And may present with compressive symptoms such as dyspnea, dysphagia, dysphonia or sleep apnea. Treatment of choice is total thyroidectomy and removal of the intrathoracic portion of thyroid. However, these cervicomediastinal masses may be separately encapsulated, and a sternotomy may be required for complete and safe excision of the mediastinal mass to achieve decompression of the surrounding structures and preventing the hemorrhagic complications if attempted from cervical incision

It used to occur at femoral ring but occurrence at ventral hernia is\u0000extremely rare.\u0000Case presentation: We report a 60-year-old female patient with tender paraumbilical hernia with absolute\u0000constipation and vomiting. On examination the patient had a low-grade fever, while the swelling was tense,\u0000tender with no impulse on cough. Plain radiograph abdomen erect position was done revealing multiple air\u0000fluid level. Repair was done after invagination of the ischemic part of the bowel loop.\u0000Conclusion: ventral hernia may be of a Richter type. Management of ischemic loop in Richter hernia does\u0000not necessitate resection and anastomosis, as invagination only is enough. Repair of the defect is mandatory\u0000while mesh placement is conditional.
