Milled precision bar with the lateral additions of two\u0000 precision attachments could significantly contribute to the primary and\u0000 secondary retention of partial denture and overdenture. This clinical case\u0000 demonstrates stability of mandibular overdenture using milled precision bar\u0000 and two coronally positioned precision attachments. Case report. A\u0000 sixty-five years old male patient had two preserved canine roots in lower\u0000 jaw. Semicircular metal-ceramic (PFM) bridge was previously fabricated for\u0000 upper teeth. Decision for fabrication of milled bar and metal framework of\u0000 overdenture was established due to the presence of PFM bridge in the upper\u0000 jaw. The root canals of the two remaining mandibular canines were prepared\u0000 for posts. After final mandibular impression using A-silicone in a custom\u0000 tray, master cast with prepared canals of roots of canines was provided. Two\u0000 posts connected with milled precision bar were designed. Coronal\u0000 attachments-patrixs were positioned onto the tip of each coronal part of the\u0000 construction with axial direction of precision attachments towards root\u0000 canals. The construction was invested and casted. Metal framework of\u0000 overdenture was designed and casted. After interocclusal record, the\u0000 artificial anatomic teeth were selected and waxed model of overdenture with\u0000 matrixes was provided for try-in clinical phase. Finally, the overdenture\u0000 was flasked and polished. Metal construction of two posts and milled bar\u0000 were cemented simultaneously with overdenture, using self-adhesive resin\u0000 cement. Conclusion. Milled bar improves retention and stability of metal\u0000 framework of overdenture. Coronally positioned patrix of attachment provides\u0000 redirection of occlusal and other functional forces towards apical root\u0000 parts.

Periodontal disease is a chronic inflammatory disease caused by\u0000 pathologic microorganisms/periopathogens from oral biofilm. Standard\u0000 periodontal therapy consists of scaling and root planing (SRP). Probiotics\u0000 can be used as an adjunctive to standard periodontal therapy, since it is\u0000 known that probiotics can modify pathogenic potential of biofilm by\u0000 suppressing the colonization of periopathogens. The aim of this study was to\u0000 assess the clinical effect of Bifidobacterium and Lactobacillus probiotic\u0000 lozenges, probiotic mouthwash, as an adjuvant to SRP in the treatment of\u0000 initial to moderate chronic periodontitis. Material and methods. Thirty\u0000 patients with initial to moderate chronic periodontitis were recruited and\u0000 monitored clinically at baseline (before SRP) and 60 days following SRP. All\u0000 patients were randomly assigned to experimental group: SRP + probiotic (n =\u0000 15) and control group: SRP only (n = 15). The probiotic mouthwash was used\u0000 twice a day for 60 days. Clinical parameters: the probing pocket depth\u0000 (PPD), clinical attachment level (CAL) and bleeding on probing (BOP) were\u0000 measured at baseline and 60th day following SRP. Data were statistically\u0000 analyzed using the one-way Anova test and SPSS 19 software (IMB Company, New\u0000 York, U.S.). The Friedman and Mann Whitney tests were used as a post hoc\u0000 test for intergroup analysis. Statistical significance was set at p < 0.05.\u0000 Results. After 60 days of treatment, the clinical parameters PPD, CAL and\u0000 BOP were significantly lower in both groups compared to the baseline. In the\u0000 experimental group, the clinical parameters PPD, CAL and BOP were\u0000 significantly reduced after 60 days of treatment compared to the initial\u0000 measurements (p < 0.05). In the control group, statistically significant\u0000 decrease after 60 days of treatment was recorded only for BOP parameter,\u0000 while there was no statistically significant decrease of PPD and CAL values\u0000 (p > 0.05).
