Was ist eine Polyneuropathie? Unter einer Polyneuropathie versteht man eine Nervenerkrankung, bei der in der Regel viele Nerven gleichzeitig betroffen sind. (Ausnahmen bilden sogenannte Mononeuropathien diese sind selten und geben immer besonderen Anlaß eine andere Erkrankung auszuschließen) Bei den Nerven, die dabei gemeint sind, handelt es sich um Nervensträngeund fasern, die für die Wahrnehmung und Leitung von Empfindungen ( wie Berührungsreize, Wärme, Kälte, Schmerz) unserer Haut und unseren Organen zum Gehirn zuständig sind. Und um die Nerven, über die unser Gehirn den Muskeln vermittelt und wie stark sie sich bei Bewegungen zusammenziehen müssen. Bei diesen Nerven handelt es sich um sichtbare und teilweise auch tastbare Leitungsbahnen, die mit Stromkabeln durchaus vergleichbar sind. An manchen körpernahen Stellen sind diese \"Kabel\" fingerdick, in ihren Endaufzweigungen sind sie haardünn. Ähnlich wie Stromkabel bestehen sie aus Leitungsbahn und Isolierung,wobei der innere Aufbau etwas komplizierter ist als bei einem Stromkabel. {"title":"Endoskopische Antirefluxtherapien - was bleibt? - Transmurale Doppelnaht scheint am effektivsten","authors":"Transmurale Doppelnaht, scheint am effektivsten","doi":"10.1055/S-2007-970169","DOIUrl":"https://doi.org/10.1055/S-2007-970169","url":null,"abstract":"Transmural double suture seems to be most effective – Endoscopic antireflux procedures: what remains? A couple of endoscopic antireflux procedures have been developed during the last years. Besides endoscopic suturing these techniques include injection or implantation of biopolymers and application of radio-frequency. Radiofrequency has proven only a very modest effect, while implantation techniques have been abandoned due to lack of long-term efficacy or serious side effects. While first generation endoluminal suturing techniques demonstrated a proof of principle, their lack of durability, due to suture loss, led to the development of a potentially durable transmural plication technique. In a prospective-randomized, sham-controlled trial the Plicator procedure proved superiority concerning reflux symptoms, medication use and esophageal acid exposure (24-hour pH-metry).
