CF patients showed little smaller SCN- serum levels, but the differences were not statistically significant. Prediction of outcome on the basis of perinatological data--results of multivariate discriminant analysis.","authors":"W Ihle,&nbsp;I Syllm-Rapoport,&nbsp;L Grauel","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of the prospective study described in this paper was to elaborate a prognostic screening method which allows to give an individual prognostic estimate for every newborn, with an accuracy acceptable for clinical purposes. This second part informs about the results of multivariate discriminant analysis, using the data from case histories as well as from the clinical and laboratory findings in the neonates (including \"criteria of hypoxia\"). According to the kind and number of abnormalities found on the occasion of out-patient consultations during the course of the first two years of life, and according to the developmental status reached at the age of two and six years, the children were classified under two alternative groups. Satisfying results were achieved in the discrimination between the groups of six-year-old children. Patients with insuline dependent diabetes show increased secretion on GH but secretion of IGF-1 may be normal, decreased or increased. These disturbances are especially distinct in the prepubescent period when susceptibility to develop complications is bigger. Increased secretion of GH and IGF-1 plays an essential role in the development of retinopathy and diabetic nephropathy. It was established that the use of pathogenic and associative markers, characteristic of the background state of a child as compared to individual peculiarities of pharmacokinetics and pharmacodynamics of a certain drug, will allow to forecast the efficiency of drug treatment of chronic diseases of breathing organs (bronchitis, pneumonia, asthmatic bronchitis) and digestion (gastritis, cholecystocholangitis). The use of aminophenazone marker in children with above mentioned pathology became the basis of research into efficiency of spasmolytics with similar type of metabolism (papaverini hydrochloridum, aminophyllinum, drotaverinum). In 39% of those children the diagnosis of an acute nonperforated appendicitis could have been ensured, in 39% the diagnosis of a perforated appendicitis was found, in 22% the was found, in 22% the laparotomy was negative. In cases of an acute perforated appendicitis typical symptoms were vomiting (100%), general stomach-ache (89%) and fever (61%). In most cases of an perforated appendicitis the state of patients was reduced drastically (80%), in 50% an ileus could be observed. Half a children with an acute nonperforated appendicitis as well as an perforated appendicitis had got a respiratory disease additionally. This fact was the main reason for the to late diagnosis particularly in the age up to 3. {"title":"[Gas gangrene in childhood--a review].","authors":"D Roloff","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Although gas gangrene in childhood is not mentioned in the paediatric literature, paediatric surgery textbooks included, this toxic wound infection also occurs in children. Prognosis totally depends on the early detection of the disease. Having checked over 2000 relevant literature sources, in which 110 cases of gas gangrene are mentioned, of which 24 are described in detail, this paper points to constellations paving the way for developing or favouring the infection. In aetiologic respect accidents range first, but also in children postoperative clostridial myonecrosis and those resulting from intramuscular injections are possible, but rare. On the basis of case reports the initial symptoms are stressed. In the foreground local (heavy pains, odematous swelling, \"dirty\" exudate, dyschromia, foetor) and general (tachycardia, rapidly deteriorating condition, shock, icterus) signs are dealt with, which occur after a latent period ranging between some few and 72 (up to 96) hours. Only through quick diagnosis and sufficient immediate therapy it will be possible to improve the prognosis and, in particular, reduce letality.
