Adequate management is crucial for glycemic control and the prevention of complications. The transition from pediatric care centers to adult care centers is a critical phase, and its effectiveness varies among different countries and even within different areas of the same country.</p><p><strong>Materials and methods: </strong>In this qualitative study conducted in 2023, at least 30 patients with T1D who had undergone the transition between 2017 and 2022 were invited to participate by completing an online semi-structured questionnaire regarding their transition experiences. Additional questionnaires were administered to healthcare professionals to gather information on organizational barriers and patient expectations.</p><p><strong>Results: </strong>The survey conducted on 52 young adults with T1D from 5 Italian diabetes centers revealed that the transition to adult care typically occurs at an average age of 19.6 years. At the time of the survey, 73% of participants had not experienced acute complications, and 55.7% had optimal metabolic control prior to the transition. However, many patients encountered issues in the operational management of the transition, with about 40% describing the process as difficult, highlighting in some cases a feeling of \"abandonment\" and a desire for greater support and communication between pediatric and adult services. The majority of respondents reported an increase in autonomy in managing their diabetes.</p><p><strong>Discussion and conclusions: </strong>Although the transition process for patients with T1D is structured, the study reveals some gaps in communication and support between pediatric and adult care centers. This retrospective observational study has identified patients potentially affected by DMD and described their utilization of healthcare resources and direct healthcare costs charged to the Italian National Healthcare Service (INHS).</p><p><strong>Methods: </strong>From the Foundation Ricerca e Salute (ReS), through a specific algorithm based only on administrative healthcare data of 5.4 million inhabitants in 2021 (index date), male patients aged <30 years potentially affected by DMD were identified. Comorbidities at baseline, and utilization of healthcare resources and direct healthcare costs charged to the INHS during the year following index date, were described.</p><p><strong>Results: </strong>In 2021, 120 male patients aged <30 years were identified as potentially affected with DMD (2.2/100,000 inhabitants; 16.1/100,000 males aged <30 years). Chronic airway disease and cardiomyopathy were found in 19.2% and 15.0% of patients, respectively. During follow-up: 41.7% of patients were treated with deflazacort, 2.5% with ataluren and about one third with cardiac drugs; 29.2% and 42.5% were admitted to overnight and day hospitalization, respectively, mainly due to neurological, cardiac, and respiratory diseases; 12.5% accessed the emergency department, mainly for traumatisms and fractures; 70.8% received local outpatient specialist care, half of which were specialist visits, and about 15% cardiac diagnostics. On average, the per capita annual total cost charged to the -INHS was € 6713; ataluren accounted for more than half of this expenditure. After having excluded the dispensation of ataluren during follow-up, the mean per capita total cost was € 2548, more than half of which due to hospitalizations.</p><p><strong>Conclusions: </strong>This study of administrative data has identified patients potentially affected by DMD, a rare disease, from a large sample of INHS beneficiaries, and assessed their healthcare pathway. The number of visits to the Emergency Department per 1,000 inhabitants varies significantly from region to region, as do the main indicators of functioning and outcome. After several decades of organizational and professional growth, culminating in the activation of the Schools of specialization in emergency medicine, the Italian emergency departments are currently going through a period of crisis that has worsened following the Covid-19 pandemic. The demanding and risky characteristics of this work environment along with the impossibility of carrying out freelance activities have pushed young doctors to prefer other disciplines, leaving approximately 75% of the specialization grants unfilled. The workload of the Emergency departments is directly related to the crisis of territorial medicine which is proving increasingly unsuitable to respond to the health needs of citizens in an era of tumultuous epidemiological and technological changes, and to the heavy reduction in hospital staff and bed capacity number of beds throughout the country. During the three-year pandemic period 2019-2022, Italy joined the mySupport study with 2 Piedmontese nursing homes. The mySupport study is a transnational project, which involved 6 European and extra-European countries (United Kingdom, Ireland, the Netherlands, the Czech Republic and Canada). Family carers were engaged in trained nurse-led family care conferences on care decisions at the end of life and received written resources. The project implementation required flexibility and to face unexpected events with poor resources, which were overcome by the establishment of trusting relationships with family carers and collaboration among nurses.
