Inclusion criteria focused on decision regret in the female population with BRCA1 and/or BRCA2 mutations, with no restrictions on the methodologies of the included studies, but only in the English language. The selection process led to the inclusion of 13 studies.</p><p><strong>Results: </strong>The analysis revealed a significant trend toward decision regret among patients facing complex medical choices. The quality of healthcare communication, decision support, and genetic counselling emerged as key factors influencing patients' perceptions and experiences, with direct implications for their quality of life and psychological well-being. The results suggest that these decisions considerably impact patients, both in terms of clinical outcomes and emotional experiences.</p><p><strong>Discussion: </strong>The investigation highlights the vital importance of a personalized care approach, emphasizing the critical role of managing patients' emotional and psychological complexity. In the context of the traditional medical examination, touch, palpation, is essential to diagnose or suspect pathology. But it is not only an issue of semeiotics. The physician's ability to examine a body through his or her hands is at the heart of the doctor-patient relationship. Moreover, we have been witnessing for many years to the progressive downgrading of the objective examination in clinical practice. First, the obsolescence of interaction with the patient due to the lure of diagnostic technology, then the \"social distancing\" of Covid-19 and, finally, the expansion of telemedicine has contributed to the distancing of caregivers from patients. The same technology that tends to distance doctor and patient provides haptic wearable devices that can digitize touch and open up application scenarios that are still largely unexplored. These, however, cannot replace human interaction. What can we do to preserve the culture of physical contact in the doctor-patient relationship? As always, awareness is the first step. The technology that separates us from patients is not the real problem. It is only when it becomes an end instead of a means that we risk losing centuries of medical tradition. It is therefore necessary to strike a balance between using technology and ensuring that health care professionals maintain a central role in providing emotional support and understanding to patients. Such issues should be introduced early in the medical student's training. In the course of study, not only the mechanisms of disease should be taught, but also how to approach care with compassion, empathy and \"human touch\". But in \"big data medicine\", the art of dissecting the human body has become an obsession. Indeed, modern biotechnology allows us to see and measure the molecular components of every single cell. But how can we put this immense number of bits and pieces back together again and see the patient as a whole? The first turning point is that proposed by René Descartes, who, inspired by dreams and visions, conceived the idea of unifying all scientific disciplines through the pervasive application of mathematics. Descartes formulates four basic rules, the second (top-down method) and third (bottom-up method) of which become crucial in modern data analysis. An instructive case study considered here is that of pulmonary tuberculosis, where the Cartesian approach of decomposing problems into smaller and smaller \"pieces\" - from organism to organ and from cellular lesion to the microscopic level - has led to the cure of the disease through antibiotics. This success story inspired Paul Ehrlich who, with the concept of the \"magic bullet\", defined modern pharmacology. However, this paradigm is being challenged today by multifactorial diseases and big data medicine, where the enormous availability of clinical and molecular data must be integrated to arrive at a therapeutic decision. The Cartesian approach shows its limitations today, as witnessed by the similar difficulty in fields other than medicine, illustrated here by the case of choosing to produce a successful television series based on user profiling. The take-home message is that the amount of data collected does not automatically guarantee success but that, instead of being data-driven, a collective \"human\" overview and assessment is inevitable. Diagnosis is based on history, physical examination, documentation of sensitization, such as the production of allergen-specific IgE, also using molecular diagnostics in selected patients. Treatments is based on education, engagement, allergen avoidance, non-pharmacological and pharmacological remedies, and allergen-specific immunotherapy (Ait). Symptomatic treatments mainly concern intranasal/oral antihistamines and/or nasal corticosteroids. This article also aims to discuss new management strategies for AR patients. The self-management of allergic rhinitis could include new strategies. They should be redesigned in line with the new proximity care model planned in the Pnrr and DM 77, while avoiding a negative domino effect on the management of chronic diseases. A guide recently published by the MaCroScopio project is useful to shed light on this issue and to understand the relationship between the distribution channels present in Italy (distribuzione diretta - DD; distribuzione per conto - Dpc; distribuzione convenzionata) and the management of chronic diseases. Analysis of OsMed data showed that the majority of medicines dispensed in DD and Dpc are for chronic patients; therefore, a change in distribution channels inevitably affects chronicity management and could lead to increased expenditure. It is necessary to evaluate the transfer of drugs from one channel to another on a case-by-case basis according to the specifics of each drug. Today, however, the challenge is on training, where the current contribution of the university does not seem sufficient.
