{"title":"Antipsychotic polypharmacy: Retrospective evaluation of cases with psychotic disorder followed in outpatient clinic (tur)","authors":"Şahabettin Çeti̇n, M. C. B. Şengül, Osman Özdel","doi":"10.5505/KPD.2020.27676","DOIUrl":"https://doi.org/10.5505/KPD.2020.27676","url":null,"abstract":"Objective: In this study, it was aimed to investigate the sociodemographic and clinical features that may be related to antipsychotic polypharmacy and to discuss the causes and effects of this condition. Method: The records of the cases followed up at the psychotic disorders outpatient clinic were analyzed retrospectively. Sociodemographic and disease related clinical features, Clinical Global Impression Scale (CGI) Severity of Illness and Advers Events in their last applications were evaluated. Results: The most common diagnosis is schizophrenia with a rate of 63.6% (n = 150). It was determined that 76.7% (n = 181) of 236 patients included in the study were using at least two antipsychotic drugs, %64.8 (n=153) were using long acting parenteral antipsychotics. When comparing antipsychotic polypharmacy and monotherapy patients;statistically significant difference was found in terms of age, gender, onset age of disease, history of electroconvulsive therapy (ECT), long termparenteral antipsychotic use, number of hospitalizations and duration of their last hospitalization, Clinical Global Impression Scale severity of illness and advers events scores. Discussion: Although there is insufficient data on the efficacy and safety of antipsychotic polypharmacy, it appears that it is preferred in practice far above the recommendations of treatment guidelines.It seems important to understand the causes of this situation, and to make adjustments in the practices and guidelines of clinicians. Identify the problem of how coordination undertaken between the Department of Agriculture, Plantation, Food and Horticulture at the Agricultural Extension Center. The research objective describe and analyze the coordination of the Department of Agriculture, Plantation, Food and Horticulture at the Agricultural Extension Centers. The methodology that used is descriptive method with qualitative approach. The data collection techniques through library research and field studies (interviews, observation). Perform data analysis using data classification phase, doing deepening through in-depth interviews and verified through a search and additions. The related informants is Section Head of Production of Rice, Coordinator of Agricultural Extension Centers, Community Conservation Pandanwangi Rice Cianjur (MP3C), Farmers Rice Pandanwangi Cianjur. The results showed that the coordination is done by the Department of Agriculture, Plantation, Food and Agricultural with Agricultural Extension Center in the preservation of rice Pandanwangi not optimal. It can be seen from the less than optimal enforcement of the rules, a lack of integration between each other, building mutual agreements as not optimal and there is some purpose yet implemented. However, COVID-19 solidarity, which is not only given to vulnerable groups, can cause only a few elderly to receive assistance. This study aimed to analyze the accuracy of the targeting of COVID-19 solidarity assistance aimed at the elderly as the most vulnerable group, concerning things that can be understood about the elderly in the midst of the COVID-19 pandemic. This research is a descriptive study with a semi-quantitative approach. This research was conducted by tracing and analyzing news about Covid-19 Solidarity, which was published in online media. The research period was started from March to June 2020. Data collection techniques were carried out through documentation, observation, and literature studies. The study found that out of 352 news, only 16 news contained the target to the elderly. So it can be concluded that during the COVID-19 pandemic, only a few elderly received assistance. This is due to the negative stigma regarding the elderly. Suggestions for future researchers, to be able to research by gathering news in a larger number to get wider news information from each region. Tujuan penelitian ini adalah untuk mengembangkan bahan ajar matematika SD yang terintegrasi karakter rasa ingin tahu yang valid, praktis dan efektif. Bahan ajar yang telah dibuat diujicoba terbatas pada kelas IV SD Petompon 01 Kota semarang. Penelitian ini merupakan penelitian pengembangan atau Reseach and Development (R & D). Prosedur yang digunakan adalah dengan menyederhanakan model pengembangan Borg & Gall. Model pengembangan Borg & Gall terdiri dari 10 langkah namun pada penelitian ini dibatasi sampai langkah ketujuh kar ena keterbatasan peneliti yaitu : 1) Potensi dan Masalah; 2) Pengumpulan data; 3) Desain produk; 4) Validasi Produk; 5) Revisi Desain; 6) Uji Coba keterpakaian; 7) Revisi Produk. Tehnik pengumpulan data adalalah observasi, wawancara, lambar validator bahan ajar dan tes hasil belajar . Tehnik analisis data terdiri dari analisis data kevalidan , kepraktisan dan keefektifan. Rendahnya hasil belajar ini disebabkan oleh berbagai faktor. Beberapa faktor diantaranya adalah penggunaan strategi atau pendekatan pembelajaran yang tidak tepat dalam pembelajaran matematika. Konsep matematika yang abstrak akan sulit diterima oleh siswa jika pendekatan yang digunakan guru tidak menjembatani konsep berpikir peserta didik sekolah dasar yang taraf berpikirnya masih pada tahap operasional konkrit. Ketidaksesuaian pendekatan pembelajaran ini menyebabkan minat belajar matematika rendah sehingga berdampak pada hasil belajar peserta didik yang rendah juga. Oleh karena itu guru harus memperhatikan taraf berpikir siswa dalam memilih pendekatan yang tepat dalam pembelajaran matematika. Salah satu pendekatan yang dapat digunakan dalam pembelajaran matematika adalah Realistic Mathematics Education (RME). Penelitian ini bertujuan untuk mengetahui keefektifan RME ditinjau dari minat dan hasil belajar matematika. Penelitian ini merupakan penelitian quasi experimental design . Populasi penelitian seluruh peserta didik kelas IV di SD Muhammadiyah Serut. Teknik pengambilan sampel menggunakan Sampling Jenuh. Pengambilan sampel dilakukan secara acak dengan mengundi kelas IV A dan IVB sebagai kelas eksperimen dan kelas kontrol. Analisis data yang digunakan adalah uji dependent t test dan uji independent t test . Hasil penelitian ini menunjukan bahwa pembelajaran matematika menggunakan RME efektif ditinjau dari minat belajar dan hasil belajar matematika.
