{"title":"[In the wake of Alphonse Laveran].","authors":"Patrick Berche","doi":"10.48327/mtsi.v3i2.2023.331","DOIUrl":"https://doi.org/10.48327/mtsi.v3i2.2023.331","url":null,"abstract":"<p><p>Alphonse Laveran (Nobel Prize 1907) played a pioneering role in discovering the causative agent of malaria, a disease that has existed since time immemorial, and long emblematic of the miasma theory until the end of the 19<sup>th</sup> century. In 1880, this unknown military doctor discovered the role of a hematazoan in malaria, designated <i>Plasmodium.</i> This was the first protozoan to be discovered in an infectious disease, at a time when bacteria were mainly suspected. This major discovery led to the identification of the role of mosquitoes in the spread of malaria by Ronald Ross (Nobel Prize 1902) and Battista Grassi. The objectives were to identify immunization stakeholders, describe their profile, their geographical distribution and determine their training needs.</p><p><strong>Methods: </strong>In December 2020, we conducted a census of Benin's immunization stakeholders via their administrative files and telephone interviews. The data were collected in each departmental directorate and health zone by focal points using an Excel<sup>®</sup> grid. The authorizations of the ethics and personal data protection committees were obtained.</p><p><strong>Results: </strong>We identified 3, 893 immunization workers, 39 years old on average, 66% of whom were women, 53% were health care assistants, 77% were vaccinators, and 56% were government contract workers. About 96% of them worked in peripheral centers. Approximately 56% were untrained and 14% had been untrained for more than 5 years. More than half of the actors were not trained for their function. The proportion of actors needing training was estimated at 70%. The training needs were at least 72% in the departmental hospitals, zone hospitals and health centers. 98% of actors reported a need for training, including immunization (74%), vaccine management (69%), cold chain (61%), monitoring (61%), and maintenance (47%).</p><p><strong>Conclusion: </strong>The predominance of nurses' aides and women and the low proportion of trained personnel among immunization workers were also observed in other sub-Saharan African countries. Its elimination as a public health problem seems to lead to its ignorance and therefore to a risk of late diagnosis. An analysis of leprosy surveillance data in Mauritania was conducted to determine epidemiological trends and clinical forms of reported cases.</p><p><strong>Material and method: </strong>The retrospective study was based on the epidemiological records of leprosy in Mauritania from 2009 to 2019. The diagnosis of leprosy was made on the basis of the diagnostic criteria of the World Health Organization (WHO). Data were analyzed using Epi Info version 7.2.5.0. The frequencies, proportions, and rates were calculated.</p><p><strong>Results: </strong>Over the past 11 years, 164 cases have been notified. Among the notified cases, 96/164 (58.5%) were males and 68/164 (41.5%) females, with a sex ratio of 1.4. The mean age (± standard deviation) was 44.0 ± 17.1 years [range, 9 - 86 years], and the median was 45 years [interquartile range, 32.5; 57.5]. Children under the age of 16 accounted for 9/164 (5.5%). The wilayas (i.e. \"regions\") of Nouakchott were the most endemic regions in the country. The multibacillary form (MB) represented 109/164 (66.5%) cases among the observed clinical forms. The average annual incidence was 0.3 case/100,000 population for MB and 0.1 case/100,000 for PB (paucibacillary). All reported cases were treated with multidrug therapy.</p><p><strong>Conclusion: </strong>The results of leprosy surveillance showed a persistence of this disease in Mauritania. The objective of this study is to determine the socio-demographic characteristics of Covid-19 patients during these epidemic waves in the province of Kongo central and to identify factors associated with deaths.</p><p><strong>Material and methods: </strong>This is a cross-sectional study of Covid-19 data from the provincial surveillance system. The data consisted of epidemiological surveillance data, laboratory data (tests), hospital data and patient follow-up data. We determined the characteristics of positive cases throughout the period and implemented logistic regression of factors associated with death.</p><p><strong>Results: </strong>During the successive waves, 9, 573 positive cases were reported in the province, 546 cases in the first wave and 6, 346 in the fourth wave. Seven positive cases out of 10 concerned people aged 25 to 64. The districts of Matadi, Moanda and Mbanza-Ngungu were the most affected. Age above 64 [OR: 7.2 CI:5.1-10.3] and wave 2 [OR: 4.4 CI:1.6-12.4] were the factors statistically associated with death.</p><p><strong>Conclusion: </strong>As in other African settings, age, comorbidities, higher socio-professional level and living in urban areas were the major risk factors for severe forms of the disease and death. At any time, it can evolve into a severe form (ex-pernicious attack or cerebral malaria) or even lethal. By reading again Alphonse Laveran's book on malarial fevers, we realized to what extent the observations made at that time allowed for a methodical and orderly description of the clinical forms of malaria, very close to what we can still observe today. No symptom or sign is pathognomonic of the disease. Only the detection of plasmodia or \"malaria microbes\" by direct or immuno-chromatographic methods allows for diagnostic confirmation, which is a prerequisite for the implementation of a curative treatment.Serendipity, synthetic chemistry and traditional medicine are the three methods that led to the discovery and large-scale production of antimalarial drugs. Serendipity for quinine, synthetic chemistry for chloroquine, and research conducted around traditional Chinese medicine for artemisinin and its derivatives. The latter have marked a real revolution in the management of malaria, both in its uncomplicated and severe forms. Le but de cette étude était d’évaluer les problèmes de stigmatisation et de discrimination des personnes vivant avec le VIH (PvVIH) dans les structures de soins et leurs implications dans l'accès aux soins de ces PvVIH. Méthodologie Il s'agit d'une étude transversale menée dans les 6 régions sanitaires du Togo ciblant les personnes âgées de 18 ans et plus, vivant avec le VIH. Un échantillonnage hybride associant un référencement en chaîne limité et un échantillonnage basé sur la cartographie des lieux de rencontre/d'activités a été utilisé. Les données ont été collectées à l'aide du questionnaire digitalisé de la version 2.0 de l'index de stigmatisation des PvVIH. Résultats Au total, 1 119 PvVIH d’âge moyen de 39 ans ont été incluses. Le sexe-ratio était de 0, 5, et 43, 4% connaissaient leur séropositivité depuis 1 à 4 ans. Au cours des 12 derniers mois les expériences de stigmatisation rapportées par les PvVIH de la part du personnel de soins étaient: les commérages (13%), la divulgation du statut sans le consentement (10%), ou le refus de tout contact physique (2, 6%). En matière de santé reproductive, il avait été conseillé à 2, 1% des enquêtés de ne pas devenir père/mère et 1, 4% avaient vu leur accès aux antirétroviraux (ARV) conditionné par l'utilisation d'une méthode contraceptive. Par ailleurs, parmi ceux qui étaient sous ARV, 28, 4% avaient manqué une fois leur dose de traitement au cours des 12 derniers mois par crainte qu'une personne apprenne leur séropositivité. Enfin, 39, 5% des enquêtés qui n’étaient pas sous ARV avaient la crainte que les travailleurs de santé les traitent mal ou révèlent leur séropositivité sans leur consentement. The current frequency of cleft is 0.3% among black people. The incriminated etiological factors of this congenital malformation are exogenous and genetic. The diagnostic is clinical and above all marked by the consequences linked to the cleft, which are multiple morphological problems entailing difficulties from the first days of life (sackling impossible in the event of total cleft lip and palate), functional disorders linked to cleft velopalatine by bucco-nasal reflux, major phonation disorder, dysfunction of the Eustachian tubes causing hearing and otorhinolaryngology (ORL) disorders, breathing disorders. Thanks to the progress of ultrasound, the diagnosis can sometimes be made during pregnancy. Obstetric ultrasound can diagnose cleft lip and palate from the 16th week of amenorrhea. The Veau-Wardill classification based on embryology makes it possible to understand the different types.The care is now organized in a multidisciplinary frameware where the coordination of each specialty will make it possible to harmonize the different treatments. The current attitude is based on the principe of early management to restore the functionality of the velopalatine muscle straps as quickly as possible, reducing the risk of phonatory and auditory sequelae.Concerning cleft palates, the data are fragmented and almost non-existent in the Central African Republic, hence the interest in publishing this clinical case highlighted by chance. The latter, among other things, had identified as natural hosts of leishmaniasis, rodent populations with which hematophagous telmophagous sand fly populations cohabited closely.When human populations emerged in these natural ecosystems, after the sedentarization of <i>Homo sapiens,</i> more or less important disturbances would have led to a transition of sand fly hematophagy, from zoophilia, to zoo-anthropophilia and anthropophilia.The creation of infrastructures that allow the breeding and integration into experimental groups of both holobiont sand flies and holobiont laboratory rodents (rats, mice, hamsters, etc.) remains crucial. This would mean forgetting his works that established the immunological origin of leprosy and its cutaneous, adverse and neurological complications.Another major aspect, the importance of his contribution to the treatment of the disease, in particular through the development of polychimiotherapy (PCT), which has made Institut Marchoux in Bamako one of the five WHO's collaborating centers in the field of clinical research in leprosy.Languillon was also involved in a holistic approach of the disease by creating the first leprosy surgery unit and implementing physiotherapy, orthopedic care with appropriate equipment, and social rehabilitation… without forgetting preventive aspects of complications through the necessary regular administration of treatments, and control of patients spread over wide territories, by creating a corps of leprosy nurse monitors and leprosy specialists.
