Histologically they were characterized by hyperkeratosis, acanthosis, elastorrexis and enlarged collagen fibers, typical of the picture known as Costa's acrokeratoelastoidosis. The results concluded that minoxidil has a very low capacity of sensitization. He had a ischemic necrosis that was quickly progressive in his fingers and toes. It was necessary to amputate them. Diffuse vascular calcifications were recorded so radiologically than pathologically. PTH and phosphocalcic product were raised. Parathyroidectomy was practised with a quick initial improvement but immediately new distal ischemic lesions and keratotic papules with histologic perforation, that became necrosed, appeared in his buttocks and inferior extremities. Antibodies raised against cytokeratins 6 and 18 decorated the eccrine glands except the acrosyringium. The anti-CEA Ab revealed a uniform and exclusive staining of the eccrine glands. The anti-cytokeratins 8, 18 and 19 Ab labelled the apocrine glands and the secretory part of the eccrine glands. The acrosyringia were revealed by anti-EMA Ab. {"title":"[Disseminated annular granuloma in a patient with scabies].","authors":"R A Vignale, E Casella de Vilaboa, J Paciel, J Bruno","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A male patient of 52 years old is studied with an scabiei by Sarcoptes scabiei v. hominis. After a month the patient developed a clinic and histopathologic generalized granuloma annulare. With the specific local treatment of his scabiei, the disease was cured in 7 days. After this treatment, the generalized granuloma annulare disappeared within 2 months. Due to the sequence of both diseases and the way in which they disappeared after local therapy, we suggest that the antigens of the scabies are the ones responsible for the granulomatous lesions. We concluded this since once the antigens have disappeared, the immunologic mechanisms depending on the interactions of lymphocyte T-macrophage which form the granuloma are interrupted. Since the patient is homosexual, we studied the HIV antibodies which were negatives in several opportunities. Nevertheless, an immune disbalance which could be the coadjuvant factor was founded. The samples selected among the sera asked for syphilis serology at a clinical laboratory, were those which the results on VDRL reactions were observed to be reactive at low titles, not reactive or inconclusive. From these 305 samples, 87 (28.5 por 100) were also processed by the FTA-Abs test. The reactivity concordance between the tests was compared and the efficiency of the MIPHA as confirmatory diagnostic test was discussed. Previous publications have reported that some mycotic infections, including sporotrichosis, can produce a negative modulation of the immune response unfavorable for the host. In this work T and B lymphocytes capable to form direct and indirect rosettes respectively with sheep erythrocytes in gerbils with experimental sporotrichosis, were measured. It was found a significant rise in the number to T and B lymphocytes which means normal immunological reactivity. The histopathologic findings were typical for \"Eosinophilic Ulcer of the Tongue\". This was contiguous with a carious molar, the remotion of which was followed by total remission of the ulcer. Its possible aetiology and pathogenesis are discussed.
