. As such, polysplenism, i.e., left isomerism, is the result of the abnormal development of the asymmetry of the laterality of the organs having the effect of situs ambiguus.","PeriodicalId":22101,"journal":{"name":"Surgery, Gastroenterology and Oncology","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136207238","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Surgical technique: We describe the operative steps of the TEP hernia repair technique with the particularities of endoscope dissection of the extra-peritoneal space and no-fixation of the mesh after placement. Discussions: The description of the trocar’s positioning and the particularities of the technique are important, especially for young surgeons and are utterly for orientation but depends on the intraoperative findings. The correct space dissection, mesh placement and non-fixation of the mesh can reduce the risk of hernia recurrence and chronic postoperative chronic pain. Conclusion: Due to the regular anatomy, TEP hernia repair operative technique can be standardized. The thorough comprehension of the anatomical landmarks and the surgical steps are important for the good outcome of the surgery.","PeriodicalId":22101,"journal":{"name":"Surgery, Gastroenterology and Oncology","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83137086","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Reliable markers are needed to reduce patient morbidity and mortality. Cytokines in drain fluid and systemic cytokine levels have shown potential as predictive markers for CAL, but more robust evidence is required.
