We present a case of a 55-year-old man who presented with a one-day history of abdominal symptoms. Emergency abdominal computed tomography (CT) revealed transverse colon volvulus, leading to immediate laparotomy and successful detorsion with resection of the affected segment. Postoperative recovery was uneventful, and the patient was discharged in stable condition. This case underscores the importance of prompt diagnosis through imaging, particularly CT, in guiding appropriate therapeutic measures. Additionally, the discussion highlights the rarity of transverse colon involvement, predisposing factors, clinical presentations, and the necessity of surgical intervention, emphasizing the higher mortality rate associated with delayed diagnosis.

We report the case of a 76-year-old male diagnosed with poorly differentiated HCC that spontaneously regressed. Although the patient did not undergo any treatment or invasive diagnostic tests, such as angiography, before surgery, the resected tumor showed complete necrosis similar to that seen after transcatheter arterial embolization. The main cause of the necrosis in the present case was thought to be the occlusion of nutrient vessels and tumor hypoxia. Tumor hypoxia and immunological reactions are reported to be the main causes of spontaneous HCC regression, although the underlying mechanisms remain unclear. In this case report, we provide additional insights into this phenomenon.

Often, SIH is misdiagnosed condition.\u0000Case Presentation: A 37-year-old female patient, presented with a four-month history of severe headaches. The first attack of headache, which was preseeded by one month of tinnitus, was in the fifth month of pregnancy. Physical examination and laboratory investigations were unremarkable. Brain MRI with and without contrast revealed thick diffuse meningeal enhancement. The MRA and MRV studies were unremarkable. Then, the patient’s headache worsens despite conservative therapy. MRI myelogram revealed a leak from the cervical nerve roots. Once the location of the leak was confirmed, a cervical epidural blood patch was performed successfully. The postoperative period, four days, was uneventful with clinical improvement of the patient’s headache and tinnitus.\u0000Discussion: Spontaneous intracranial hypotension is a challenging event that requires rapid diagnosis and management. Usually, the clinical history is suggestive of SIH diagnosis but still, it is underdiagnosed. Both imaging modalities and diagnostic criteria are important diagnostic tools for SIH. Several treatment modalities are available, including conservative options, epidural blood patches, and surgical intervention.\u0000Conclusion: Targeted epidural blood patch is a valid approach to managing the SIH that has been performed after a failed trial of conservative measures. This condition is usually misdiagnosed due to its non-specific clinical features. Radiological investigations are essential to evaluate and diagnose gallstone ileus cases.\u0000Case Presentation: A 92-year-old female patient, cholecystomized, presented to our hospital with a one-week history of an acute confusional state, generalized abdominal pain, and vomiting. The physical examination showed a disoriented patient with a distended abdomen and generalized tenderness, especially in the epigastric area. Hyperactive bowel sounds were audible. Laboratory investigations were non-specific. A plain abdominal X-ray was insignificant. The abdominal ultrasound showed marked abdominal gaseous distention. Then, an abdominal CT scan was performed, revealing an impression of gallstone ileus causing small bowel obstruction with no evidence of fistula formation. So, surgical intervention was decided, and an 8-gram gallstone was exteriorized through an enterolithotomy. The postoperative period was uneventful. \u0000Discussion: Gallstone ileus is an unusual entity that often affects elderly females. The clinical features of gallstone ileus include abdominal pain, abdominal distension, nausea, and vomiting. Diagnosis of gallstone ileus relies on imaging and physical examination. Abdominal CT with contrast provides a definitive diagnostic tool. The surgical options to manage the gallstone ileus can be either one or two-staged enterolithotomy. \u0000Conclusion: Due to the rare etiologic origin of the gallstone ileus in post-cholecystectomy cases, we report the case of a 92-year-old female with an acute confusional state and generalized abdominal pain, found to be caused by detected gallstone ileus, lacking evidence of fistula formation.
