Evaluation of the effectiveness of methods for the treatment of chronic ischemic retinopathy associated with hemodynamically significant stenosis of the internal carotid artery (ICA). Material and methods. 40 patients were included in the research (25 men, 15 women) with grade 3 chronic ischemic retinopathy (CIR) aged 61 to 75 years (average 73 ± 6.2). Depending on the treatment, the patients were divided into 2 groups. The 1st group 20 people who received combined treatment. The 1st stage was laser coagulation of retinal nonperfusion zones. Then, after 2 weeks, during the 2nd stage carotid endarterectomy (CE) of the ICA was performed. The 2nd group 20 people who underwent CE ICA. In addition to standard research methods, Doppler ultrasound was performed (Logiqe, General Electric, USA). Results. The use of a staged approach to the treatment of severe CIR led to an improvement in functional parameters and reduced the period of rehabilitation of patients after the treatment by 2 times. Conclusion. The technology of staged treatment of CIR proposed by us makes it possible to stop the progression of retinal ischemia, to avoid the occurrence of complications that lead to loss of vision. The criteria for their inclusion in the study were the presence of a verified diagnosis of atopic dermatitis (in the acute stage) (48 children) or ichthyosis (vulgar and congenital) (17 children). All were examined for the severity of subjective discomfort, the condition of the ocular surface, the production and stability of the pre-corneal tear film. The results obtained indicate the presence of clinical and functional signs of DES (mainly due to dysfunction of the meibomian glands) in 31 children (64.6%) with atopic dermatitis and 14 (82.4%) with ichthyosis. At the same time, in children with the studied types of chronic dermatoses, DES occurred mainly in mild and moderate forms. It is advisable to conduct an examination of patients with other forms of chronic dermatoses in order to identify and treat their meibomian gland dysfunction and the associated dry eye syndrome. Material and methods. The study was crosssectional and included a comparative analysis of the effectiveness of face-to-face and remote examinations by an ophthalmologist for suspected glaucoma. According to the results of the examination, 40 patients (52 eyes) were diagnosed with suspected glaucoma. The diagnosis was made after a previous face-to-face examination by an ophthalmologist using standard methods for examining patients with POAG. Results. Based on the analysis of the data received from the operator during a remote consultation via telemedicine, it was possible to determine the diagnosis of suspected glaucoma in the eyes of 40 eyes, which accounted for 76,9% of the total number of cases detected during face-to-face examination. The calculation of indicators shows that the sensitivity of “teleglaucoma» screening was 81.25%, the specificity was 95.2%, the positive predictive value was 94.5, and the negative predictive value was 81.1. Conclusion. The results of the study prove the high potential for the full implementation of the “teleglaucoma» system in clinical practice, which was clearly demonstrated by the use of this method in the process of disease screening. To study the dynamics changes in the internal and deep structures of the optic nerve head (ОNН) against the background of three monthly injections of an anti-VEGF drug. Material and Methods. The study included 20 patients (20 eyes) with newly diagnosed exudative form of age-related macular degeneration (nAMD). Intraocular pressure (IOP) was measured with an ICare Pro before intravitreal injection (IVI), 1 minute after IVI, 30 minutes, 60 minutes during the first, second, and third injections. All patients underwent optical coherence tomography of the optic disc using a Heidelberg Spectralis OCT scanner in the Enhanced Depth Imagine mode. The study was carried out before IVI anti-VEGF drug, 1 month after the first injection, a month after 3 injections. Results. Statistically significant increase in IOP was found 1 min after IVI in dynamics (p < 0.001). Against the background of three loading IVI of the anti-VEGF drug, there was a tendency to increase the size of the excavation width, excavation depth, distance to the cribriform plate, however, no statistically significant changes were detected. Thus, 4 months after the start of antiangiogenic therapy after three IVI anti-VEGF drugs, increasing IOP immediately after IVI, which is transient, did not lead to morphological changes in the ONH. Conclusions. Аccording to optical coherence tomography, in patients with nAMD against the background of three loading IVIs of the antiVEGF drug, no statistically significant changes in the parameters of the internal and deep structures of the ONH were observed. To compare the anatomical and functional results of various techniques macular hole (MH) closure. Material and methods. 24 patients (age from 67 to 81 years; 6 men and 18 women): stage III MH, complete detachment of the posterior hyaloid membrane. MH sizes were from 427x202 to 1392x758 µm. Initial best-corrected visual acuity (BCVA) were from 0.03 to 0.2. Endovitreal intervention 27G with removal of the posterior hyaloid membrane, complete membrane peel was performed. In 18 patients, tamponade was performed with silicone oil, in the remaining 6 patients – with air. 2 groups were formed, comparable in age, sex, MH diameter. In the 1st group (12 patients) vacuum massage was performed, in the 2nd group (12 patients) – partial membrane peel with inverted internal limiting membrane flap technique. Results. All surgery perfomed without complications. In 18 eyes with silicone oil tamponade, all MH were completely closed, BCVA was from 0.15 to 0.5. In 2 of 3 eyes of the 1st group with combination of vacuum massage and pneumotamponade, incomplete closure of the edges of the MH with residual defects 112 µm and 84 µm, respectively, was noted. BCVA ranged from 0.1 to 0.2. In the third case of combination of vacuum massage with pneumotamponade, the MH completely closed, the BCVA increased from 0.2 to 0.6. In all 3 eyes, using the inverted flap technique and pneumotamponade, the MH were completely closed, the BCVA ranged from 0.45 to 0.55. After 1 month after ended of silicone oil tamponade in all 18 patients, the MH were closure, BCVA values increased from 0.25 to 0.5. Conclusion. After a month of follow-up, better anatomical results of complete MH closure were shown by the inverted flap technique in combination with air or silicone oil tamponade. After silicone removal, complete MH closure occurred in 10 eyes of the 1st group and 12 eyes of the 2nd group. In the future, the terms of dynamic observation of these patients will be continued for more objective assessment of the long-term effectiveness of both methods. Macular rupture as a complication of vitreomacular traction syndrome was the least favorable prognostic situation compared to traumatic (p < 0.01) and myopic ruptures (p < 0.05). To present a case report of serpiginous chorioiditis to perform the differential diagnosis for treatment. Material and methods. The patient, 33 year old, with a decrease of visual acuity. The comprehensive ophthalmologic examinations ware carried out, including the examination of best corrected visual acuity (BCVA), slit lamp indirect ophthalmoscopy, microperimetry, optical coherence tomography (OCT), OCT – angiography, fundus autofluorescence (AF). Results. During the examination the diagnosis was «OU Classical serpiginous chorioiditis with activity on the left eye. Subretinal neovascular membranes type 2 on the left eye». Conclusions. A non-infectious cause of serpiginous choroiditis was diagnosed. It was recommended the glucocorticosteroids' treatment. In this case, the process was complicated by the development of macular neovascularization, which required the administration of the angiogenesis inhibitors. To evaluate the effectiveness of the retinal pigment epithelium and choroid autotranslocation performed for traumatic rupture of the choroid in the macular area. Material and methods. A 30-year-old patient with subretinal fibrosis due to traumatic rupture of the choroid in the macular area underwent subretinal surgery with retinal pigment epithelium and choroid autotranslocation. Visual acuity and morphological status were assessed according to optical coherence tomography and ophthalmoscopy. The observation period is 12 months. Results. No side effects and complications were detected during the entire observation period. Visual acuity increased from 0.05 initially to 0.15 by the end of the observation period. The RPE-choroid graft is located in the macular area without signs of exudation and fibrosis, the fovea is in the center of the retinal pigment epithelium – choroid graft, the pigment epithelium is viable. Conclusion. Subretinal surgery with retinal pigment epithelium and choroid autotranslocation is the method of choice in the treatment of post-traumatic rupture of the choroid in the macular area, that can lead to a significant improvement in visual functions. The aim of the research was to study the effectiveness of surgical treatment of patients with retinal detachment complicated by macular rupture of the РVR В-C, depending on the method of subretinal fluid drainage. We studied the data of 18 patients (18 eyes), which were divided into 2 groups depending on the presence of a hole in the periphery of the retina. Patients of the 1st group underwent surgical treatment with drainage of the subretinal fluid through the central hole, patients of the 2nd group – with drainage through the primary peripheral hole. Visual acuity, microperimetry, and optical coherence tomography were used to assess the anatomical and functional outcome. Based on the analysis of the results of the study, it can be concluded that the peripheral method of drainage of subretinal fluid using a perfluoroorganic compound is better. This technique leads to a higher functional and anatomical result.
