Generally, it relieves with movement. There are several studies that argue the existence of an association between this syndrome and pregnancy. However, the pathophysiological mechanisms of this disorder in pregnancy are misunderstood. The objective of this narrative review is to identify and discuss some possible etiologies of RLS in pregnancy. A literature search was performed in the PubMed and ResearchGate databases by using the following search strategies: \"restless legs syndrome\", \"restless legs syndrome in pregnancy\", \"pregnancy and vitamin D deficiency\" and \"pregnancy and zinc\". The publications were initially sorted through their title. After the initial process, inclusion and exclusion criteria were applied. The included articles were sorted by authors, year, journal of publication, type of study, and organized by chronological order of publication. Among the main findings, hormonal changes, iron metabolism, vitamin D deficiency, genetic factors, zinc and magnesium fluctuations have been some of the hypotheses supporting the development or worsening of this disorder in pregnancy. Dopamine also appears to be correlated with hormonal changes, iron metabolism, ferritin, folic acid and vitamin D deficiency. In conclusion, there are several hypotheses trying to link restless legs syndrome with pregnancy. The most covered were hormonal fluctuations and iron metabolism. Its diagnosis relies on a polysomnographic or polygraphic study, but clinical findings remain as an important part of the diagnostic process. Nocturia is a common symptom that may indicate severe OSAS, but it is often forgotten in the initial evaluation of these kind of patients. Positive airway pressure (CPAP) is known to reduce nocturia, but the roll of surgery is not clear about it.</p><p><strong>Material and methods: </strong>A case series is presented. We compare 2 groups of male adults with severe OSAS, the first group treated with CPAP for 3 months, while group 2 underwent a multilevel surgical management. Apnea-hypopnea index (AHI) and the nocturia events number (NEN) were assessed before and after the treatment. Frequencies, descriptive statistics and a related sample Student's <i>t</i>-test were performed for statistical analysis.</p><p><strong>Results: </strong>97 male patients were included, age ranged from 29 to 71 years old. In group 1, treated with CPAP, AHI mean was 54.59 and nocturia mean 4.53 before treatment. With CPAP, the AHI mean was 6.63 and NEN mean 0.51. In group 2, AHI mean before surgery was 40.02, NEN mean 3.78, and after the surgical management AHI mean was 7.74 and NEN mean 0.7. Student's <i>t</i>-test in groups 1 and 2 showed a <i>p</i>=0.000.</p><p><strong>Conclusions: </strong>AHI and NEN were clearly related in both groups, and the improvement of NEN and AHI were consistent in both groups, the CPAP and the surgical treatment. For each, 17 anatomical variables were identified and examined using Invivo Dental software on CBCT scans, WebCeph software on laterolateral teleradiographs, and Rhinoceros 6.0 software on dental casts.</p><p><strong>Results: </strong>A generalized linear model of all the anatomical factors identified only two statistically significant variables. This study aimed to systematically examine the correlation between the snoring sounds and the apnea-hypopnea index (AHI) as the measures of OSA severity.</p><p><strong>Material and methods: </strong>A comprehensive literature review using the MEDLINE, Embase, Cochrane Library, Scopus, and PubMed databases identified the published studies reporting the correlations between and severity of snoring and the AHI values by meta-regression analysis.</p><p><strong>Results: </strong>In total, 13 studies involving 3,153 adult patients were included in this study. The pooled correlation coefficient for snoring sounds and AHI values was 0.71 (95%CI: 0.49, 0.85) from the random-effects meta-analysis with the Knapp and Hartung adjustment. The <i>I</i> <sup>2</sup> and chi-square <i>Q</i> test demonstrated significant heterogeneity (97.6% and <i>p</i><0.001). After adjusting for the effects of the other covariates, the mean value of the Fisher's <i>r</i>-to-<i>z</i> transformed correlation coefficient would have 0.80 less by the snoring rate (95%CI = -1.02, -0.57), 1.46 less by the snoring index (95%CI = -1.85, -1.07), and 0.21 less in the mean body mass index (95%CI = -0.31, -0.11), but 0.15 more in the mean age (95%CI = 0.10, 0.20). It fitted the data very well (<i>R</i> <sup>2</sup>=0.9641).</p><p><strong>Conclusion: </strong>A high correlation between the severity of snoring and the AHI was found in the studies with PSG. The COVID-19 pandemic brought significant changes and high psychological stress, causing a great impact on this population. Here we aim to analyze the influence of the pandemic on the sleep quality of medical students.</p><p><strong>Methods: </strong>Cross-sectional, observational, and descriptive study with a quantitative approach carried out with students from medical universities in Rio Grande do Norte state (Brazil) through the online application of two questionnaires: Pittsburgh sleep quality index (PSQI-BR) and sociodemographic questionnaire (SQ).</p><p><strong>Results: </strong>A total of 142 medical students participated in this study: 103 women and 39 men. We observed a prevalence of low sleep quality in 78.16% of the sample and that the pandemic significantly affected the sleep quality among medical students (<i>p</i><0.05). We also found an alteration in the sleep pattern in 83% of the participants, mainly due to anxiety symptoms (38%). Finally, we observed no statistically significant difference in sleep quality or sleep patterns between genders or college period (<i>p</i>>0.05).</p><p><strong>Discussion: </strong>This rate of poor sleep quality is higher than the prevalence of periods before the pandemic (58%). After they have reached a final version of the ESS-HAD a sample of asymptomatic children and adolescents that were participants in another sleep project were invited to read and complete the questionnaire to evaluate comprehension by the aimed population.</p><p><strong>Results: </strong>Two independent researchers made the forward translation and it has around 90% of concordance. Minor disagreements were related to the position of adjectives in the sentence. No major difficulties were reported by the 3<sup>rd</sup> researcher that performed the back translation. After a consensus meeting with the four participants, we have reached a final version of the questionnaire. In the cognitive interviews, the scale was reportedly easy to understand to the 23 respondents. One adolescent suggested clarifying whether question 2 (likelihood of falling asleep watching TV or a video), referred to daytime or nighttime. The total ESS-CHAD score in this asymptomatic sample varied from 0-17, a mean score of 7.08±5.65.</p><p><strong>Discussion: </strong>The final version of the ESS-CHAD in Brazilian Portuguese was approved by the copyright owners and was well understandable by caregivers and adolescents. The functional role of sleep spindles is not sufficiently clear, but there is a large body of literature that indicates the relationship between spindle activity and neural plasticity. Many of the spindle parameters (frequency, configuration, duration, density, and topography) vary significantly throughout life. However, the long duration, asynchrony and sharp morphology are the most distinctive characteristics of sleep spindles in infants. This unique infantile phenotype of sleep spindles typically changes after approximately one year of postnatal life in humans. Considering that EEG reflects brain electrochemical activity, there is evidence to suggest that substantial neurochemical events underlie these changes. In this paper, we hypothesize that the GABA (gamma-aminobutyric acid) shift is a key event influencing the sleep spindle phenotype during infancy. The patient had a history of mild obstructive sleep apnea with an AHI of 12.6/hour with an oxygen saturation nadir of 95%, which resolved after bariatric surgery. Treatment with topiramate 100mg at bedtime controlled all three parasomnias for ten months at the latest follow-up, with relapse occurring whenever topiramate was stopped. To our knowledge, this is the first reported successful treatment of sleep-related smoking. Exercise treatment included 50 minutes of moderate-intensity aerobic exercise (50% of reserve heart rate), on a treadmill, 3 times/wk. Exercise plus acupuncture treatment included the exercise protocol plus acupuncture once per week. Pre- and post-treatments measures included insomnia severity index (ISI), Pittsburgh sleep quality index (PSQI), polysomnography (PSG), 10 days-sleep diary, state-trait anxiety inventory, Beck depression inventory, quality of life (SF-36), and morning cortisol level.</p><p><strong>Results: </strong>No group by time interaction was found for insomnia severity, sleep, mood or quality of life. Significant time differences (<i>p</i><0.05) were observed for ISI, PSQI, and some variables of sleep diary. Polysomnography data showed a decrease in rapid eye movement (REM) latency after the interventions. Significant time improvements were also observed for mood, anxiety, depression, and quality of life. A significant moderate correlation was found between changes in the ISI and morning cortisol level.</p><p><strong>Conclusion: </strong>There were no significant differences between treatments on insomnia severity, sleep, mood or quality of life. Exercise and exercise plus acupuncture were efficacious for decreasing insomnia severity to subthreshold insomnia.
