Occurrence in pelvic lymph nodes must be differentiated from metastasis. We present a case of 30-year-old female who underwent staging laparotomy for suspected bilateral ovarian cancer. Histopathology revealed bilateral borderline serous tumor with two pelvic nodes having benign glandular structures of fallopian tubal morphology. WTI was positive in ovarian tumor and nodal structures but with pattern difference. The Ki-67 labeling index was 2% in nodal lesions as compared to 9% in ovarian tumor, confirming endosalpingiosis. Nodal endosalpingiosis must not be misdiagnosed as “metastasis” to avoid overtreatment in ovarian neoplasms. It is pertinent to recognize this entity rightly as it has a definitive role in clinical decision and treatment protocols. Glandular structures in lymph nodes must undergo a thorough workup, especially when associated with neoplastic lesions elsewhere.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Though treatment is mainly supportive care, understanding the pathophysiology and correcting the precipitating causes will help prevent the patient from progressing to a catastrophic outcome.\u0000 \u0000 \u0000 \u0000 This was a retrospective study conducted in the emergency department of a tertiary care hospital. Medical records of cases admitted from the emergency medicine department from January 2021 to July 2022 with bradycardia and hyperkalemia were retrieved from hospital software systems. Data regarding presenting symptoms, vitals, laboratory and radiological investigations, and treatment received were noted, and the data was analyzed.\u0000 \u0000 \u0000 \u0000 This study includes a total of seven cases that fit the definition of BRASH syndrome. The majority were elderly co-morbid patients with a median age of 53 (49-79) years, on anti-hypertensives, with varied presentations. They had mild to moderate hyperkalemia, with a mean value of 7.15 ± 2.31 mEq/L. Acute kidney injury was evident, with a mean creatinine value of 3.55 mg/dl. Initial ECG revealed bradyarrhythmias, with five junctional bradycardias and two sinus bradycardias. Among the seven cases, three were managed conservatively, three were dialyzed, and one underwent transvenous pacing. Six cases recovered and discharged home, with one mortality.\u0000 \u0000 \u0000 \u0000 BRASH syndrome is an under-recognized entity. Early recognition and treatment can avoid invasive procedures like pacing and dialysis and also prevent morbidity and mortality.\u0000","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Limitations of conventional smears have to be dealt with daily while reporting cytology. Despite the many advantages of the cell block method, it has been an underestimated diagnostic tool. The purpose of this study is to compare conventional smears and cell block preparations with different parameters and find out which one is more effective in evaluating body fluids.\u0000 \u0000 \u0000 \u0000 Sixty body fluids were studied by the conventional smear method and cell block method. Data were tabulated and analyzed.\u0000 \u0000 \u0000 \u0000 The cell block method showed a significant increase in cellularity (83.3%) compared to the conventional method (50%) [P = 0.0001, significant]. The limitation of low cellularity was significantly reduced by the cell block method (8.3%) as compared to conventional smears (33.3%) [P = 0.0007, significant]. Morphology preservation was significantly more by cell block method (95%) versus conventional smears (60%) [P = 0.00001, significant]. The yield of malignancy increased by 3.33% in the present study. Suspicious cases on conventional smears could be confidently segregated as malignant or benign by the cell block method.\u0000 \u0000 \u0000 \u0000 The cell block technique should be routinely used along with conventional smears to improve diagnostic accuracy and resolve diagnostic dilemmas on conventional smears.\u0000","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Modi","doi":"10.4103/mjdrdypu.mjdrdypu_818_23","DOIUrl":"https://doi.org/10.4103/mjdrdypu.mjdrdypu_818_23","url":null,"abstract":"ABSTRACT\u0000 \u0000 While the radio-cephalic arterio-venous fistula (RCAVF) remains the procedure of choice for hemodialysis, the value of the ulnar-basilic arterio-venous fistula (UBAVF) is perhaps underappreciated. The challenge of placing the forearm in a suitable and comfortable position for both the surgeon and the patient during the creation of the UBAVF may be an important factor inhibiting its use. We present a new method of hand positioning in the creation of the UBAVF that we have termed the “Royal Wave” position, which facilitates both patient comfort and surgical access intraoperatively. Patients suffering from chronic kidney disease stage -V were operated upon for the creation of UBAVF following the standardized protocol in the Royal Wave position. Patency (by Color Doppler) was recorded before discharge. Patients were clinically followed up for a period of 3 months. A total of five patients (four males) with the age range of 43–66 years were studied. The position was found to be comfortable for patients and suitable for surgeons. All five patients had successful creation of fistulas that remained patent, which was documented by Doppler report prior to discharge. The patients were followed up till 3 months post fistula creation. Thrombosis was not seen in any patient, and the basilic veins arterialized and were used for dialysis in all five patients. This innovation in positioning, the “Royal Wave” position improves the comfort of both the operator and the patient during surgery, and merits consideration when creating a UBAVF.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. EUS was first used clinically in 1980. Since its inception, it has emerged as a valuable diagnostic tool for GI lesions, to a versatile therapeutic modality, encompassing a wide range of indications. EUS faces challenges in distinguishing benign from malignant lesions. The partnership between cytopathologists and endoscopists, coupled with rapid on-site evaluation (ROSE), has become crucial in patient management. Recent technical advancements along with the addition of FNAC have greatly increased its diagnostic accuracy. Endoscopic ultrasonography-guided fine-needle aspiration cytology (EUS-FNAC) has become a routine practice in many healthcare facilities, aiding in the diagnosis of mediastinal, GI, and pancreatic malignancies. EUS-FNAC offers several advantages, such as multi-site aspiration and lower cost as compared to other procedures. Even its potential in the area of molecular studies has been recently investigated. In this case study, a 45-year-old man who underwent EUS-FNAC is presented. The combination of histology, immunohistochemistry, and EUS-FNAC allowed us to make a conclusive diagnosis of GI stromal tumors (GIST).","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. We report the case of a 21-year-old woman with recurrent episodes of lower respiratory tract infections requiring hospitalization since 2008. She was diagnosed with right middle lobe and lower lobe atelectasis and underwent fiber-optic bronchoscopy for further evaluation, which revealed a red, smooth, pedunculated mobile mass in the right bronchus intermedius. The post-bronchoscopy patient had hemoptysis. A review bronchoscopy performed for surgical management surprisingly revealed the disappearance of mass. The patient probably expectorated the mass during the episode of hemoptysis.
