A week prior, he had undergone orchidectomy for testicular carcinoma. CT showed a cavitary lesion in the right upper lobe with a dense nodule. Bronchoscopy findings were inconclusive, VATS resection revealed a shrapnel. Every year, this type of cancer is newly diagnosed in almost 15,000 women and over 150 men in the Netherlands. Survival depends on the stage of disease and has improved over time. However, every year approximately 3100 women die of breast cancer in the Netherlands. Recent developments in breast cancer care focus on more breast and axilla conserving treatments, and the omission or more limited application of (neo)adjuvant systemic or additional radiotherapy if it is deemed safe. Taping of an umbilical hernia in children has been shown to accelerate the closure of the hernia defect. A five-week-old girl with a reducible umbilical hernia, associated with episodes of distress, was seen at the youth health care center. After consultation with a pediatric surgeon, the parents initiated adhesive strapping of the hernia. Within three months, the swelling had resolved completely. {"title":"[Being a good and happy doctor at the same time? This is how it's done].","authors":"Kiki M J M H Lombarts","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Over the past decade, discussions surrounding the medical profession have shifted from focusing on what makes a 'good' doctor to prioritizing the happiness and well-being of physicians. In discussions, being good and being happy are increasingly presented as opposites. Excellence seems to become a forbidden word, instead we focus on physicians being well. I argue that such polarization is both misguiding and unjustified. It distracts from what the conversation should be about: the loss of medicine as a relational service where the doctor-patient relationship is at the center. This does not negate the fact that good performance and professional happiness are under pressure - as I acknowledge in this article. The solution may be as simple as it is radical: reunite 'good' and 'happy' in conversations, in organizations, and in healthcare policies. The typical presentation of an atypical femoral fracture consists of a transverse subtrochanteric femoral fracture originating from the lateral cortex with a medial spike, occurring after minimal or no trauma. For patients presenting with a spontaneous femoral fracture who are using bisphosphonates, it is recommended to discontinue bisphosphonate use and to determine the future fracture risk. Imaging of the contralateral femur should always be performed in patients with an AFF. If radiological abnormalities are present, preventive surgical fixation of the contralateral side should be considered. We calculated incidence rates and determined changes in trends using Joinpoint regression analyses. Furthermore, we made a prediction of the number of early-onset colorectal cancer patients in 2035.</p><p><strong>Results: </strong>The number of new early-onset colorectal cancer patients has increased; from 6.3 per 100,000 in 1998 to 9.3 per 100,000 in 2023 (Annual Percentage Change 1.41% (95% confidence interval 1.18-1.71)). This incidence is expected to increase further in the future, reaching between 629 and 871 cases by 2035.</p><p><strong>Conclusion: </strong>There is an increase in the incidence of early-onset colorectal cancer in the Netherlands which appears to persist over the next 10 years. Guidelines recommend a multifactorial fall risk assessment for every older patient presenting with a fall. We describe two young infants who were presented with initially unexplained hematoma on the forearm. Additional medical examination was performed. Finally it turned out that the hematoma resulted from the vigorous sucking by the infant on the forearm with a so-called sucking bruise as a result. A thoroughly taken history is necessary and a fully head-to-toe exam is mandatory. When infants in the neonatal period present with bruises, the diagnosis of sucking bruise can be considered in some cases. {"title":"[Liberal blood transfusion in acute brain injury?]","authors":"Korné Jellema, Teun P Saltzherr, Bram Jacobs","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Whether a liberal or restrictive blood transfusion strategy is the best treatment option in patients with an acute, severe neurological disease, such as traumatic brain injury, an intracranial hemorrhage or a subarachnoid hemorrhage, is unknown. In this article we discuss the TRAIN trial and the current literature regarding a liberal versus a restrictive blood transfusion strategy in this patient category. The TRAIN trial showed a positive effect of a liberal strategy compared to a restrictive blood transfusion strategy in patients with an acute neurological disease. Another study as well as a meta-analysis were also unable to show a positive effect of a liberal blood transfusion strategy in patients with traumatic brain injury.
