[Multifactorial treatment of depression]. [抑郁症的多因素治疗]。

{"title":"[Multifactorial treatment of depression].","authors":"P Kielholz","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>According to the results of all recent epidemiological enquiries, the number of cases of depression diagnosed is steadily increasing, especially in urban communities. Parallel with this increase, there has been a clear tendency for the symptomatology of depression to change in the direction of somatization. Essential prerequisites for the success of treatment of depression are not only a carefully established diagnosis but - ordinarily - also combination of the psycho-, pharmaco-, socio- and physiotherapy. In case of depressive states the nosological diagnosis determines the choice of basic therapy and the phenomenological diagnosis the selection of the right antidepressant. All antidepressants have a delayed onset of action and are apt to provoke side effects of varying severity. Prior to the initiation of treatment the patient's attention should be drawn to the possibility of side effects. The basic principles of psychotherapy for depression are briefly outlined. The possibilities to treat therapy-resistant depressions with a combined infusion therapy are prescribed. {"title":"[Sex character of human movement answers in the Rorschach test in transsexual and transvestite patients].","authors":"K W Bash","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>As a complement to a former study on the sexual character of the kinaesthetic human movement responses in the Rorschach tests of healthy and neurotic men and women, the same characteristic was investigated in Rorschach records of transsexuals and transvestites from the Psychiatric Out-Patient Department of the University Hospital of Zurich. In none of these groups was the distribution of masculine, feminine and neuter movement responses congruent with that in one of the preceding groups. The anatomically male transsexuals who desired a change of sex to female (TXmw) gave considerably more interpretations of the opposite or goal sex than did the anatomically female (TXwm). The two transsexual groups TXmw and TXwm do not constitute mirror images of each other. Whereas the mean experience type of most groups was not far from ambiequality, the TXmw and the male transvestites (TVmw) were decidedly introverted. All three of the newly studied groups gave more space responses than would be expected in a \"normal\" Rorschach record, especially the TXwm and the TVmw. In this paper two groups of clients of the University Psychiatric Outpatient Clinic of Basle with different durations of treatment were compared by using the social indicators of the AMDP-System: the first group of outpatients has been in treatment for less than one year (short term patients, KZP), the second has been in treatment for more than five years (long term patients, LZP). Long term patients are more often than short term patients unmarried. They live more often in family fragments, are/were less often in professional leading positions and are less often integrated in the labour-market. Before as well as after their first contact to the University Psychiatric Outpatient Clinic long term patients were more often hospitalized in psychiatric clinics. Social sex differences are elaborated for both groups. The heavier institutional dependence of the second group is discussed in the light of their greater social disintegration. {"title":"[Development of a HAWIE (Hamburg Wechsler Intelligence Test for Adults) short form for examination and assessment].","authors":"W Demuth","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>As the use of the HAWIE complete form within the framework of conducting a forensic examination and producing an expert report seemed too extravagant to us, by means of using statistical procedures, we developed a short form consisting of the four sub-tests General Information (AW), Digit Span (ZN), Similarities (GF), and Block Design (MT), which correlated to the power of the long form (rxy = 0.905). 300 HAWIE reports by Ss, which were examined for forensic expert report purposes, served as our basis. Our new test form is to be recommended because of its great time saving factor (about 60%), especially when the question at issue within the framework of a forensic expert report is ability to feel guilt, credibility and fitness for questioning, and the general intellectual performance is to be globally determined. During the years of development the psychopharmacological treatment has to include psychotherapeutic work with the parents. Psychotherapy without pharmacotherapy is often indicated, but not the contrary. A treatment with psychotropic drugs seems not to be indicated in cases of light disturbances of a child or adolescent or when they can be influenced by changes in the social environment. Psychopharmacology is a useful supply in the treatment of children with psychic disturbances of medium or high degree. Besides the development of a capacity for identification with the patient the child psychiatrist has to develop a capacity for delimitation, especially than, when there is a tendency of overidentification, which is connected with his choice of the profession. Psychopharmacology will in child psychiatry remain on a empirical level until the clinical entities will be clearer defined, whether in a psychodynamic or in a biologic way. Zerssen, 1976) raised the question of the clinical usefulness of an overall summation score. 2 samples (215 patients, 326 healthy subjects) participated; emphasis was mainly on patients. From the results obtained through factor analysis the interpretation of a single factor-solution (general factor) is justified. This factor correlated highly with the \"neuroticism-\", \"depression-\", and \"general well-being\"-dimensions in inventories like FPI, GT and PND-S. The rotated factor matrix (mineigen greater than or equal to 1) permits a reasonable interpretation of 6 factors. The first and partly the second factor represent domains like mood, well-being and drive (factor I: gloomy, depressive mood; factor II: exhaustion, fatigue). Distinctive somatic complaints are muscle and joint pains , breathing and swallowing difficulties, and (less pronounced) gastro-intestinal and urogenital pains.
