Pang, Xueqiang Sun, Guang Chen","doi":"10.1109/ITME.2016.0065","DOIUrl":"https://doi.org/10.1109/ITME.2016.0065","url":null,"abstract":"Objective To determine the incidence and significance of delphian lymph node metastasis in papillary thyroid cancer(PTC) and evaluate the clinical significance of delphian lymphnode metastasis in PTC patients. Methods We reviewed themedical records of 182 patients with pathologically confirmedPTC who underwent thyroid surgery and were detected delphian lymph nodes between September 2013 and January 2015 in the First Hospital of Jilin University. The relationship between related clinical factors and delphian lymph node metastasis was analyzed. The statistical significance of clinical factors were analyzed by logistic regression analysis, and were to further validate effect. Correlation analysis was carried between positive delphian lymph nodes and central paratracheal and lateral lymph nodes metastasis, the relationship between delphian lymph node metastasis and ipsilateral or contralateral VI region lymph node metastasis was analyzed. Results Delphian lymph node metastasis was related with factors of age, maximum diameter of tumor, capsule invasion, concomitant with Hashimoto's thyroiditis. Tumor larger than 1 cm and capsule invasion were the risk factors of delphian lymph node metastasis. Central and lateral lymph node metastasis rates were positive related with the number of metastatic delphian lymph nodes. When tumor was located in one side and with metastatic delphian lymph nodes, the metastasis rates of ipsilateral and contralateral VI region were higher. Conclusion Special attention should be paid on evaluation of delphian lymph nodes and intraoperative exploration. We should determine whether there are metastatic delphian lymph nodes before operationwhen patients have relevant risk factors. Methods: 12 patients with maxillary sinus cyst were selected, all cases were confirmed by the jaw of surface fault CT or MRI examination, disinfection of double flank cheek skin and oral vestibular mucosa, after local anesthesia or surface anesthesia all patients with nasal endoscopic examination of the nasal passages, were punctured into maxillary sinus cyst from inferior nasal meatus, the needle core was removed, the cyst fluid was taken out by cannula, then through cannula place the right amount of maxillary sinus cyst drug composition were injected. The bases were iodine and dexamethasone solution with 2:1 injection (i.e., take out 2 ml cyst fluid, inject of 1 ml drugs, and so on). As routine is not easy to puncture biopsy in place of the cyst, image guided puncture was choosen. Results: 12 patients with maxillary sinus cyst came back to the hospital for a check of sinus CT after treatment of 1 to 3 months, the results are excellent, and 1 to 2 years follow-up were carried on, the follow-up of 11 cases, 1 case was lost of follow-up. During follow-up of patients, before treatment with clinical symptoms caused by maxillary sinus cyst, symptoms disappeared after treatment, the sinus CT showed cyst disappear significantly. Because of the complex etiology and long duration, the readmission rate for patients is extremely high during therapy, consuming a large amount of medical resources as well as producing expensive hospitalization costs. In order to solve this problem, this paper summarizes the current situation and problems of risk prediction relevant to the readmission for patients. Combined with data mining technology, a risk assessment model for patients was designed, followed by the design and development of readmission risk assessment system for patients with cardiovascular disease. The risk assessment model includes three parts: risk prediction, clustering analysis and regression analysis of risk factors, which can automatically predicate the risk level and risk factors for the discharged patients in thirty days. The model was accurate 90.62% of the time. Four healthy male volunteers were enrolled in this study. For each subject, electrocardiography (ECG) and finger pulse were recorded in ten days to obtain the heart rate (HR) time series. For each day, the measurement lasted 150 min and was divided into six consecutive experimental sections: 3 repeat rest sections (each 30 min) and 3 repeat exercise sections (each 20 min). The repeat rest and repeat exercise sections were in turn. In each rest or exercise section, the segment of HR time series from the second 5 min episode (from the 6th min to the 10th min) was selected for the FuzzyMEn calculation. The effects of subject, day, repeat measurement and state (rest or exercise) on the FuzzyMEn were investigated. The mean FuzzyMEn differences among ten days and between rest and exercise states were also analyzed.
