Elgayar","doi":"10.59204/2314-6788.1026","DOIUrl":"https://doi.org/10.59204/2314-6788.1026","url":null,"abstract":"Objectives: To evaluate left ventricular (LV) systolic function in pediatric patients after ventricular septal defect (VSD) closure using conventional echocardiography and 2D speckle tracking derived strain. Background: VSD is a common congenital heart disease. Transcatheter device closure is an effective alternative to surgical closure with fewer complications. Methods: This prospective study included 54 children, divided into three groups: surgical closure (n=18), transcatheter closure (n=18), and control group (n=18). LV function was assessed before and one week and six weeks after closure using conventional echocardiogram and 2D speckle tracking derived strain including longitudinal, circumferential, and radial strain. Results: The study included 54 children aged from 6 months to 3 years. Perimembranous VSD was the most common type in the surgical group, while muscular VSD was the most common in the transcatheter group. Global longitudinal strain (GLS) was better in both VSD groups compared to control group before closure. LV ejection fraction (EF) and 2D global strain values decreased in both VSD groups after closure in the first assessment, but then recovered in the second assessment. Conclusion: There is a significant decrease in LV function immediately after closure detected by LVEF and 2D LV strain. The decline is more significant in patients who underwent surgical closure compared to transcatheter closure. Background: from the variable types of free flaps, the free transfer Latissimus dorsi (LD) musculocutaneous flap is considered a workhorse in lower limb reconstruction. Methods: A total of 16 patients with complex lower limb defects and deformities required surgical reconstruction were enrolled in this study. They underwent free LD flap reconstruction and followed up for 6 months. Results: The total number of patient was 16 patients; they had a mean age of 24.63 ± 11.9, with males constituting 75% of patients SD ( 11.94 ) .Most patients had RTA as a mode of injury (87.5%). Four patients (25%) presented to the hospital and underwent debridement after 6 months or more from the injury. At the end of follow up period, all the 16 patients had their constructed limbs preserved with no failure cases. Overall, three cases (18.8%) required reoperation (P = 0.013). Conclusion: The present study elucidates that the free LD flap had an excellent outcome in reconstruction of the lower limb distal extensive injuries, with 100% salvage rate, and low complication and reoperation rate. Background: Pneumonia is a serious health problem. TUS is quickly becoming one of the most useful non-ionizing radiation diagnostic techniques for identifying various lung illnesses. Methods: This prospective observational study was carried out in the Menoufia University Hospitals' Chest Diseases Department. From December 2022 to June 2023, it covered 50 patients who were clinically suspected of having pneumonia. The diagnosis was based on symptoms, signs, and chest HRCT. TUS and chest X-ray (CXR) were done and follow-up TUS after 7, 14, and 30 days. Results: TUS was able to identify consolidations in 44 out of 45 cases with HRCT consolidation, in which an air-bronchogram appeared in 37 cases, and a shred sign was in 17 cases. In addition, TUS revealed increased B lines in 7 of the 9 interstitial pneumonia patients, and pleural effusion was found in 14 cases. TUS had a sensitivity and specificity of 97.8% and 100% in the diagnosis of consolidation, with an accuracy of 98%, PPV100%, and NPV 83.3%, compared to 70% and 100% in the diagnosis of interstitial pneumonia, with an accuracy of 94%, PPV100%, and NPV 93%. Conclusion: TUS is more reliable than CXR for diagnosing pneumonia patients. Although there are a variety of alloplastic and autologous techniques, they all aim to achieve the same objective: the successful reconstruction of a breast mound that appears as natural as possible, Methods: This study will be conducted on 19 patients at General Surgery Department, Menoufia University and Patients will be randomly selected, Results: Among our studied patients 31.6% developed post-operative complications after reconstruction. Among our studied patients Seroma and Minor infection were developed in 31.6% for each followed by lack of symmetry in 26.3 than unacceptable scar, pain in 21.1% for each then areola Depigmentation, restricted arm movement, numbness or tingling in 15.8% for each then hematoma and delayed wound healing in 10.5% for each. There was a significant improvement of EORTC Quality of Life Questionnaire C30 (QLQ‐C30) 6 month after operation than before operation in the studied population. Conclusion: Women who had breast reconstruction with implants were quite happy with the results and experienced minimal complications. Reconstructive surgery on the breast has been shown to substantially improve both patient satisfaction and quality of life. In group B bilateral anterior QLB with (0.4ml/kg) bupivacaine 0.25% in each space between QL and psoas major muscle then continuous infusion of bupivacaine 0.25% at dose 10ml/hr. In group S bilateral anterior QLB with equal volume of 0.9 saline in the same plane.
