Soluble TNF-related weak inducer of apoptosis is a proinflammatory cytokine that may cause glomerular and tubular injury and increase sclerostin expression. This study aimed to investigate serum sclerostin and soluble TNF-related weak inducer of apoptosis levels in patients with glomerulonephritis and the effects they may be associated with.</p><p><strong>Methods: </strong>This cross-sectional study included 93 patients, 63 of whom were glomerulonephritis and 30 were healthy controls. Serum sclerostin, soluble TNF-related weak inducer of apoptosis, and 24-h urinary protein excretion were measured, and pulse wave velocity was calculated for arterial stiffness.</p><p><strong>Results: </strong>Serum sclerostin and soluble TNF-related weak inducer of apoptosis were higher in glomerulonephritis patients than in the control group, and serum sclerostin and soluble TNF-related weak inducer of apoptosis levels were correlated with both proteinuria and pulse wave velocity. In addition, in the regression analysis, serum sclerostin and soluble TNF-related weak inducer of apoptosis levels were found to be independent predictors of proteinuria in patients with glomerulonephritis.</p><p><strong>Conclusion: </strong>This is the first study to show that serum sclerostin and soluble TNF-related weak inducer of apoptosis are elevated in glomerulonephritis patients, and these two markers correlate with arterial stiffness and proteinuria in these patients. The presence of fetal acidemia at birth was identified by analyzing the pH of umbilical cord arterial blood (pH<7.1).</p><p><strong>Results: </strong>No significant effect of the cardiotocography category on the arterial (p=0.543) and venous (p=0.770) pH of umbilical cord blood was observed. No significant association was observed between the cardiotocography category and the presence of fetal acidemia (p=0.706), 1-min Apgar score <7 (p=0.260), hospitalization in the neonatal intensive care unit (p=0.605), newborn death within the first 48 h, need for neonatal resuscitation (p=0.637), and adverse perinatal outcomes (p=0.373). Nascimento et al.1 investigated the relationship between lesionlevel characteristics assessed by chest computed tomography, probable sarcopenia, and the percentage of diffusing capacity of the lung for carbon monoxide with clinical and functional variables in COVID-19 survivors1. Muscle impairment and lung dysfunction are widespread in COVID-19 survivors, according to Nascimento et al.1 and hospitalization was related to the worst muscle force and carbon monoxide diffusing capacity of the lung1. According to Nascimento et al.1 the findings underscore the need for long-term follow-up of such patients as well as rehabilitation programs1. The long-term consequences of COVID-19 have a concern that must be addressed immediately. There are significant issues that need to be resolved. Despite the likelihood that the patient had undiscovered co-morbid conditions, COVID-19 supported the patient’s first apparent clinical diagnosis. The patient might also get a second COVID192. It is important to discuss about prior vaccines as well. To draw a conclusion on how the disease influences health issues, there must be enough data and adequate knowledge. According to the International Diabetes Federation (IDF), there were 537 million people worldwide (aged 20–79) suffering from diabetes in 2021. This number is expected to rise to 783 million in 20451. Vietnam is among the top 10 countries with the highest increase rate of diabetes cases, at 5.5% per year. As reported by the Vietnamese Association of Diabetes and Endocrinology (VADE), there are currently 5 million Vietnamese people living with diabetes, accounting for 6% of the population. This number is predicted to increase to 7 or 8 million by 20252. Diabetes is estimated to be the cause of death for at least 80 people each day. The percentage of undiagnosed diabetes cases in Vietnam is nearly 62.6%3. If left undetected and untreated in the early stages, pre-diabetes can increase the risk of dangerous complications and severely affect the patient’s health. In recent years, Nesfatin-1 has been identified as one of the cytokines associated with diabetes4,5. Peripheral Nesfatin-1 was linked to several clinical laboratory parameters that influenced nutrition and metabolism6. Therefore, determining the concentration of Nesfatin-1 serum is important for assessing the disease progression, predicting the damage to target organs, and evaluating the impact of treatment measures. A cross-sectional study was carried out on 524 patients diagnosed with prediabetes and 205 healthy people serving as the control group. These participants were taken from periodic health check-up groups at general hospitals in northern Vietnam. Pre-diabetes was diagnosed for those with fasting blood glucose (FBG) ≥126 mg/dL, hemoglobin A1c (HbA1c) ≥6.5%, or with classic symptoms of hyperglycemia7. The concentration of Nesfatin-1 serum, anthropometry, and clinical parameters associated with the cardiovascular, hepatic, and renal organs were determined and analyzed. Nesfatin-1 has the effects of suppressing appetite, reducing gastric motility, reducing cholesterol, triglycerides, and white adipose mass, as well as lowering lipid production and glucose in the blood. The Nesfatin-1 serum level of pre-diabetes patients was 1.5 times lower than in the control group (0.66 vs. 1.12 ng/mL) (Table 1). {"title":"Foley catheter plus misoprostol versus misoprostol alone for labor induction.","authors":"Jhonathan Alcides Elpo,&nbsp;Bruna de Aguiar Araújo,&nbsp;Lia Karina Volpato","doi":"10.1590/1806-9282.20220897","DOIUrl":"https://doi.org/10.1590/1806-9282.20220897","url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to analyze the effects of Foley catheter combined with misoprostol in the labor induction process.</p><p><strong>Methods: </strong>This is a nonblinded, block randomized, controlled trial that compared the association between transcervical Foley catheter/vaginal misoprostol 25 μg combination and vaginal misoprostol 25 μg alone in normal-risk and healthy pregnant women undergoing labor induction in the south of Brazil.</p><p><strong>Results: </strong>A total of 230 patients with indications for labor induction were evaluated and classified into the \"combined\" group (Foley catheter plus misoprostol), consisting of 107 patients, and the \"misoprostol\" group (misoprostol only), consisting of 123 patients. The \"combined\" group was observed to have a shorter labor induction time (p=0.008). In addition, there was a lower need for misoprostol use for overall cervical ripening (p<0.001) and a lower relative risk of needing a second, third, or fourth misoprostol tablet in the \"combined\" group (risk ratio [RR] 0.80, 95% confidence interval [CI] 0.71-0.91; RR 0.41; 95%CI 0.31-0.56; and RR 0.29, 95%CI 0.17-0.52, respectively) (p<0.001). No statistically significant difference was found in induction failure rate, cesarean section rate, or perinatal outcomes.</p><p><strong>Conclusion: </strong>A combination of methods leads to shorter labor induction, lower need for misoprostol doses, and lower risk of cesarean section, with no increase in the rate of perinatal complications. Patients were randomized into two groups, namely, the radial extracorporeal shock wave therapy (group 1, n=33) and the sham radial extracorporeal shock wave therapy (group 2, n=33). Night splints and tendon nerve gliding exercises were given to all participants. The participants were evaluated at baseline and the first month after treatment. Participants were evaluated using a visual analog scale, the Boston Carpal Tunnel Questionnaire, Leeds Neuropathic Symptom and Symptom Assessment, and electrophysiological examinations.</p><p><strong>Results: </strong>A total of 37 participants (a total of 55 wrists, radial extracorporeal shock wave therapy n=27, and sham radial extracorporeal shock wave therapy n=28) completed the study. After the intervention, there was a significant decrease in visual analog scale values (p<0.001) and a significant increase in Boston Carpal Tunnel Questionnaire scores (p<0.001) and Leeds Neuropathic Symptom and Symptom Assessment scores (p<0.001). The aim of this study was to investigate the effects of open kinetic chain exercises and closed kinetic chain exercises on pain, muscle strength, functional status, and quality of life in patients with knee osteoarthritis.</p><p><strong>Methods: </strong>The study included a total of 60 patients with primary unilateral knee osteoarthritis grade I and II. The patients were categorized into three groups as open kinetic chain exercises (n=20), closed kinetic chain exercises (n=20), and control group (n=20). The outcome measures, including pain, isokinetic muscle strength, functional status, and quality of life, were collected at baseline and at the end of 6 and 12 weeks.</p><p><strong>Results: </strong>Closed kinetic chain exercises and open kinetic chain exercises had significant improvement in pain, muscle strength, WOMAC, and SF-36 scores after the treatment and at their 6th and 12th week follow-ups compared to their baseline values and compared to the control group (p<0.05).</p><p><strong>Conclusion: </strong>The changes in all outcome measures were similar between closed kinetic chain exercises and open kinetic chain exercises (p>0.05). Closed kinetic chain exercises and open kinetic chain exercises were similar for knee osteoarthritis grade I and II. {"title":"Evaluation of the efficacy of systemic inflammatory indices in determining mortality in very low birth weight infants.","authors":"Ufuk Cakir,&nbsp;Cuneyt Tayman,&nbsp;Ali Ulas Tugcu","doi":"10.1590/1806-9282.20230150","DOIUrl":"https://doi.org/10.1590/1806-9282.20230150","url":null,"abstract":"<p><strong>Objective: </strong>In our study, we aimed to investigate whether systemic inflammatory indices could be an indicator of mortality in very low birth weight (<1,500 g) preterm infants.</p><p><strong>Methods: </strong>Very low birth weight preterm infants were included in our study, and patient data were recorded retrospectively. Neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, monocyte-to-lymphocyte ratio, systemic immune-inflammation index, pan-immune-inflammation value, and systemic inflammation response index were calculated and recorded. The survivors and infants who died were compared for systemic inflammatory indices.</p><p><strong>Results: </strong>A total of 1,243 very low birth weight infants were included in the study. Of the patients, 1,034 survived and 209 died. Neutrophil-to-lymphocyte ratio, monocyte-to-lymphocyte ratio, platelet-to-lymphocyte ratio, pan-immune-inflammation value, systemic immune-inflammation index, and systemic inflammation response index were found to be statistically significantly lower in the mortality group than those in the survivor group (p=0.039, p=0.001, p<0.001, p<0.001, p<0.001, and p=0.002, respectively). According to the receiver operating curve analysis, systemic immune-inflammation index with the highest area under the curve (0.844) was found to be the most effective systemic inflammatory indices in predicting mortality with a cutoff level of ≤28.87 (p=0.0001).
