{"title":"Implementation of a varicose vein module added to Swedvasc, the Swedish National Registry for vascular surgery.","authors":"Lena Blomgren","doi":"10.1177/02683555231201757","DOIUrl":"10.1177/02683555231201757","url":null,"abstract":"<p><strong>Objectives: </strong>The addition of a varicose veins (VV) module to the existing Swedish National Registry for Vascular Surgery (Swedvasc) and its impact on quality of care were evaluated.</p><p><strong>Methods: </strong>Vascular departments and private VV clinics were invited to enter data from 2016.</p><p><strong>Results: </strong>Registrations were approximately 10,000 yearly but dropped to 5390 in 2022 when a fee was introduced for private clinics due to reduced funding. 88% more interventions were reported to Swedvasc 2020 than to the National Board of Health and Welfare. Regions differed in interventions per 100,000 inhabitants/year from 21 to 233 and in preoperative CEAP C4-6 from 30.8%-90.4%. Follow-up was 9.4%. These data contributed to the decision to prioritize the patient group for national guidelines and pathways of care, which will be monitored by Swedvasc.</p><p><strong>Conclusions: </strong>A national VV registry with high coverage is possible and can contribute to national quality of care. Long-term outcome is still lacking. The purpose was to report long-term outcome and to analyse possible predictors for failure.</p><p><strong>Methods: </strong>The study was a retrospective pooled analysis of two prospective cohorts previously reported, but with prolonged long-term outcome up to 5-years follow-up.</p><p><strong>Results: </strong>163 treated legs were analysed. Mean follow-up was 5.4±0.6 years, in which 33 total failures occurred. Four procedures were partially successful. VCSS improved significantly and remained stable after 1 and 2-years, but significantly rose again after 5-years. AVVQ dropped significantly, but increased after 1 year to 4.3 and 6.1 at 5-years follow-up.</p><p><strong>Conclusion: </strong>MOCA was effective with minimal pain perioperative. However, anatomical success deteriorated after 1-year and showed even less results after 5-years, mainly due to partly recanalization, while clinical results were less affected. {"title":"Durability and efficacy of the ELVeS® Radial® 2ring slim fiber for multiple ablations.","authors":"Jürgen Verbist, Veerle Laeremans, Fien Gryffroy, Wouter Van den Eynde, Caroline Heerinckx, Dorien Haesen","doi":"10.1177/02683555231193883","DOIUrl":"10.1177/02683555231193883","url":null,"abstract":"<p><strong>Objectives: </strong>To prove that the ELVeS® Radial® 2ring slim fiber (Biolitec AG, Wien, Austria) is safe and effective, with a reliable fiber durability.</p><p><strong>Methods: </strong>This prospective, non-randomized, registry recruited patients with venous incompetence of the great saphenous vein, with or without concomitant incompetence of the small, anterior accessory and posterior accessory saphenous veins. Anatomic success, clinical success and pain were assessed.</p><p><strong>Results: </strong>150 patients (410 veins) underwent endovenous laser ablation (EVLA). Mean number of veins treated was 2.7 (1-7) with a mean total length of 80.7 cm. 98.3% occlusions were observed after 12 months. Clinical improvement was significant with reduced venous clinical severity scores at 1 month (1.7 ±1.9; <i>p</i> < .0001) and 12 months (0.7 ±1.3; <i>p</i> < .0001) versus pre-operation (4.4 ± 2.2). Mean pain level after 1-week was 5.8/100. No major surgery- or device-related complications occurred.</p><p><strong>Conclusions: </strong>1470-nm EVLA with the 2ring slim fiber is a highly effective treatment option well tolerated by patients. {"title":"Comparison of cyanoacrylate closure and radiofrequency ablation for the treatment of small saphenous veins.","authors":"Oguz Arslanturk, Fatih Gumus","doi":"10.1177/02683555231198625","DOIUrl":"10.1177/02683555231198625","url":null,"abstract":"<p><strong>Background: </strong>The objective of this study was to compare the early and mid-term results of radiofrequency ablation and cyanoacrylate ablation used in the treatment of small saphenous insufficiency.</p><p><strong>Methods: </strong>A total of 84 patients with isolated small saphenous vein insufficiency who underwent either cyanoacrylate ablation (CA) (Group 1, <i>n</i> = 40) or radiofrequency ablation (RFA) (Group 2, <i>n</i> = 44) were analyzed retrospectively.</p><p><strong>Results: </strong>The occlusion rate of target vessel was 95% in Group 1 and 93.1% in Group 2 patients, respectively, at 1-year follow-up without any significant difference. LEVVs were selected based on diagnostic criteria of Clinical, Etiology, Anatomy, and Pathophysiology levels 2-5 and classified into six types on the basis of their anatomical positions. The MTrPs in the lower extremities were localized in accordance with the classification of LEVVs and treated by MTrPs acupuncture combined with self-massage and self-stretching. The interval between each treatment was 2 weeks to 1 month, depending on needling pain tolerance of each patient. An in-house evaluation was used to estimate the proportion of varicose veins in the lower limbs and their accompanying symptoms. The treatment effect was evaluated before each treatment and at 1-year follow-up.</p><p><strong>Results: </strong>The mean evaluation score of LEVVs before the treatment course was 3.66 ± 1.19. After the course, this reduced to 1.18 ± 0.97, with the following response rates: 85% for excellent and good and 15% for medium. After 1-year follow-up, the mean evaluation score of all patients was 1.11 ± 0.92, with the following response rates: 87% for excellent and good, and 13% for medium.</p><p><strong>Conclusions: </strong>In some patients, MTrP acupuncture could cure LEVVs and its accompanying symptoms. Data collected at baseline and after 14 days.</p><p><strong>Results: </strong>Edema was significantly equal reduced to follow-up; mean -129.0 cm<sup>3</sup> (SD 105; <i>p</i> = .004, Class I) and -223.7 cm<sup>3</sup> (SD 120; <i>p</i> = .002, Class III), respectively. The investigational stocking lost significantly less compression pressure than the comparator stocking (<i>p</i> ≤ .013). Participants in both groups perceived significant improvement regarding leg heaviness, leg swelling, and feelings of tightness and tingling (<i>p</i> ≤ .016).</p><p><strong>Conclusion: </strong>The innovative investigational class I stocking appears to offer similar edema reduction and benefits to the comparator class III stocking. However, a larger and prolonged study is required. The demographic characteristics of the patients were questioned. All patients were assessed with maximum inspiratory and expiratory pressure (MIP/MEP) for inspiratory and expiratory muscle strength, photoplethysmography for venous refilling time (VRT), venous clinical severity score (VCSS) for disease severity, and 6-min walk test (6-MWT) for functional capacity.</p><p><strong>Results: </strong>The mean age of the patients was 49.48 ± 13.19 years, and the mean duration of disease was 9.18 ± 6.57 years. Samples were taken from pigmented regions and compared with control non-lesional samples from the same patient. Perl's Prussian Blue was used to identify haemosiderin and Schmorl's for melanin.</p><p><strong>Results: </strong>Seven patients presented with CVI, one with concurrent livedo vasculopathy (LV). One patient had LV only. Two patients had acroangiodermatitis (AAD). Six patients had post-sclerotherapy pigmentation (PSP), one with concurrent post-inflammatory hyperpigmentation (PIH). One patient had PIH only. The predominant pigment in CVI samples was haemosiderin. C5-C6 patients showed increased epidermal melanin. LV, AAD, and PSP samples showed dermal haemosiderin but no increase in epidermal melanin. PIH samples showed prominent epidermal melanin whilst no haemosiderin was detected.</p><p><strong>Conclusion: </strong>The predominant pigment in CVI and other vascular conditions was haemosiderin.
