{"title":"Vocal Health Knowledge Status and Attitudes of Professional and Amateur Performing Artists in Greece.","authors":"Eleftheria Iliadou,&nbsp;Joanna Giannopoulou,&nbsp;Vasiliki Bousi,&nbsp;Ilias Papathanasiou,&nbsp;Athanasios Bibas","doi":"10.21091/mppa.2023.1005","DOIUrl":"https://doi.org/10.21091/mppa.2023.1005","url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to investigate the vocal health literacy level and attitudes of professional and amateur performing artists in Greece and identify possible gaps and limitations that should be addressed in the future.</p><p><strong>Methods: </strong>This mixed-methods study took place in two successive stages. The first stage involved 4 focus groups with professional and amateur actors and singers. The focused discussions of this stage, which were moderated by one otolaryngologist and one speech and language therapist, provided necessary information for the design of the structured questionnaire used during the second stage of the study (online survey). The online questionnaire included questions about responders' knowledge and attitudes of vocal health. Although not compulsory, the Greek version of the Singing Voice Handicap Index (SVHI) was also part of the online survey.</p><p><strong>Results: </strong>The online questionnaire was validly answered by 305 respondents during a 10-month period (May 2020 to March 2021). Considerable knowledge gaps were observed regarding anatomy, physiology, and vocal disorders management. The vast majority of participants (94.1%) had at some point experienced problems with their voice. Gastroesophageal reflux (21.6%), vocal nodules (12.5%), and vocal polyps (3.9%) were the most frequently mentioned diagnoses in our sample. A small proportion of participants (9.5%) stated that they have never visited an ENT, while 84.3% have never seen a speech and language therapist. The primary goal of this study was an exploratory analysis of relationships between attenuation and perceptions of sound quality. Participants were 16 adults tested using commercial HiFi HPDs marketed for use at recreational music events. Survey responses did not reveal statistically significant differences in perceived sound quality across HiFi HPD brands. However, increases in music quality ratings were correlated with decreases in average attenuation at 3, 4, and 6 kHz and increasing uniformity of attenuation. The correlations were weak, however, explaining only about 15% of the variance. The data suggest listeners may prefer HiFi HPDs that provide more uniform sound attenuation at frequencies from 0.25 to 4 kHz and less attenuation from 3 to 6 kHz. We aimed to assess the prevalence of musculoskeletal pain and the factors associated with the presence of musculoskeletal pain among music students in public music conservatories in Tehran, Iran.</p><p><strong>Methods: </strong>Participants included 383 music students, age 12-18 years, from Tehran, Iran. A cross-sectional study was performed from October to November 2021 through convenience sampling. Demographic characteristics and musculoskeletal pain were collected using a self-report questionnaire, the Numerical Rating Scale, and the Nordic Body Map questionnaire, respectively. Postural behavior of participants was assessed using the Rapid Entire Body Assessment checklist.</p><p><strong>Results: </strong>In the convenience sample, 59.3% of music students reported at least one symptomatic musculoskeletal pain in the last week. Female gender (OR 2.34, 95% C:1.37-3.99, p<0.001), age (OR 1.66, 95% C:1.08-2.54, p<0.001), postural behavior (OR 1.07, 95% CI 0.81-1.39, p<0.001), and duration of playing (OR 0.97, 95% CI 0.95-0.99, p<0.001) were significantly associated with the playing-related musculoskeletal pain. History of playing and instrument type had no relationship to pain.</p><p><strong>Discussion: </strong>The main risk factors associated with playing-related musculoskeletal pain in music students were the participants' characteristics (female gender and age), postural behavior, and playing duration. This situation may lead to an asymmetry of spinal segments, which in turn may correlate with muscle contractions around the lumbopelvic area and can lead to musculoskeletal pelvic girdle pain. The aim of this study was to investigate the segmental mobility of the vertebral column in two groups of musicians, those with and without pelvic girdle pain.</p><p><strong>Methods: </strong>This study included 45 musicians who played their instrument for at least 10 years. Musicians were divided into two groups depending on if they had pelvic girdle pain or not. Spinal mobility was evaluated by a hand-held computer-assisted device called the Spinal Mouse system, and pelvic girdle pain assessments were evaluated using orthopedic tests including the active straight leg raise (ASLR), flexion abduction external rotation (FABER), pelvic pain provocation (P4), and long dorsal ligament palpation (LDL).</p><p><strong>Results: </strong>The current study found no significant differences in the thoracic, lumbar, and sacral curves in the trunk flexion tests between musicians with and without pelvic girdle pain (p=0.28, 0.54, and 0.63). However, analysis of spinal mobility revealed that musicians with pelvic pain had significant limitations of spinal flexion mobility, mainly in the thoracolumbar region in the sagittal plane (p=0.02 and 0.01).</p><p><strong>Conclusions: </strong>This study revealed a significant correlation between pelvic girdle pain and limitations of segmental spinal flexion mainly in the thoracolumbar region of the sagittal plane in musicians. But whether Louis Armstrong really had this problem or not has never been documented.</p><p><strong>Aims: </strong>This study is a biographical review and an analysis of the existing photographs of Armstrong's lip with the aim of better understanding the origin and progression of his disorder.</p><p><strong>Methods: </strong>A review was made of the most important biographies, newspapers, and magazines referring to Louis Armstrong and medical reports about Satchmo's syndrome. Major archives related to Louis Armstrong were searched for documents, photographs, articles, videos, letters, and other items, using the terms \"lip,\" \"embouchure,\" \"mouth,\" and \"problem.\" Information was sought on Armstrong's playing technique, playing schedule, lip changes, lip problems, and other related health issues.</p><p><strong>Results: </strong>It was found that Louis Armstrong suffered dermatological lip problems due to his intense playing schedule and the way he played from the very beginning of his career. This first created swelling and superficial erosions that later became cracks and fissures. These lesions finally produced ulcers which evolved into scar tissue after healing. This made the mucosa thicker and stiffer, making it more difficult for the lip to vibrate and forcing him to increase the tension and pressure in the area in order to play the trumpet. This created a vicious cycle with the scar tissue leading to further problems. These injuries even bled several times while he was playing and, on some occasions, left his mouth and clothes bloodied in the middle of concerts. His biography does not make any reference to difficulties or symptoms that could indicate that he had orbicularis oris muscle rupture, except for the fact that he had trouble playing, a symptom common to any injury to the lips in a wind instrument player.</p><p><strong>Conclusion: </strong>Analysis of the existing data indicates that Louis Armstrong's lip problems were most likely due to mucosa fibromatous hyperplasia resulting from chronic microtrauma rather than a rupture of the orbicularis oris muscle. To date, data from the professional dance sector concerning this matter are lacking. The aim of this exploratory project was to analyze and evaluate skin health in the foot area with regard to the prevalence of dermatoses, their locations, as well as gender-specific and load-specific differences.</p><p><strong>Methods: </strong>Professional classical and neo-classical ballet dancers were examined at two time points: in a phase with increased stress (T0: daily training sessions, rehearsals, and high performance frequency) (n=51, 35 females, 16 males) and after a 24-day rest phase (T1: n=35, 28 females, 7 males). In addition, gender-specific and load-specific (T0 and T1) differences were evaluated.</p><p><strong>Results: </strong>All professional dancers were affected by skin lesions of the feet at T0. Hyperkeratosis (96.1%), onychomycosis (27.5%), and subungual hematoma (11.8%) were the most frequent dermatoses of the feet of professional dancers. Onychomycosis affected the nails of the big toes in particular (right 15.7%; left 13.7%), and subungual hematomas were found exclusively on the nails of the first toe (right 7.8%; left 7.8%). Women tended to be more frequently affected by hyperkeratosis, men more frequently by onychomycosis. There were no load-specific differences between the stress and rest phases.</p><p><strong>Conclusion: </strong>The prevalence of work-related dermatoses is equally high among female and male dancers. The aim of this study was to examine whether there was a difference in manual and finger dexterity between musicians and nonmusicians, and between musicians divided by instrument groups and nonmusicians.</p><p><strong>Methods: </strong>The study included 128 participants (96 musicians, 32 nonmusicians). Musicians were divided into three groups as wind, string, and keyboard instrument players. The Minnesota Manual Dexterity Test (MMDT) for manual dexterity and Purdue Pegboard Test (PPT) for finger dexterity were used. Independent samples t-test and Mann-Whitney U test were used for MMDT, and the Mann-Whitney U test was used for PPT between musicians and nonmusicians. The Kruskal-Wallis H test and one-way ANOVA were used for comparisons of the MMDT and PPT between nonmusicians and musicians divided by instrument groups.</p><p><strong>Results: </strong>There were no differences between musicians and nonmusicians in terms of MMDT and PPT (p>0.05). When musicians were divided by instrument groups, there was a statistically significant difference in only the left-hand subtest of PPT (p=0.038). In pairwise comparisons, significant differences were found between wind and string instrument players (p=0.036), keyboard and string players (p=0.012), and nonmusicians and string players (p=0.015). Studies have emphasized the effectiveness of music-specific physiotherapy for affected musicians. This study was designed to evaluate if physiotherapy treatment of pain-affected music students had an impact on pain perception as well as psychological well-being. To explore the possible development of musculoskeletal pain, depression, and anxiety, a second sample of pain-free music students, matched for age and gender, was examined twice at identical time intervals.</p><p><strong>Methods: </strong>A convenience sample of 31 university music students with moderate to severe musculoskeletal pain and 31 pain-free music students, matched in age and gender, were included in the study. Both groups were examined physically and completed biographical, music-related, and psychological questionnaires. Perceived pain intensity was assessed with a visual-analogue scale (VAS), and depression and anxiety symptoms were assessed with the Beck Depression Inventory II (BDI-II) and the Hospital Anxiety and Depression Scale (HAD). Music students with pain received a series of 12 sessions of musician-specific physiotherapy, while controls waited for the same amount of time for retesting.</p><p><strong>Results: </strong>On the 10-cm VAS, music students with pain reported an average improvement in pain intensity from a baseline of 6.25 (SD 1.95) to 2.7 (2.03) after the intervention, while the controls (music students without pain) did not change. Furthermore, music students with pain indicated higher depression and anxiety scores as compared to the control group before and after therapy. After intervention, music students with pain with higher BDI-II scores demonstrated clinical improvement concerning depression, but no significant improvement in mental health was found in the pain group taken as a whole.</p><p><strong>Conclusion: </strong>Physiotherapy was effective in reducing pain symptoms in music students affected by chronic musculoskeletal pain. However, physiotherapy did not improve mental health in pain-affected music students. {"title":"Relationship of Musculoskeletal Symptoms and Musical Practice Intensity among Musicians During the COVID-19 Pandemic: A Descriptive Questionnaire Study.","authors":"Jenyffer Rodrigues Ferreira,&nbsp;Helen Cristina N Carrer,&nbsp;Marina M Cid,&nbsp;Ana B Oliveira","doi":"10.21091/mppa.2022.4029","DOIUrl":"https://doi.org/10.21091/mppa.2022.4029","url":null,"abstract":"<p><strong>Introduction: </strong>As soon as COVID-19 spread around the world, prevention and control measures were taken, such as masking and physical distancing, which changed people's routines, including musical practice among professional and amateur musicians.</p><p><strong>Objective: </strong>This descriptive questionnaire study was designed to: 1) investigate how the pandemic affected musical practice among musicians; 2) determine how musicians remained motivated in their new routine; 3) identify the presence of pre-COVID musculoskeletal symptoms; and 4) analyze whether musculoskeletal symptoms decreased, maintained, or increased during the COVID-19 pandemic.</p><p><strong>Methods: </strong>A total of 89 musicians from Brazil participated in an online survey, from August 2020 to January 2021. The survey form included questions about motivation, musical practice frequency, and the presence and characterization of musculoskeletal symptoms (adapted from the Nordic Musculoskeletal Questionnaire, NMQ). Musicians were asked to consider the periods before and during the pandemic to answer the questions.</p><p><strong>Results: </strong>During the pandemic, the frequency of musical practice decreased (p<0.01), and it could be associated with the individual's perceptions of motivation. Musculoskeletal symptoms in the previous 12 months were reported by 58%. When comparing the period before the pandemic to the present, 35.3% of musicians reported their musculoskeletal symptoms had not changed, while 33.3% reported that they had increased. Wrists and hands, lower and upper back, shoulders, and neck were the body sites with most frequent complaints.</p><p><strong>Conclusion: </strong>The COVID-19 pandemic had a negative impact on the frequency of musical practice of the musicians assessed in this study, with a significant decrease in practice time. As a result, artists may not have access to non-publicly funded health services that are essential to their well-being. A health centre in Canada that specializes in providing healthcare to artists offers eligible artists subsidized health services, with the aim to treat acute health issues that impact an artists' ability to engage in their artistic practice.</p><p><strong>Purpose: </strong>We evaluated the use of the subsidized health services and explored the subsidy recipients' and the selection committee's perspectives on the impact of these services on the health of performing artists.</p><p><strong>Methods: </strong>We applied an interpretive descriptive approach to our qualitative inquiry. We conducted individual, semi-structured interviews with recipients of the subsidy and a focus group with the selection committee that selected recipients of the subsidy. Data were analyzed using thematic analysis.</p><p><strong>Results: </strong>A total of 14 artists and all members of the selection committee (n=3) participated. Recipients and selection committee perceived that subsidized health services were critical in enabling consistent and timely diagnosis and treatment. Several themes emerged from the data: 1) need for universal health benefits to restore equity and offset healthcare insecurity, 2) the critical role of subsidies in accessing health services, 3) risks of abruptly ending health services when subsidy runs out, 4) barriers in applying for and accessing subsidies, 5) mental health challenges, and 6) importance of the subsidy in the context of the COVID-19 pandemic. All recipients saw noticeable improvement in health outcomes that they believed would have been otherwise unattainable if they did not have timely access to care.</p><p><strong>Conclusions: </strong>Subsidized health services play an important role in ensuring that performing artists have access to care for injuries and health conditions that are related to their profession.
