Na, Jong Bouk Lee","doi":"10.4235/JKGS.2014.18.4.199","DOIUrl":"https://doi.org/10.4235/JKGS.2014.18.4.199","url":null,"abstract":"Background: To assess the functional outcomes and morbidity in patients undergoing holmium laser enucleation of the prostate (HoLEP) with and without previous transurethral prostate surgery. Methods: Patients were stratified into two groups, 558 patients who underwent primary HoLEP (group I) and 50 patients who underwent ‘secondary-HoLEP’ with prior transurethral prostate surgery (group II). Results: There were no significant differences in the preoperative parameters (median age, International Prostate Symptom Score [IPSS], prostate-specific antigen [PSA], prostate volume, maximum urinary flow rate [Qmax], and postvoid residual urine volume [PVR]). No significant intraoperative differences were noted in the use of total energy, resected volume, enucleation time, resection efficiency, and mean catheterization time. There were significant improvements in Qmax, PVR, and IPSS in both groups. Complications in groups I and II included 19 (3.1%) and 1 (2.0%) bladder injuries, 25 (4.4%) and 2 (4.0%) recatheterization for transient voiding difficulty, 20 (3.5%) and 2 (4%) cases of severe hematuria requiring additional treatment, and 5 (0.8%) and 0 cases for remorcellation due to remaining adenoma. Transient incontinence was the most common complication for both groups I and II, 30 cases (5.4%) and 3 cases (6.0%) respectively. Urinary tract infection showed improvement subsequent to antibiotic treatment. During the 6 month follow-up period, urethral stricture occurred in 7 cases (1.3%) for group I and in 1 case (2.0%) for group II. Ko","doi":"10.4235/JKGS.2014.18.4.205","DOIUrl":"https://doi.org/10.4235/JKGS.2014.18.4.205","url":null,"abstract":"Background: Incidence rate of ureterolithiasis has been increasing worldwide in general, with an especially rapid increase in the elderly, over 65 years of age. Therefore, the characteristics of geriatric patients diagnosed with uureterolithiasis in an Emer- gency Center were examined. Methods: A total of 613 consecutive patients who were diagnosed with ureterolithiasis through computed tomography from January 2012 to March 2014 were analyzed retrospectively. The patients were divided in two groups: the geriatric group and nongeriatric group. Results: Among the 613 patients, there were 64 geriatric patients (≥65 years, 10.4%) and 549 nongeriatric patients (<65 years, 89.6%). In the geriatric patients, 13 patients (20.3%) appeared without any pain or with atypical types of pain, while 39 of the nongeriatric patients (7.1%) had no pain or atypical symptoms, presenting a statistically significant difference (p<0.001). Gastrointestinal symptoms such as nausea and vomiting were more common in geriatric patients than in nongeriatric patients (42.2% vs. 29.9%, p=0.044), while lower positive rate of microscopic hematuria was reported (78.1% vs. 90.5%, p=0.002). Furthermore, in geriatric patients, the positive rate of costovertebral angle tenderness was lower and distal ureter stones were found to be more common, while urine pH and serum creatinine levels were higher. Conclusion: In comparison to nongeriatric patients, geriatric patients with ureterolithiasis showed lower rates of renal colic and hematuria, while showing higher rates of having gastrointestinal symptoms such as nausea and vomiting. The mortality rate is high at 12-27% and early diagnosis and prompt management can improve its prognosis. An 83-yearold woman was admitted to the hospital with abdominal pain. The patient previously had a hysterectomy and had received radiation therapy for uterine cancer. Plain abdominal x-ray showed typical findings of small bowel ileus with step ladder patterns. Computed tomography (CT) scan revealed biliary-enteric fistula with a 3-cm-sized gallstone in the jejunal loop. Surgical treatment was planned but due to the patient’s wish, conservative treatment was provided for 10 days. In the follow-up CT scan, the stone had moved to the distal ileum but intestinal perforation was suspected. Ileocecectomy and adhesiolysis were performed and the patient recovered fully. {"title":"The Relationship of Long-term Care Workers' Health Awareness on Life Satisfaction","authors":"Jung Gyu Park, H. The approach is based on the analysis of research on the influe nce of long-term care workers’ health awareness on their life satisfaction. Methods: The participants of this study include 600 long-term care workers working at Nursing homes and Nursing facilities located in Seoul. The data collected from the participants are analyzed with Multi Regression Analysis. Results: As a result of T-test and analysis of variance, statistically significant parameters to the subjective awareness of health are as follows: marriage, education, religion and work places. And as the minor parameters that influence the rate of life satisfaction the positive relationships between the emotional health (β=0.178, p<0.01) and the intellectual health (β=0.281, p<0.001) is statistically significant. Conclusion: The results of analysis indicate as follows: there is a positively significant relationship between social demographic characteristics, health awareness and life satisfaction; the higher participation in social activities, more women than men, and the higher income, the higher health awareness. The findings point out the importance of emotional and intellectual health and efforts to promote better health and demand for a societal change in health awareness.
