{"title":"Intraoral De-Epithelialized Gingival Graft in the Treatment of Multiple Adjacent Gingival Recessions: Case Report with a 1-Year Follow-Up","authors":"Hikmat Bakhishov","doi":"10.19080/adoh.2021.14.555886","DOIUrl":"https://doi.org/10.19080/adoh.2021.14.555886","url":null,"abstract":"Background: This article depicts a clinical case with 1 year follow-up in which de-epithelialized gingival graft (DGG) was used with the tunnel technique to treat multiple gingival recessions (GRs), and describes a technique used to intraoral de-epithelialization with diamond burr. Methods: A 45-year-old male patient was referred for evaluation and treatment of anterior maxillary multiple GRs. DGG with the tunnel technique was proposed to coverage of Cairo Recession Type I GRs on teeth #21 through #22 and an on tooth#23. Tunnel bed area had been prepared with microblades and tunnel knives. Donor are measured and de-epithelialized with diamond burs intraorally. After DGG harvested split cross-sectionally. The DGG was placed through the tunnel bed with auxillary sutures and sutured with vertical double-crossed technique. Results: Ten days after the surgical procedure, the tissue region was homogeneous where there is not uncovered DGG part, and clinically increased tissue thickness was observed. After 1 years of follow-up, the mean coverage 92.2%. Besides, increasing of tissue thickness was observed. This has not been reported previously. Sixty-one patients (41 males, 20 females) who were treated for obstructive sleep apnea with an oral appliance were enrolled in this study. Mean age and body mass index were 55.5±12.7 years and 24.4±4.0kg/m2, respectively. Patients were classified into groups based on body mass index (non-obese group, < 25.0kg/m2 and obese group, ≧ 25.0kg/m2) and severity of RDI (mild group, 5 ≤ RDI < 15 and moderate and severe group, RDI ≥15). RDI was measured by in-home (portable) monitoring and oral function was measured as maximum occlusal force, tongue pressure and labial closure force. A significant negative correlation was found between labial closure force and RDI in the total cohort and in non-obese cases (P<0.05). This study suggests that labial closure force may affect sleep-disordered breathing, supporting past reports. In the course of this COVID-19 pandemic, Emergency Root Canal Treatment is often needed to treat symptomatic irreversible pulpitis with or without space infections. Since dental practitioners cannot often maintain a safe interpersonal distance of more than a meter and are exposed to saliva, blood, and other body fluids during surgical operations, they are especially vulnerable to SARS-CoV-2 infection. Furthermore, many dental procedures produce aerosol, increasing the risk of contamination spread through the breath. Aim: Prioritizing this situation, we aimed to investigate the impact of HPV on well-established risk profile of oral cancer in Sri Lanka, using a representative sub sample consisted of 29 oral squamous cell carcinoma cases(OSCC) and 25 fibro epithelial polyps (FEP), from a large case control study conducted in Sri Lanka. Methodology: Incisional biopsies of cases and excisional biopsies of controls were collected, transported, stored at =80 0 C and dispatched as frozen tissues. DNA was extracted from frozen specimens, using the Gentra Puregene Tissue kit (Qiagen, Germany), solid tissue protocol according to the manufacturer’s instructions. Nested PCR was performed to detect HPV and HPV geno types were discovered by sequencing. Results: HPV was detected in 4/54 (7.4 %) of overall samples. Upon sequencing, high risk HPV α 9 and HPV 31 were detected in controls and not in the HPV positive OSCC case. between cases and controls*. Moreover, statistically significant difference was obtained in mean ages of cases and controls. (p= 0.0001)*. Cases were older than controls. Conclusion: There was no impact of HPV on established risk profile of oral cancer in Sri Lanka.
