The main location of thrombosis is in the extremity and pulmonary vein. Cerebral venous thrombosis is a rare location for thrombosis with high mortality. One of the risk factors for developing thrombosis is in association with surgery, especially during an active phase of Ulcerative Colitis (UC), and very few studies report cases occurring during the inactive phase. Hence, it is important for the surgeon to understand the risk of thrombosis, especially of cerebral sinus thrombosis in the Ulcerative Colitis patient. \u0000Case: We are presenting a fatal complication in a male patient of 62, who had sagittal sinus thrombosis and multiple intra-cranial hemorrhages in both frontal lobes 3 weeks after microvascular decompression surgery for hemifacial spasm.\u0000Conclusion: The risk of cerebral venous sinus thrombosis is higher in Ulcerative Colitis patients, especially in those who undergo surgery regardless of its type. Careful peri-operative assessment and treatment related to thromboembolism prevention are important to reduce complications in such cases.

Subsequent SCSA was diagnosed and treated with antibiotics. Strep pharyngitis could be a risk factor for SCSA in healthy patients, and conservative management may be adequate in treating strep A SCSA, depending on the patient’s risk factors and presentation. Our report reveals the necessity for establishing meaningful measures in staging and management to minimize risks of unnecessary harm.

The decision to intentionally leave an abdominal incision open may be required in the aftermath of surgery for intra-abdominal sepsis or where there is concern regarding abdominal compartment syndrome (ACS). Untreated ACS following elective, or emergency abdominal aortic aneurysm repair (AAA) is associated with mortality rates of up to 60% [1]. Management of the open abdomen, particularly in obese patient, is challenging and labour-intensive. The aim of wound management in this setting is to contain drainage, manage the peri-wound skin and facilitate closure of the wound. This case report discusses the successful management of a complex abdominal wound in a morbidly obese patient complicated by a colo-cutaneous fistula following emergency abdominal aortic aneurysm repair.

The pathologic fractures following osteomyelitis represent a possibly limb-threatening condition and if associated with a concomitant calcaneal ulcer, they present defiance for the surgeon and furthermore an outstandingly challenging condition to handle. \u0000Methods: A 61-year-old diabetic female patient on insulin therapy has been treated for a penetrating foot ulcer. In sequence the following procedures were carried out: Negative pressure wound therapy with instillation and dwell time (NPWTi-d), Negative Pressure Wound Therapy (NPWT), arterial percutaneous angioplasty (PTA), calcanectomy and temporary arthrodesis, application of Dermal Regeneration Template (DRT) and transplantation of microfractured autologous adipose graft (LIPOGEMS®), treatment of residual dehiscence with physical cold plasma (kINPen®MED). \u0000Results: The patient completely healed within 20 months and was able to walk without further aids. \u0000Conclusion: The authors point to the promptness within the treatment of pathologic calcaneal fractures, whether throughout the usage of technical devices targeting the reduction of the bacterial load and stimulation of healthy tissue, like in NPWTi-d and kINPen®MED. The use of a dermal regeneration template followed by skin grafting enriched with autologous microfractured adipose tissue (LIPOGEMS®), therefore represents today the most advanced regenerative surgical procedures.

Since intrahepatic cholangiocarcinoma (I-CCA) is an uncommon malignancy, treatment guidelines for I-CCA therapy have a low level of support, and guidelines for the treatment of recurrence are particularly challenging to establish. While a surgical approach is not clearly recommended for recurrent I-CCA in current guidelines, this approach has been reported as reasonable in a particular patient demographic and should be considered [1-3]. Understanding anatomy and leakage site is essential, although imaging techniques for thoracic lymphatics are challenging and not always available. We describe a patient with chylothorax post-esophagectomy from a left-sided thoracic duct traversing to the right chest. We applied intranodal Lipiodol® lymphangiography, computed tomographic lymphangiography, and performed fluorescence-guided thoracic duct ligation at the level of the aortic arch. These techniques are not only useful in a postoperative setting but may be used as preventive strategies to reduce morbidity and mortality from chylothorax after any thoracic surgical procedure where the thoracic duct is at risk.
