Dawoud","doi":"10.59204/2314-6788.1100","DOIUrl":"https://doi.org/10.59204/2314-6788.1100","url":null,"abstract":"Objectives: To assess glycerol antibacterial activity against both Gram positive and negative microorganisms, detect impact of increasing glycerol concentration in bacterial cultures on extra/ intracellular Glycerol-3-phosphate (G3P) concentration, evaluate antiseptic efficacy of different glycerol/alcohol formulations and monitor HCWs tolerability to these formulations to recommend the best antiseptic preparation in vitro. Background: The progressively rising nosocomial infections have enforced the need for novel infection prevention modalities. Glycerol emerges as a significant and cheap antibacterial agent against both Gram positive and negative organisms without any toxic effects. Methodology: Different glycerol/ethanol formulations were evaluated for both bacterial inhibitory and bactericidal activity against selected bacteria (4 reference and 5 clinical strains). Effect of glycerol/ethanol combination was tested by checkerboard method. Colorimetric assay of extra and intracellular bacterial concentration of G3P was done. Skin tolerance of involved HCWs was evaluated against obtained bactericidal formulations including; 80% ethanol (E80)without glycerol (G0) as group (A), E80G1.45 (B), E80G20 (C)and E0G80 as group (D). Results: Microbial growth inhibition was detected at G40E40, G40E60, G1.45 E80 and G20 E80 for most strains. No visible growth (bactericidal effect) was detected with G80, G20 E80 and G1.45 E80. Glycerol/ethanol combination showed synergistic effect against Enterococcus faecalis and biofilm producing MRSA with additive effect against Proteus mirabilis, staphylococcus aureus. Acinetobacter baumannii displayed indifference. No antagonism was detected. G3P levels were positively correlated with added glycerol concentration. E80G20 achieved HCWs maximum tolerability (100%) followed by E0G80 (92.3%). Conclusion: E80G20 and E0G80 were superior to WHO recommended E80G1.45. Background: Thrombocytosis can be caused by either reactive or autonomous processes as a result of genetic alterations that affect signaling pathway through gain-of-function activity of MPL (myeloproliferative leukemia virus), Janus kinase 2 (JAK 2), calreticulin (CALR). Methods: This study was conducted on 60 Egyptian patients having clonal thrombocytosis attending to clinical pathology department, Ain Shams University Hospitals during the period from August 2020 to December 2021. Complete blood count, LDH level and MPL gene mutation by Real-time PCR were measured. Results: JAK2 gene mutation was more prevalent than MPL gene mutation in our study (40% and 8.3% respectively). MPL mutation has an aggressive nature than JAK2 mutation as evident by; older age (68.80± 2.28 vs 55.5 ± 8.28 accordingly), more thrombotic manifestation (100% vs 58.3%), higher platelets count (1221.4 ± 309.70 vs 879.38 ± 215.98) and poor response to therapy (0% of MPL positive patients achieved complete response to therapy vs 41.7% for JAK2 positive patients) but with no marked morphological difference regarding megakaryocytes morphology. Conclusion: MPL W515L/K mutations present in 10.3% in essential thrombocythemia Egyptian patients, included in our current study. MPL gene mutation has a bad prognostic effect. Background Phosphorus is essential element for various biological functions but data about serum phosphorus level abnormalities among critically ill children are limited. Patients and methods Prospective observational study including children admitted into Pediatric Intensive Care Unit (PICU). Serum phosphorus was measured on admission, day4, day8, and day12. Pediatric Sequential Organ Failure Assessment (pSOFA) score was calculated on admission. Hypophosphatemia and hyperphosphatemia risk factors were determined. Results Hypophosphatemia and hyperphosphatemia were generally non-severe. Hypophosphatemic patients had higher frequency of sepsis on admission compared with normophosphatemic patients (36.2% vs. 12.5%, p=0.019). Frequency of steroid use was higher among hypophosphatemic children (27.7% vs. 6.3%, p=0.017). Hypophosphatemia was associated with longer PICU stay (p=0.028) and hospitalacquired infections (p=0.022). pSOFA and frequencies of both sepsis and elevated serum creatinine were significantly higher among hyperphosphatemic compared with normophosphatemic patients. Hyperphosphatemia was associated with higher mechanical ventilation rate (p=0.018). No significant difference in mortality rate was found between hypophosphatemic, hyperphosphatemic, and normophosphatemic patients. However, phosphorus level on day8 was lower among non-survivors [median (interquartile range): 3.2 (2.3–4.7) vs. 3.9 (3.4–5.2), p=0.046]. Conclusion Phosphorus abnormalities, particularly hypophosphatemia, are common among critically ill children. Hyperphosphatemia was more likely to be found among patients with renal dysfunction, sepsis, and higher pSOFA score.
