Our hypothesis was that falling FEV1, FEV1/FVC ratio or FEF25-75 or rising FeNO measurements are associated with future adverse asthma outcomes. Methods: Data were obtained from 7 trials using FeNO to guide asthma treatment. Predictors were change in % FEV1, FEV1/FVC ratio or FEF25-75 and % change in FeNO between baseline and 3 months. Loss of asthma control and asthma attack between 3 and 6 months were the outcomes. A one-stage individual patient data meta-analysis was conducted using a mixed effects logistic model, including a random effect for study. Results: Data were available in 1112 children (58% male, mean age 12.6 years [SD 3.1]) from seven trials. A reduction of 10% FEV1 was associated with increased risk for future attack (OR 1.28 [1.02, 1.58]) and for loss of control (OR 1.21 [1, 1.45]), and a reduction of 10% FVC with increased risk for future attack (OR 1.40 [1.04, 1.88]). Changes in %FEF25-75 and %FEV1/FVC ratio were not linked to future outcomes. Each 20% increase in FeNO was associated with increased risk for future loss of control (OR 1.041 [1.000, 1.062]). Baseline %FEV1/FVC was associated with increased risk for attack over the next three months (OR per % predicted reduction 1.037 [1.007, 1.067]). Otherwise no measurement at baseline was associated with later outcome. Conclusions: Change in %FEV1 over a three month period may be useful as part of dynamic risk assessment. Whether a large change in FeNO may also be useful for dynamic risk assessment remains uncertain.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"8 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-09-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116083966","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Characterizing these phenotypes aids in better management of asthma for individuals in the pursuit of personalized medicine. Aim: To identify and characterize the asthma phenotypes in an asthma-rich population of children. Methods: The study population included 752 twins, aged 9-14, selected from a larger twin study. Parents answered questions on symptoms, risk factors and medication use. Clinical tests assessed lung function and immunological biomarkers. A latent class analysis (LCA) using current and historical symptom data was used to generate asthma phenotypes (unsupervised). Multinomial regression was then applied to characterize these phenotypes further based on medication use, risk factors, biomarkers and lung function (supervised). Results: The LCA identified five phenotypes; healthy (61%), early transient wheeze (15%), persistent wheeze (5%), mild asthma (9%) and severe asthma (10%). All wheeze and asthma phenotypes were found to have reduced lung function, however early transient wheeze did not show reversibility with bronchodilators. Persistent wheeze was identified as an at-risk group as they had wheeze and sensitization but no asthma diagnosis and only used controller medication intermittently. Those with severe asthma were more likely to have used preventer medication and β2 agonists in the last year than those with mild asthma. They were also more likely to have a family history of asthma and an allergic profile. Conclusion: Characterizing asthma using supervised and unsupervised methods we have been able to identify several phenotypes that may improve the clinical management of asthma.","PeriodicalId":190855,"journal":{"name":"Paediatric respiratory epidemiology","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-09-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129736078","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patient involvement is integral and through partnering with the chILD Lung Foundation, Starke Lunge Foundation and European Lung Foundation, patient/parent contacts globally are being strengthened. Methods: To understand challenges that parents face and to inform activities, a mixed methods survey was devised and made available online in 9 languages from August - November 2017. Results: 156 respondents from 13 countries responded relating to the following chILD conditions: neuroendocrine cell hyperplasia (NEHI) (39%), undiagnosed/unknown (20%), bronchiolitis obliterans (15%), surfactant dysfunction mutations (SPC, ABCA3) (13%), pulmonary interstitial glycogenosis (PIG) (6%) and other (7%). Described by one responder as ‘a very lonely disease’, the most challenging aspects were: not knowing how the disease will progress (82%), anxiety/depression (74%), time management (67%), daily activities (64%) and feeding issues (60%). On rating aspects of treatment and care, 100% wanted better clinicians working interaction; 94% to know how to get a second opinion and 80% the option for genetic counselling. More availability of online information was requested by 95%. Future research direction included causes, genetics, risk factors, diagnostics and new treatments and reduction of impact of the condition on patients over the long-term.
