The primary aim of the study was to analyze the vertical jump power (squat jump, countermovement jump, and countermovement jump with arm swing), Closed Kinetic Chain Upper Extremity Stability test (CKCUES test), throwing ball velocity, and handgrip strength among volleyball, basketball, and handball players, and the secondary aim was to find out the correlation of handgrip strength with all these parameters. Materials and Methods: This was a cross-sectional study where 75 university male volleyball (Group A, n = 25), basketball (Group B, n = 25), and handball (Group C, n = 25) players participated. Throwing ball velocity was measured with SRA3000 Radar Gun, vertical jump power was assessed by force platform (Kinematic Measurement System), and handgrip strength was measured by using a digital handheld dynamometer (CAMRY EH101). Results: By using SPSS version 21, one-way analysis of variance and Pearson correlation were found. Group A showed significantly higher scores in handgrip strength, vertical jump power, and CKCUES test as compared to Group B and Group C. Handgrip strength showed a positive correlation with throwing ball velocity, vertical jump power, and CKCUES test (P < 0.05). Conclusion: Significant differences in fitness and strength parameters were found among all the groups, and there was a positive correlation of handgrip with upper- and lower-limb strength. These findings will empower the coaches for objective selection of players on the basis of strength and fitness baselines as well as better understanding of injury risks.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Objective: The objective is to study the utility of CD200 expression by flow cytometry (FC) in various LPDs and plasma cell disorders. Materials and Methods: This is a retrospective study done over a period of 2 years. The study group included 52 cases with a clinical suspicion of LPD (n = 40) or plasma cell disorder (n = 12). Clinical data, morphological data on peripheral blood, and/or bone marrow examination were analyzed and correlated with the final results on FC. Results: Out of 40 LPDs, chronic lymphocytic leukemia (CLL) accounted for a majority of the cases accounting for 57.5% (23 cases). Plasma cell myelomas (PCM) were the most common plasma cell disorders accounting for 75% (nine cases). All cases of CLL showed CD200 expression and the two cases of mantle cell lymphoma (MCL) were CD200 negative. Splenic marginal zone lymphomas (MZL) involving marrow showed dim CD200 expression. Bright CD200 expression was also observed in all cases of hairy cell leukemia (HCL) and 67% of cases diagnosed as PCM. Conclusion: CD200 is a very useful marker in the diagnosis of various LPDs especially CLL, HCL, and PCMs. It can be used as an additional marker particularly in distinguishing CLL/small lymphocytic lymphoma (SLL) from MCL and atypical CLL from other CD5+ B-cell neoplasms and extranodal MZL.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Consequently, short-lived relations and multiple romantic or sexual partners. Methods and Material: The study aimed to assess couple satisfaction in relation to sexual satisfaction, mental health, and stigma in gay individuals. It was a correlational cross-sectional study conducted online through social media platforms using Google Forms. The assessment measures used were: Couple Satisfaction Index, New Sexual Satisfaction Scale, General Health Questionnaire, and Self-Stigma Scale. Out of a total of 115 posts, 92 responses were received from which a sample of 30 was analyzed using Statistical Package for the Social Sciences version 28. Descriptive statistics, Spearman's correlation, and regression were computed to determine predictors of relationship satisfaction. Results: The correlation between sexual satisfaction and couple satisfaction (r = 0.52, P ≤0.01) was significant indicating that sexual satisfaction increases couple satisfaction. Psychological distress and stigma were inversely related to couple satisfaction but not to sexual satisfaction. Conclusions: Sexual satisfaction largely determined the couple's satisfaction while psychological distress or stigma accounts for a small percentage of couple satisfaction.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Carbapenems have broad-spectrum antibacterial activity and thus known as Extended-spectrum β-lactamases. Therefore, the emergence of carbapenem-resistant bacteria poses great significance in HAI. Methods: The study was undertaken to see the prevalence of carbapenem-resistant Acinetobacter baumannii (CRAB) and carbapenem-resistant enterobacteriaceae (CRE) in patient samples and surrounding environment. During the study, all the patients' clinical samples sent to the microbiology department for culture sensitivity were considered. All isolates were processed according to standard microbiological protocols and reported according to the Clinical and Laboratory Standards Institute guidelines. Environmental swabs were taken from HI-touch areas from all intensive care unit (ICU)-admitted patients' rooms. Results: Around 290 isolates were reported during the study from concerned ICUs. Out of which 80 samples were carbapenemase producers. Environmental samples were collected from ICUs from Hi-Touch areas showed growth in 23 samples out of a total of 270. Carbapenem-resistant isolates identified in the study belonged equally to Enterobacteriaceae (most common-Klebsiella pneumoniae and Escherichia coli) and nonfermenters classes (CRAB and Pseudomonas aeruginosa), were of similar concern and also difficult to treat, and further compounds the morbidity and mortality of associated infections. The majority of carbapenem-resistant pathogens in this study were resistant to extended-spectrum cephalosporins, fluoroquinolones, and piperacillin-tazobactam (multidrug-resistant [MDR]). Interpretation and Conclusions: In a setting where CRE and CRAB persist, environmental contamination is common. Screening of patients, ensuring strict contact precautions, and strictly following decontamination guidelines in ICUs can be a powerful tool in discerning HAI caused due to CRE and CRAB.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. Most common histopathological variant is adenocarcinoma (>90%). Squamous cell carcinoma is an uncommon variant of carcinoma gallbladder. Its clinical presentation is similar to those with adenocarcinoma. They grow very rapidly and has diffuse hepatic and local organ infiltrations. Early detection and radical surgical resection remain the cornerstone of the definitive management. We report a case of locally advanced carcinoma gallbladder of squamous cell variant. Patient presented with features of acute cholecystitis and had palpable gallbladder mass. Clinically and radiologically, she was diagnosed as a case of carcinoma gallbladder and underwent exploratory laparotomy, radical cholecystectomy with right hemicolectomy. Common locations of occurrence include the diaphysis and metaphysis of femur, tibia and humerus with a juxtacortical position. The involvement of metatarsals is very rare. The diagnosis of osteoid osteoma rests upon its typical radiological appearance of central sclerotic nidus with lysis around along with non-traumatic pain out of proportion to the size and signs. He had a history of penetrating high-velocity impact to his Left eye (LE) two months back. He was unable to receive immediate proper ophthalmic care owing to COVID restrictions after sustaining trauma and continued with his daily work. On examination, vision in the RE was reduced to finger counting less than 1 feet. Clinical features of RE showed mutton fat keratic precipitates and reaction in anterior chamber and posterior synechiae. LE showed features of a traumatic subluxated cataract with a sclerocorneal injury. Fundus could not be evaluated in both eyes. B scan revealed retinal detachment and chorioretinal thickening in the RE. Patient was immediately started on oral corticosteroids and topical medication and the patient vision had drastically improved to 6/24 in one week and was able to walk around and perform his daily activities by himself. Prompt treatment of a case of sympathetic ophthalmia (SO) is necessary and essential for the patient to recover his vision to a functional capacity. It is a rare congenital anomaly which occurs from anomalous embryogenesis in which a malformed parasitic twin is found within the body of the normally developed host. In most cases, the parasitic twin is anencephalic and usually contains a vertebral column and budding limbs. The upper limbs are less developed than lower limbs, and are usually located in the abdomen of the autosite. Diagnosis is often made preoperatively with ultrasonography, plain radiography, computed tomography, or magnetic resonance imaging. Histopathological findings confirm the diagnosis and the recommended treatment is complete surgical excision of mass. Many drugs have been used to alleviate this pain with variable efficacy, among them lidocaine pretreatment is most popular. Propofol medium-chain triglyceride (MCT)/LCT emulsion has the ability to decrease pain on injection. The availability of propofol with two different formulations necessitates a comparison of pain on injection with lidocaine pretreatment. Aim: The aim of this study was to study the incidence and intensity of pain on injection with propofol LCTs versus propofol MCT/LCTs with lignocaine. Materials and Methods: This prospective, double-blind, and randomized controlled study included 120 American Society of Anesthesiology Grade I and II participants undergoing General Anesthesia. In Group A, patients were induced with 1% propofol LCT (2 mg/kg) with 2% lidocaine 2 ml and in Group B 1% propofol MCT/LCT (2 mg/kg) with 2% lidocaine 2 ml. Assessment of pain on injection was performed after 30% of total induction dose was injected at a rate of 1 ml/s by Verbal Rating Scale. Results: Group A reported an incidence of pain in 28.4% and Group B with 13.3%. There was a statistically significant difference in the incidence and intensity of pain with propofol MCT/LCT along with lignocaine. Sixteen patients in Group A and eight patients in Group B had mild pain, whereas one patient in Group A and none of them in Group B had moderate pain. Conclusion: Premixing lignocaine with MCT/LCT propofol significantly reduces both the incidence and intensity of pain as compared to LCT propofol with lignocaine.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y. The disease is prevalent in the rural population of Odisha. Clinical presentation of SCA and SBT is similar, though there is a difference in treatment modality for both the entities. However, there are a few studies to delineate between these two diseases in children below 15 years. Aim: A hospital-based prospective study was conducted in eastern India to assess the clinicohematological profile of children with SSA and SBT. Methods: The prospective study was conducted in Srirama Chandra Bhanja Medical College, Cuttack, Odisha, from January to December 2020. A total of 43 confirmed cases by Hb electrophoresis of SCD were enrolled. Detailed clinical manifestations and hematological and biochemical tests were recorded as per the pro forma. Results: Clinical findings validated the major manifestations of SCD as splenomegaly, hepatomegaly, jaundice, fever, abdominal pain, crisis, and myopathy. Most of the SCD patients have severe anemia (range; 2.7–12.4 g/dL) at presentation. Splenomegaly was found in 43.8% of SCA versus 85.2% of SBT cases. Jaundice was more marked in SCA (62.5%) compared to SBT (48.1%) cases. Both the mean corpuscular Hb and mean corpuscular volume are more in SCA compared to SBT. Conclusions: Presentation of SCA is more severe compared to SBT. Enhanced implementation of newborn screening programs in Odisha state will help in reduction of the disease burden and morbidity due to SCD.","PeriodicalId":18412,"journal":{"name":"Medical Journal of Dr. D.Y.
