Despite the spontaneous acquisition of tolerance in the majority of cases at school age, the impact of CMPA on the quality of life of the child and his/her family is clearly more deleterious than in other food allergies. The milk scale, a form of oral immunotherapy, accelerates the acquisition of tolerance such as to curb this negative impact, through a process of gradually reintroducing milk into the diet of the allergic patient. It represents a major breakthrough in the treatment of osteoporosis: while most treatments inhibit bone resorption, romosozumab has a dual effect, by increasing bone formation and reducing bone resorption. It is reimbursed in postmenopausal osteoporosis in patients with very high fracture risk (i.e. after a recent major fracture, occurring within two years). Its ideal use, in the therapeutic sequence for post-menopausal women, is as first line treatment in case of a recent major fracture. {"title":"[Obesity, type 2 diabetes, metabolic syndrome and depression : a two-way relationship].","authors":"André Scheen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Metabolic disorders, especially obesity, type 2 diabetes and metabolic syndrome, are frequently associated with depressive symptomatology, including severe depression. In fact, there is a two-way relationship : metabolic disorders could promote depression and depression could favour metabolic disorders, which may lead to a vicious circle. The reasons for this astonishing relationship are diverse : a genetic predisposition, shared biochemical abnormalities including low-grade inflammation, bad habits that combine unhealthy food and excessive sedentary life-style, poor therapeutic compliance. Medications that target metabolic disorders may positively impact depression, with recent promising, yet preliminary, data with glucagon-like peptide-1 receptor agonists. We report the case of a woman in whom a thickened PS was diagnosed following the onset of central diabetes insipidus revealed by polyuria-polydypsia syndrome of late pregnancy and postpartum. The pituitary exploration showed panhypopituitarism with disconnecting hyperprolactinemia. An etiological investigation for an inflammatory, granulomatous or tumour cause was carried out, but was negative. Postpartum lymphocytic hypophysitis was then retained. However, the course was puzzling with a control pituitary MRI showing disappearance of the PS thickening with paradoxical appearance of a supra-pituitary tumour, the biopsy of which concluded of being a Langerhansian histiocytosis. This paradoxical sequence is unusual and has not been reported before. Magistral formulations offer ideal flexibility for prescriptions tailored to vitamin A, D and E blood levels. However, they expose to human errors, mainly leading to vitamin D intoxication whose clinical features are related to hypercalcaemia. Symptoms are mostly digestive (vomiting, constipation, abdominal pain …) and, less frequently, renal (nycturia …) complaints. When symptoms and/or serum calcium levels ≥ 14 mg/100 ml are present, prompt management is required. Besides interruption of supplementation, rapid intravenous hyperhydration (saline) is essential. Once hydration has been restored, and still under close biological supervision, a loop diuretic (furosemide) may be used but the drug of choice to achieve rapid normalization of blood calcium levels will often be intravenous pamidronate. Normalization of serum vitamin 25(OH)-D levels may take several months but the prognosis is very good. Our aim is to evaluate the diagnostic performance of adrenal venous sampling in lateralizing aldosterone hypersecretion and to compare it with other imaging techniques. Preventive strategies can be effective if they include multiple dimensions and are well-structured. Five points appear essential in order to obtain a high-quality preventive strategy: progressive muscle strengthening, optimal workload management, lumbopelvic stability exercises, development of physical condition and optimization of sprint technique. While recognizing the limitations of preventive screening and the difficulty of predicting future injury, screening tests appear relevant for the identification of an individual risk profile for each footballer and in defining each player's work priorities.
